Neurofibroma of the maxillary antrum: A rare case.

Neurofibromas are benign tumors of peripheral nerve tissue, frequently associated with neurofibromatosis type 1. Their isolated occurrence in the maxillary antrum is rare, with only 6 cases described in the English literature to the best of our knowledge. Primary neurogenic tumors in the maxillary sinus are unusual entities. The majority of the reported cases that have dealt with neurilemmomas and isolated neurofibromas are extremely rare. Here, a case of neurofibroma of the maxillary sinus. We present the case of a 60-year-old female patient with the chief complain of growth in the upper righ

Computed tomography evaluation of the morphometry and variations of the infraorbital canal relating to endoscopic surgery.

INTRODUCTION: The course of the infraorbital canal may leave the infraorbital nerve susceptible to injury during reconstructive and endoscopic surgery, particularly when surgically manipulating the roof of the maxillary sinus. OBJECTIVE: We investigated both the morphometry and variations of the infraorbital canal with the aim to show the relationship between them relative to endoscopic approaches. METHODS: This retrospective study was performed on paranasal multidetector computed tomography images of 200 patients. RESULTS: The infraorbital canal corpus types were categorized as Type 1: wit

An unusual complication of condylectomy: fracture of the temporal bone and intratemporal facial paralysis.

The incidence of temporomandibular joint (TMJ) ankylosis appears to be decreasing with the increased socioeconomic status of society. The intraoperative complications were reported to be few, the most dangerous of which are facial nerve injury during the extended preauricular incision and maxillary artery injury during condylectomy. The authors report a case of fracture of the temporal bone tearing the intratemporal portion of the facial nerve, resulting in a total facial nerve paralysis.

The blood supply of the intracavernous cranial nerves: an anatomic study.

Cranial nerve deficits are the most common complications of cavernous sinus surgery. Often the deficit occurs despite anatomic preservation of the nerve, and ischemic injury is thought to be the cause. A better understanding of the blood supply of these nerves may help to prevent such complications. The authors performed a cadaveric microsurgical study of the intracavernous cranial nerves and their blood supply in 20 cavernous sinuses. The oculomotor nerve received branches from the inferolateral trunk or its equivalent in all specimens (100%). The proximal trochlear nerve received branches fr

Maxillary neurilemmoma-Rarest of the rare tumour: Report of 2 cases.

INTRODUCTION: Intraoral neurilemmomas (schwannoma) are rare, even rarer are intraosseous ones, and the rarest are the maxillary neurilemmomas. Going by the literature only 5 cases of maxillary neurilemomas are reported till now. Neurilemmomas are benign tumours of nerve sheath origin. Approximately 30% arise in head and neck region, of these 1-12% arise intraorally mainly involving tongue. PRESENTATION OF CASE: Here we report two cases of maxillary neurilemmoma, one in a 9 year old girl and second one in a 27 year old female, both involving the lateral surface of maxilla on right side. Both t

Innervation of the rat pineal gland by PACAP-immunoreactive nerve fibers originating in the trigeminal ganglion: a degeneration study.

In order to establish that the pineal gland is innervated by pituitary adenylate cyclase-activating polypeptide (PACAP)-immunoreactive nerve fibers originating in the trigeminal ganglion, ophthalmic and maxillary nerves were transected by using a subtemporal fossa approach. The number of PACAP-immunoreactive nerve fibers in the pineal gland of rats with a total transection of the nerve was compared with that of rats without surgery. In the operated rat, PACAP-immunoreactive nerve fibers in the superficial pineal decreased remarkably, indicating that the trigeminal ganglion was the origin of th…

Medial maxillary fractures revisited.

BACKGROUND: We consider medial maxillary fractures to be a unique type of nasomaxillary buttress fracture involving the lateral margin of th … wed by small object impact. All of the patients exhibited epistaxis and hypoaesthesia affecting the maxillary nerve and/or its branches, and some patients also displayed nasal deformities and/or diplopia. An oral approach was employed in every case, and it was combined with a subciliary approach in some cases. Medial maxillary fractures involve the maxilla, lacri …

Conjunctival pain at termination of cataract surgery.

An explanation is proposed for the conjunctival pain at the conclusion of cataract surgery. Pain associated with injection of antibiotics under the conjunctiva is sometimes observed at the end of cataract surgery performed under retrobulbar block. Partial block of the sensory nerves of the eye has been proposed as a reason for this. Failure to block orbital-ciliary nerves, branches of the maxillary branch of the fifth cranial nerve is put forward as an additional explanation for the pain.

