Fracture management is becoming less conservative, and more frequently surgical intervention is the treatment of choice. We studied the cases of four patients who sustained a fresh femoral subtrochanteric fracture during late removal of a Zickel nail that had been inserted for treatment of a fracture. The configuration of the intramedullary device appeared to be a significant factor in the production of the complication. Comminution of the proximal part of the femur (sufficient to require supplementary fixation with interfragmentary screws) and relative youth of the patient may increase the incidence of the complication.

A 36-year-old man with long-standing alopecia areata was noted to have both leukonychia punctata and pitted nails. Histopathologic evaluation of the leukonychia punctata revealed multiple discrete parakeratotic foci throughout the entire thickness of the nail plate, suggesting both proximal and distal matrix involvement. The finding of shallow pits lined with residual adherent parakeratotic cells suggests that the pathogenesis of nail pits in alopecia areata is similar to that of psoriasis; a result of shedding of parakeratotic cell aggregates from the nail plate.

The nail unit is constructed by distinctly regulated components. The nail isthmus is a lately proposed region as a transitional zone between the most distal part of the nail bed and the hyponychium. It is difficult to recognize the nail isthmus in the normal nail, but it is easy to identify the region in nail disorders such as pterygium inversum unguis and ectopic nail. We describe structure and putative function of the nail isthmus via histopathologic features of pterygium inversum unguis and ectopic nail.

In circumstances where the nail complex is intact and undamaged it may be wiser to relocate the nail as a flap based on its vascular supply rather than resort to pulp replacement procedures such as a cross-finger flap or similar techniques which leave a hyposensitive pulp and a "parrot-beak" deformity of the nail. In "acute" finger injuries this procedure is decidedly preferable to amputation through the head of the middle phalanx.

Depending on the structure involved and the nature and duration of the lesion, a certain disturbance pattern presents. Matrix disorders, which are often observed, may manifest as such characteristic lesions as spots, tracheonychia or Beau's lines. Treatment is determined by the respective underlying disease.
