From the experimental myocardial infarction to the clinical acute myocardial infarction: limitations of thrombolytic therapy.

Early administration of thrombolytic agents in acute myocardial infarction lowers mortality and preserves left ventricular function. Currently, only one third of infarct patients receive this treatment, the vast majority being excluded because of restrictive criteria and delayed hospital admission. When correctly administered, thrombolytic therapy achieves reperfusion in 50-85% of occluded vessels. Five to 15% of these initially recanalized vessels eventually reocclude. Possible mechanisms of failure of thrombolytic therapy to induce stable coronary reperfusion include thrombus formation durin

Prognosis of patients with cardiogenic shock following acute myocardial infarction: The difference between ST-segment elevation myocardial infarction and non-ST-segment elevation myocardial infarction.

Acute myocardial infarction (AMI) complicated by cardiogenic shock has high mortality and remains challenging even in the revascularization era. We conducted this study to understand patients' outcomes. We retrospectively analyzed electronic medical records data from 1175 patients with AMI complicated by cardiogenic shock that developed within 3 days of admission to a multicenter medical care system between January 1, 2000, and July 31, 2018. Patients with AMI were classified into the ST-segment elevation MI (STEMI) group or the non-ST-segment elevation MI (NSTEMI) group. The short-term and 1-

Incidence and clinical impact of right bundle branch block in patients with acute myocardial infarction: ST elevation myocardial infarction versus non-ST elevation myocardial infarction.

BACKGROUND: Both left bundle branch block and right bundle branch block (RBBB) have been associated with increased inhospital and long-term mortality in patients with acute ST elevation myocardial infarction (STEMI). However, the prognostic role of RBBB in acute non-ST elevation myocardial infarction (NSTEMI) is not well known. Therefore, the aim of the study was to evaluate the incidence and clinical impact of RBBB in patients with NSTEMI compared to patients with STEMI. METHODS: From the German prospective multicenter registry "Maximal Individual Therapy of Acute Myocardial Infarction" (MIT

Apelin-13 protects against myocardial infarction-induced myocardial fibrosis.

Myocardial infarction is a serious health threat. Apelin is an endogenous ligand of angiotensin II receptor-like 1 (APJ) and the apelin/APJ system is associated with various types of heart disease. However, whether apelin protects against myocardial infarction‑induced myocardial fibrosis remains unclear. The present study aimed to investigate the function of apelin‑13 during myocardial infarction‑induced myocardial fibrosis, and to determine the mechanism underlying the effects of apelin‑13. Apelin‑13 was demonstrated to improve left ventricular function and results of hematoxylin and eosin st

[Biographical analysis and myocardial infarct: a study of myocardial infarction patients and 2 reference groups].

In a retrospective case-referent study, 105 male myocardial infarction patients (age: 35-69), 105 hospital referents, and 105 neighbourhood referents, all of the same age, were interviewed with respect to exposure to some psychosocial risk indicators in their lives. These indicators include: Type A coronary-prone behavior pattern; the prevalence of stressful changes in childhood and adolescence, work and career, and family and social life; the syndrome of vital exhaustion and depression, which may precede myocardial infarction. The central question of this study is in what manner and in what c

Occlusion Myocardial Infarction Missed by ST-Elevation Myocardial Infarction (STEMI) Criteria: An Angiographic Emergency Department Cohort.

BACKGROUND: The ST-elevation myocardial infarction (STEMI)/non-STEMI (NSTEMI) paradigm uses ST-elevation as a surrogate for acute coronary occlusion, but ST-elevation criteria miss many occlusions. The occlusion myocardial infarction (OMI) vs. non-occlusion (NOMI) paradigm classifies patients by the presence of an acute occlusion. STUDY OBJECTIVES: To compare the 2 paradigms in an angiography-selected emergency department (ED) cohort of acute myocardial infarction. METHODS: Retrospective single-center cohort of consecutive adults given an ED diagnosis of STEMI or NSTEMI who underwent coronar

Association Between Subsequent Hospitalizations and Recurrent Acute Myocardial Infarction Within 1 Year After Acute Myocardial Infarction.

Background Patients who survive acute myocardial infarction (AMI) are at high risk for recurrence. We determined whether rehospitalizations after AMI further increased risk of recurrent AMI. Methods and Results The study included Medicare fee-for-service patients aged ≥65 years discharged alive after AMI from acute-care hospitals in fiscal years 2009-2014. The outcome was recurrent AMI within 1 year of the index AMI. The Clinical Classifications Software (CCS) was used to classify rehospitalizations into disease categories. A Cox regression model was fit accounting for CCS-specific hospitaliza

The thrombolysis in myocardial infarction risk score in unstable angina/non-ST-segment elevation myocardial infarction.

Risk stratification in unstable angina (UA)/non-ST-segment elevation myocardial infarction (NSTEMI) can provide an estimate of a patient's prognosis and optimize clinical choices. The Thrombolysis In Myocardial Infarction (TIMI) risk score for UA/NSTEMI is an integrated approach that uses baseline variables that are part of the routine medical evaluation to identify patients at high risk for death and other major cardiac ischemic events. Using multivariable logistic regression, seven independent predictor variables were identified: age > or = 65 years, > or = 3 risk factors for coronary artery

[Features of myocardial infarction in women].

