Microbiology of subperiosteal orbital abscess and associated maxillary sinusitis.

Aspirate of pus from eight subperiosteal orbital abscesses (SPOAs) and their corresponding infected sinuses were studied for aerobic and anaerobic bacteria. Polymicrobial flora was found in all instances, and the number of isolates varied from two to five. Anaerobes were recovered from all specimens. The predominant isolates were Peptostreptococcus spp, Prevotella spp, Fusobacterium spp, Staphylococcus aureus, and microaerophilic streptococci. Concordance in the microbiological findings between SPOA and the infected sinus was found in all instances. However, certain organisms were only present

[Clinical criteria for acute maxillary sinusitis in general practice].

The purpose of the investigation was to examine the diagnostic value of various symptoms, clinical findings, erythrocyte sedimentation rate (ESR) and serum C-reactive protein (CRP) in acute maxillary sinusitis using the aspiration of purulent or mucopurulent from the sinus of a patient with a pathological CT-scanning as the golden standard. Of 174 patients with suspected acute maxillary sinusitis seen in general practice we found 70% to have a pathological CT-scanning and 53% to have pus or mucous pus in the sinus aspirate. Only the ESR and CRP level were found to be significantly and independ…

Acute maxillary sinusitis in general practice: a decision problem.

OBJECTIVE: This paper describes a qualitative analysis of a decision problem of acute maxillary sinusitis in general practice. The criteria and expected outcomes on which general practitioners (GPs) base their choice of a management strategy are presented. DESIGN: Structured open-ended interviews, all done by the first author, were transcribed, and summarized for each management strategy. These summaries were sent back to the experts for verification. PARTICIPANTS: Eight expert GPs from The Netherlands participated; all had been practising for at least three years, and had conducted postgrad…

Occipito-mental side projection radiography in childhood maxillary sinusitis.

152 sinuses of children with acute maxillary sinusitis were roentgenographically examined with both a standard occipito-mental (OM) and an occipito-mental side (OMS) projection. In the acute phase of the disease the OMS projection was superior in 13% of sinuses in demonstrating the presence of retained maxillary secretion. When the result of medical treatment was controlled after two weeks the superiority was seen only in 1% of sinuses due to the major difference between the subgroups' 'mucosal thickening'. The clinical importance of the OM finding 'mucosal thickening' seems to be different in…

Gallium-67 scanning in the differentiation of maxillary sinus carcinoma from chronic maxillary sinusitis.

Gallium-67 scans of 25 patients in whom the clinical symptoms and radiographic findings were suggestive of either maxillary sinus carcinoma or chronic sinusitis proved to be valuable in the differentiation between the two disease processes. Those patients with carcinoma had positive scans, while those with sinusitis had either negative or only weakly positive scans.

Penetration of erythromycin stearate into maxillary sinus mucosa and secretion in chronic maxillary sinusitis.

The penetration of oral erythromycin stearate (Abboticin), administered in a dosage of 500 mg three times a day, into the maxillary sinus mucosa and secretion was studied in 15 patients (22 sinuses) operated on for chronic maxillary sinusitis. The average concentration in serum was 2.3 microgram/ml, 1.2 microgram/ml in secretion, and 1.8 microgram/ml in mucosa. These concentrations are highly effective against diplococci and most aerobic and anaerobic streptococci (MIC value 0.06 microgram/ml) but not against Haemophilus influenzae (MIC value for 80% of 2 microgram/ml).

Is maxillary sinusitis and radiographic maxillary sinus opacification associated with an altered microbiology of MRONJ?

OBJECTIVE: Maxillary sinusitis can be a sequela of medication-related osteonecrosis of the jaw (MRONJ). This study aims to characterize the microbiome of maxillary MRONJ with concurrent maxillary sinusitis and radiographic maxillary sinus opacification to determine if there is a relationship between the microbiome of MRONJ and sinus disease. STUDY DESIGN: This retrospective case series was conducted using electronic health records from the University of Pennsylvania and affiliated hospitals. The target population was surgically managed maxillary MRONJ patients. The primary predictor variables…

Maxillary sinusitis caused by dental implants extending into the maxillary sinus and the nasal cavities.

This report describes the case of a patient who underwent osseointegrated dental implant placement. The implants were misplaced inside the nasal fossae and in the right maxillary sinus, causing chronic purulent sinusitis. CT scan without contrast showed signs of right maxillary sinusitis and confirmed the misplacement of four dental implants that surfaced into the nasal cavities. The imaging also revealed the presence of another implant that emerged inside the maxillary sinus. The patient underwent functional endoscopic sinus surgery with complete symptom remission at the long-term follow-up. …

[The value of maxillary sinus irrigation in children with maxillary sinusitis using the Waters film].

Eighty children with chronic sinusitis receiving a conservative treatment were followed. They found that without any further therapy the Waters view of 30 children became normal. The usefulness of irrigation of the maxillary sinus was studied in the other 50 children. In one group of 25 children a sinus endoscopy and sinus irrigation was performed. In a control group of another 25 children no further treatment was performed. A comparison between the Waters view and sinus endoscopy is also included in the study. The authors found little difference in the treatment results in the two groups. Fur

Bacteriological evaluation of 194 adult patients with acute frontal sinusitis and findings of simultaneous maxillary sinusitis.

In a prospective study bacteriological findings from frontal and maxillary sinus secretes were analyzed. Frontal sinus trephination was performed to 103 and maxillary irrigation to 192 patients. Main pathogens in the frontal as well as in the maxillary sinus secretes were Haemophilus influenzae and Streptococcus pneumoniae. beta-lactamase production of H. influenzae was found in 17% of frontal and in 16% of maxillary sinus samples. Bacteriological findings were supported by the staining results. The portion of anaerobic pathogens in cultures were poor, though Bacteroides fragilis antigen was d

Management of odontogenic maxillary sinusitis with endodontic microsurgery in maxillary posterior teeth: a case series.

BACKGROUND: Odontogenic maxillary sinusitis (OMS) can develop from apical periodontitis (AP) of maxillary posterior teeth, known as maxillary sinusitis of endodontic origin(MSEO). The treatment of OMS remains controversial. Endodontic microsurgery (EMS) plays a crucial role for treatment of AP when root canal treatment and retreatment fails. With multidisciplinary management, EMS alnoe may be effective for MSEO . CASE PRESENTATION: Ten cases of MSEO undergoing EMS are reported at the First Affiliated Hospital of Wenzhou Medical University between 2022 and 2023. Clinical and radio-graphic eval…
