Rapid Anterior Segmental Maxillary Retraction by Compression Osteogenesis.

Maxillary protrusion is one of the deformities of the upper jaw. Orthodontics and orthodontics combined with anterior segmental osteotomy are the common treatment strategies for this deformity. The hypothesis of this study was that the maxillary bone segment could be gradually moved backward safely by using a tooth-borne-specific compression device that causes bone compression at the bony segment interface after performance of surgical bone cuts in cases of anterior maxillary protrusion. Ten patients with skeletal maxillary protrusion were treated by modified subapical maxillary osteotomy. The

The Assessment of External Nasal Valve Efficiency After Le Fort I Surgery.

BACKGROUND: The present study investigated how external nasal valve function is affected after Le Fort I osteotomy using external nasal valve efficiency (ENVE) index. MATERIALS AND METHODS: Twenty-one patients who underwent Le Fort I osteotomy with or without mandibular osteotomy were included in the retrospective study. Preoperative and postoperative standardized videos of basal view of the nose during breathing were recorded, and 2 screenshots were taken, 1 in the resting position and the other after deep inspiration. External nasal valve efficiency was calculated at preoperative and postop

Vital staining of palatal soft tissue in horseshoe Le Fort I osteotomy for superior repositioning of the maxilla.

In maxillary orthognathic surgery, superior repositioning of the maxilla is sometimes difficult, and removal of the bony interference, especially around the descending palatine artery, is very time-consuming in cases of severe maxillary impaction. A useful method introduced for superior repositioning of the maxilla is horseshoe-shaped osteotomy combined with Le Fort I osteotomy (horseshoe Le Fort I osteotomy). However, injury to the palatal soft tissue during horseshoe-shaped osteotomy may cause aseptic complications of the maxilla. Therefore, a safe method is required to prevent such injury t

Does Le Fort I osteotomy have an influence on nasal cavity and septum deviation?

AIMS: Le Fort I (LI) osteotomy has been used for the correction of dento-facial deformities of the midface. The aim of this study was to determine the effects of advancement and impaction of the maxilla with LI osteotomy on the nasal cavity and septum. PATIENTS AND METHODS: In this study, 40 adult patients, 23 females and 17 males (mean age 20.52 ± 4.4 years), who underwent single-piece LI advancement and impaction surgery combined with a bilateral sagittal split osteotomy (BSSO) were included. Posterior-anterior (PA) and lateral cephalometric radiographs taken before surgery (T0) and at leas

[Non-union of the maxilla: a rare complication after Le Fort I osteotomy].

INTRODUCTION: Maxillary non-union following Le Fort I osteotomy is a rare complication in orthognathic surgery. We report our experience and analyze the possible etiological factors. PATIENTS AND METHOD: We retrospectively analyzed the files of 150 patients having undergone Le Fort I osteotomy, between 1996 and 2006, screening for maxillary non-union. We documented patients' features of: sex, age, medical history, indication, orthodontics, osteotomy type(s) and displacement(s), osteosynthesis, quality of dental occlusion, orthodontics, clinical signs supporting a diagnosis of non-union, radio

Personal Guide Assisted Orthognathic Surgery: Application of a T-shaped Tooth Bone Combined Supporting Osteotomy Guide Plate and Positioning Guide Plate.

The precise movement of the maxilla is particularly important for orthognathic surgery, especially for patients with maxillary segmentation. In this preliminary study, the authors present a new tooth bone combined with a supporting osteotomy guide and positioning guide to guide the osteotomy and reduction of the maxilla. Through our preoperative simulation and postoperative image fusion, the authors found that the overlapping area is more than 90%. According to compare of the virtual plans and the postoperative results based on distances from the maxillary land- marks to the horizontal plane,

Evaluation of stress by finite element analysis of the midface and skull base at the time of midpalatal osteotomy in models with or without pterygomaxillary dysjunction.

Surgically-assisted rapid maxillary expansion (SARME) is commonly used to treat skeletally mature patients with transverse discrepancies. Some osteotomies are made in areas that resist expansion, but there is no clear consensus about the sequence in which the osteotomies are made. Some clinicians do the pterygomaxillary osteotomy last, while others do it before the midpalatal osteotomy. We used the finite element method to measure the stresses on the midface, cranial base and pterygoid plates at the time of midpalatal osteotomy in two models, one with and one without pterygomaxillary dysjuncti

A Digitally Planned "Dental-Implant-First" Protocol for Surgical Advancement and Prosthetic Restoration of Edentulous Maxilla With Skeletal Class III Deficiency: A Surgical Innovation.