Myocardial infarction is a major public health concern for women. Hospitalization rates are increasing, particularly among young women. This trend is probably linked to the growing prevalence of cardiovascular risk factors and specific conditions that can increase the risk of myocardial infraction, such as early menopause, estrogen-progestogen contraception, endometriosis and chest radiation therapy for breast cancer. From a patho-physiological perspective, women have distinct characteristics. They have a higher proportion of myocardial infraction without significant coronary obstruction, incl

Cancer and Myocardial Infarction in Women.

Women who present with myocardial infarction (MI) are more likely to be diagnosed with nonobstructive coronary arteries (MINOCAs), spontaneous coronary artery dissection (SCAD), and takotsubo syndrome (TS) than men. Malignancy may predispose to MI and TS through shared risk factors and inflammatory mediators. This study aimed to determine the prevalence of cancer in women presenting with clinical syndrome of MI and the association between cancer and mechanism of MI presentation. Among 520 women with MI who underwent coronary angiography at NYU Langone Health from March 2016 to March 2020 or Se

TRPA1 as Target in Myocardial Infarction.

Transient receptor potential cation channel subfamily A member 1 (TRPA1), an ion channel primarily expressed on sensory neurons, can be activated by substances occurring during myocardial infarction. Aims were to investigate whether activation, inhibition, or absence of TRPA1 affects infarcts and to explore underlying mechanisms. In the context of myocardial infarction, rats received a TRPA1 agonist, an antagonist, or vehicle at different time points, and infarct size was assessed. Wild type and TRPA1 knockout mice were also compared in this regard. In vitro, sensory neurons were co-cultured w

Colchicine and myocardial infarction: A review.

The inflammatory response is frequent after acute myocardial infarction, and may worsen ischaemia-reperfusion injuries, leading to increased infarct size and poor prognosis. Therefore, inflammation may be a promising therapeutic target, and anti-inflammatory drugs appear to be potential additional treatments in this context. Among these treatments, colchicine-a well-known drug that has been used for centuries in clinical practice for rheumatism-may represent the ideal candidate. Indeed, colchicine exerts direct anti-inflammatory and pleiotropic effects, with potential anti-arrhythmic, anti-fib

Meningococcemia Presenting as a Myocardial Infarction.

Neisseria meningitidis is an encapsulated gram negative diplococcus that colonizes the nasopharynx and is transmitted by aerosol or secretions with the majority of cases occurring in infants and adolescents. Meningococcemia carries a high mortality which is in part due to myocarditis. Early recognition and prompt use of antibiotics improve morbidity and mortality. We report a 55-year-old male presenting to the emergency department with chest pain, shortness of breath, and electrocardiogram changes suggestive of ST elevation MI who developed cardiogenic shock and multisystem organ failure from

[Complications of an unrecognized myocardial infarction].

Patients with an untreated myocardial infarction may present with serious late complications. 3 patients are described. A 63-year-old woman became progressively more short of breath 4 days after an acute episode of chest pain accompanied by nausea and sweating. It proved to be a cardiogenic shock following a rupture of a papillary muscle. A man aged 65 collapsed 5 days after an episode of back pain and nausea. This was a cardiac tamponade due to rupture of the left ventricle. A woman aged 74 had transient aphasia and during investigations for this was seen to have anomalies on ECG. She had cer

[Acute myocardial infarct in infants].

A retrospective study was made of 6 children, with nonsurgical-related acute myocardial infarction (AMI), between January 1987 and December 1994. The ratio for gender was 1 and mean age at AMI was 49 days, 4 cases being associated with congenital heart disease (Fallot's tetralogy, truncus arteriosus and DiGeorge syndrome, one case each, and anomalous origin of left coronary artery, 2 cases). Kawasaki disease and coronary embolisation from thrombosis of the renal vein occurred in the other 2 cases respectively. All developed congestive cardiac failure and cardiomegaly. The management of acute myocardial infarction.

Greater understanding of the underlying pathophysiology of acute myocardial infarction (AMI) has led to more aggressive management and lower mortality, both in-hospital and long term. AMI results mainly from thrombotic occlusion of the infarct-related coronary artery. The ensuing necrosis evolves over a 6-12 h period providing a time window for interventions designed to reduce eventual infarct size. The most appropriate interventions are those which restore coronary artery patency and hence myocardial blood flow as soon as possible. Occasionally, disruption of the occluding thrombus and compre

Hostility and myocardial infarction in men.

This case-control study examines the association of hostility (as measured by the Cook-Medley Hostility Scale) with myocardial infarction in adult males from The Netherlands. Subjects included patients with first myocardial infarction (MI; N = 81), who were compared with a neighbourhood control (NC) group (N = 168). Cases had somewhat higher scores on the Cook-Medley Hostility Scale, although the differences between cases and controls were not statistically significant. Multivariate analysis revealed the presence of an interaction between hostility and age, suggesting that the association of h

Aspirin and prevention of myocardial infarction.

Determining the cardioprotective effects of aspirin (acetylsalicylic acid) remains a focus for both basic science and clinical investigation. Although other contributors are probably present, the favorable prostacyclin-to-thromboxane ratio induced by low-dose aspirin appears beneficial for reducing cardiovascular mortality associated with unstable angina and myocardial infarction. The precise dosage, frequency and timing of aspirin's administration to reduce the incidence of vaso-occlusive events remains to be determined. This article reviews aspirin's mechanism of action and use for the preve