Endoscopic Infraorbital Microdissection for Localized V2 Trigeminal Neuralgia.

Trigeminal neuralgia is a chronic and debilitating syndrome characterized by short paroxysms of lancinating facial pain. Patients may be medically managed; however, in cases of medically refractory trigeminal neuralgia, surgical management is often required. Our objective was to present and describe a technique for endoscopic microdissection of the infraorbital nerve, a peripheral method of management for refractory V2 trigeminal neuralgia in patients without evidence of neurovascular compression. The technique is designed to spare sensation in unaffected portions of the V2 distribution. We pr

[Malignant peripheral nerve sheath tumour of the infra-orbital nerve].

A malignant peripheral nerve sheath tumour (MPNST) is an uncommon neoplasm that rarely involves the head and neck region. It is even more infrequent for these tumours to affect cranial nerves. We report the case of a 53-year-old man who presented a MPNST involving the infra-orbital nerve, which extended through the orbit and the base of the skull, progressing intracranially. Histological studies identified the tumour as an MPNST. Response to radiotherapy was not complete and radical surgical resection was impossible, so the patient died 10months later. This rare case of MPNST with intracranial

The Hidden Trap: Tolosa-Hunt Syndrome Masking Entrapped Great Occipital Nerve-A Case of Headache Surgery.

Headache syndromes may coexist with peripheral nerve entrapment, which can obscure the true pain generator and delay effective treatment. Tolosa-Hunt syndrome is a rare, painful ophthalmoplegia syndrome characterized by unilateral orbital pain and cranial nerve palsy, often showing a marked response to corticosteroids. We report the case of a 56-year-old woman with a long history of migraine who developed recurrent right-sided orbital pain, maxillary pain, and ophthalmoplegia. She was diagnosed with Tolosa-Hunt syndrome by an experienced neurologist after skull magnetic resonance imaging and c

Transient cranial neuropathies as sequelae of Onyx embolization of arteriovenous shunt lesions near the skull base: possible axonotmetic traction injuries.

INTRODUCTION: Transarterial embolization with Onyx is a relatively safe and increasingly common method of treating cranial dural arteriovenous fistulas (DAVF) and arteriovenous malformations (AVM). Cranial neuropathy resulting from this procedure has been reported. CASE REPORTS: Three case histories are presented in two patients treated for a DAVF and an AVM near the skull base with heavily parasitized supply from external carotid artery branches. Transarterial embolization resulted in transient cranial neuropathies including two cases of lower facial nerve palsy and one case of trigeminal ne

Orofacial neuropathic pain mouse model induced by Trigeminal Inflammatory Compression (TIC) of the infraorbital nerve.

BACKGROUND: Trigeminal neuropathic pain attacks can be excruciating for patients, even after being lightly touched. Although there are rodent trigeminal nerve research models to study orofacial pain, few models have been applied to studies in mice. A mouse trigeminal inflammatory compression (TIC) model is introduced here which successfully and reliably promotes vibrissal whisker pad hypersensitivity. RESULTS: The chronic orofacial neuropathic pain model is induced after surgical placement of chromic gut suture in the infraorbital nerve fissure in the maxillary bone. Slight compression and ch

Head and Neck Schwannomas: A Surgical Challenge-A Series of 5 Cases.

BACKGROUND: Schwannomas, also known as neurilemmomas, are benign peripheral nerve sheath tumors. They originate from any nerve covered with schwann cell sheath. Schwannomas constitute 25-45% of tumors of the head and neck. About 4% of head and neck schwannomas present as a sinonasal schwannoma. Brachial plexus schwannoma constitute only about 5% of schwannomas. Cervical vagal schwannomas constitute about 2-5% of neurogenic tumors. METHODS: We present a case series of 5 patients of schwannomas, one arising from the maxillary branch of trigeminal nerve in the maxillary sinus, second arising fro

Endoscopic ligation of the sphenopalatine artery and the maxillary artery for the treatment of intractable posterior epistaxis.

BACKGROUND: Intractable posterior epistaxis sometimes requires intensive treatment, such as surgery or embolization. Maxillary artery ligation has been widely used for the treatment of intractable posterior epistaxis. It is highly effective, but significant complications may occur, including an oroantral fistula and damage to the infraorbital nerve. Embolization is less invasive and can be performed in poor surgical candidates. However, it has more serious complications, such as facial nerve paralysis and hemiplegia. This investigation evaluates the effectiveness and complications of endoscopi
