The past few years have seen the advent of several new antifungal agents. The echinocandin, caspofungin, has greatly expanded the antifungal armamentarium by providing a cell wall-active agent with candidacidal activity as well as demonstrated clinical efficacy in the therapy of aspergillosis refractory to available therapy. PURPOSE OF REVIEW: Invasive fungal infections remain a leading cause of infectious morbidity and mortality in heavily immunosuppressed patients. As the efficacy of current antifungal agents is suboptimal, combinations of antifungal agents, administered concomitantly or sequentially, are increasingly used (mostly empirically) as a strategy to improve the treatment outcomes for refractory mycoses. Here we review recent developments in the area of antifungal combinations based on both laboratory (in vitro and in animal models) and clinical studies.

Histoplasmosis, blastomycosis, and coccidioidomycosis are uncommon, but often life-threatening infections in patients who have hematologic malignancies. Disease occurs either as a result of new infection after an environmental exposure or as a result of reactivation of a remote infection. The authors surveyed the present situation for imported (introduced) mycoses from papers published in Japan as of the end of October, 1998. There were 16 cases of coccidioidomycosis, 22 of histoplasmosis, 13 of paracoccidioidomycosis and one of penicilliosis marneffei. In a 2-year period, 30 patients with culture-proven mycoses (chromoblastomycosis, sporotrichosis, and paracoccidioidomycosis) were treated with the new orally administered triazole, saperconazole (SPZ) (R66905). The daily dose varied from 100 to 200 mg. Systemic mycoses were found in 19 (4.7%) of 406 calves less than 6 months old which were autopsied during the past 10 years. Alimentary mycosis occurred in 12 (63.2%) of 19 cases. Fluconazole is a recently licensed antifungal agent that has gained widespread use in the medical community. Despite a lack of controlled trials in invasive fungal infections, this agent is often prescribed because of ease of administration and concern over amphotericin B toxicity. Three cases of systemic fungal infections in which fluconazole use resulted in unambiguous microbiologic and clinical failure are reported.

Striking results were obtained with oral itraconazole therapy in two opportunistic mycoses. Of 28 patients with cryptococcal meningitis, 18 achieved complete responses, including 16 of 24 patients with acquired immunodeficiency syndrome. An 11 year old boy with medullary aplasia developed extensive mycosis of the face. This mucormycosis is a rare affection that appears in patients with predisposing disease (diabetes, blood disorders) and is easily diagnosed, if considered, from clinical features. Diagnosis is confirmed by histopathology, culture sometimes identifying the causative fungus. Despite the intrinsic serious course of this affection (facial necrosis, death) prognosis appears to be related essentially to the rapid control of the subjacent disease.

Immunologic methods have been used with varying success in the diagnosis and prognosis of systemic mycoses in animals. Information on the application of the tests and on the interpretation of test results in monitoring therapy was complied. Serotesting provided diagnostic and prognostic data in immunocompetent patients, and predicted relapse in one case after apparent clinical cure. 4103 cases suspected of mycoses were analysed as to sex, age and site of disease and 3891 were proved cases. This group formed 50% of total mycoses or 13-93% of all dermatoses recorded in the Government General Hospital, Madras, during the period of study.