In edentulous patients with skeletal Class III maxillary deficiency, Le Fort I osteotomy followed by delayed implant placement faces maxillary segment instability during healing. This article describes a reversed dental-implant-first protocol that alters the traditional sequence by placing implants prior to osteotomy within a 24-hour interval. On day 1, 4 implants were placed with a minimum insertion torque of 35 Ncm, maintaining a 3.0 mm safety distance from the planned osteotomy plane. On day 2, Le Fort I osteotomy was performed. Nasal profile changes after maxillary impaction and advancement surgery.

PURPOSE: The profile changes in the nasal tip and columello-labial region that occur after maxillary advancement and impaction surgery were measured to test the hypothesis that a subspinal osteotomy would induce less nasal tip change and would result in a more acute columello-labial angle than a conventional Le Fort I-type osteotomy. PATIENTS AND METHODS: The lateral cephalograms of 2 matched groups of 23 advancement/impaction cases with and without subspinal osteotomy were analyzed electronically. RESULTS: There was no difference in nasal tip elevation and change in nasal tip projection bet

[Horseshoe Le Fort I osteotomy].

INTRODUCTION: Performing Le Fort I impaction osteotomy can be compromised or impossible. The intranasal volume of the lower turbinates prevents an important maxillary impaction. In this case, horseshoe osteotomy is an alternative. SURGICAL TECHNIQUE: The first step is to lower the inferior edge of the pyriform aperture and the anterior floor of the nasal fossae with a bur. The palatine alveolar wall is cut through resection of the maxillary sinus anterior and lateral walls. The maxillary dental arch may be impacted without freeing the vomer or cutting through lateral walls of the nasal fossa,

Study of condylar positional changes after sagittal split osteotomy for mandibular advancement: A prospective cohort study.

PURPOSE: To analyse the possible morphologic and positional changes of the mandibular condyles after orthognathic surgery. MATERIAL AND METHODS: A prospective cohort study was performed. Patients with mandibular retrognathism were surgically treated to advance the mandible. The study group included seventeen patients (34 condyles) treated with sagittal split osteotomies alone (4 patients) or in combination with maxillary osteotomies (13 patients). Only condyles located on the mandibular side that advance during surgery were studied, therefore only 25 condyles entered this prospective study. B

The surgical correction of vertical maxillary excess (long face syndrome).

The surgical correction of vertical maxillary excess is a relatively new technique. Vertical maxillary excess (VME) may exist alone or in combination with a horizontal mandibular deficiency with or without an anterior open bite. The facial contour is characterized by a long, tapering face with anterior and posterior maxillary overgrowth, a narrow alar base, and lip in competence. Cephalometric analysis demonstrates steep mandibular and occlusal planes in relationship to the cranial base, and increase in facial height, and retroposition of the mandible. Evaluation of study models exhibits incre

[A method for the surgical treatment of maxillary micro- and retrognathism].

The proposed method of surgical treatment of maxillary micro- and retrognathia radically differs from the known methods: osteotomy of the osseous base of the hard palate is carried out parallel to the vomer to reach the level of the first premolars by a median dissection of the mucosa and periosteum, and vertical osteotomy of the anterior portion of the vomer is carried out. Horizontal osteotomy is performed from the vestibule of the mouth. Subsequent osteotomy of the anterior and lateral surfaces of the upper jaws under mucosal periosteal "tunnels" and detachment of the maxillary tuberosities

[Embolization of Arteriovenous Fistulae of the Maxillary Artery after Le Fort I Osteotomy:A Case Report].

BACKGROUND: Le Fort I osteotomy, one of the most common techniques applied to correct jaw deformities, is generally considered operatively safe. However, a few articles reported that this procedure can lead to formation of arteriovenous fistulae(AVF)involving the maxillary artery infrequently. CASE REPORT: A man in his 40s with a jaw deformity underwent Le Fort I osteotomy. Two days later, he noticed pulsating tinnitus in his right ear. Angiography revealed AVF between the proximal part of the maxillary artery(first segment)and the pterygoid plexus. On further evaluation, AVF were embolized w

[Research progress in the treatment of maxillary transverse deficiency with combined orthodontic-orthognathic approach].

In combined orthodontic-orthognathic treatment, the maxillary palatine suture is closed in most patients with insufficient maxillary width, and bony expansion of the maxilla cannot be achieved by dental expansion or rapid palatal expansion (RPE) which causes buccal inclination of the maxillary posterior teeth leading to unstable results. Therefore, segmental LeFort Ⅰ osteotomy and surgically assisted RPE are often used in clinical practice. In recent years, with the application of implant anchorage technology, implant anchorage assisted RPE has been gradually applied in orthognathic treatment.
