Pharmacogenetics of naltrexone and disulfiram in alcohol dependent, dually diagnosed veterans.

BACKGROUND: Disulfiram and naltrexone were evaluated in treatment of individuals with co-occurring alcohol dependence and other Axis I disorders (e.g., Major Depression). We explored pharmacogenetic interactions in genotyped subjects. METHODS: Alcohol dependent (AD) subjects received naltrexone alone, placebo alone, disulfiram with placebo or disulfiram with naltrexone. They were genotyped for OPRM1 rs1799971 (Asn40Asp), and DBH rs1611115 (C-1021T). N = 107 male European-American subjects were included. RESULTS: There were no significant interactions with OPRM1. DBH interacted with naltrexon…

Injectable and implantable sustained release naltrexone in the treatment of opioid addiction.

Sustained release technologies for administering the opioid antagonist naltrexone (SRX) have the potential to assist opioid-addicted patients in their efforts to maintain abstinence from heroin and other opioid agonists. Recently, reliable SRX formulations in intramuscular or implantable polymers that release naltrexone for 1-7 months have become available for clinical use and research. This qualitative review of the literature provides an overview of the technologies currently available for SRX and their effectiveness in reducing opioid use and other relevant outcomes. The majority of studies…

Challenges in the perioperative management of the patient receiving extended-release naltrexone.

Patients receiving extended-release (XR) naltrexone who are having surgery present unique challenges to anesthesia providers, the most obvious of which is an altered response to the effects of opioid agonists. Based on the timing of the last XR naltrexone dose, patients may be refractory to the effects of opioid agonists or potentially more sensitive to dangerous side effects due to receptor upregulation and hypersensitivity. Complicating matters, redosing XR naltrexone soon after opioid use may precipitate opioid withdrawal. We present a case of a 22-year-old woman receiving XR naltrexone for…

Cost-effectiveness of buprenorphine and naltrexone treatments for heroin dependence in Malaysia.

AIMS: To aid public health policymaking, we studied the cost-effectiveness of buprenorphine, naltrexone, and placebo interventions for heroin dependence in Malaysia. DESIGN: We estimated the cost-effectiveness ratios of three treatments for heroin dependence. We used a microcosting methodology to determine fixed, variable, and societal costs of each intervention. Cost data were collected from investigators, staff, and project records on the number and type of resources used and unit costs; societal costs for participants' time were estimated using Malaysia's minimum wage. Costs were estimated…

Naltrexone depot formulations for opioid and alcohol dependence: a systematic review.

Naltrexone is an opioid receptor antagonist that blocks the reinforcing effects of opioids and reduces alcohol consumption and craving. It has no abuse potential, mild and transient side effects, and thus appears an ideal pharmacotherapy for opioid dependence. Its effectiveness in alcohol dependence is less evident, but compliance with naltrexone combined with psychosocial support has been repeatedly shown to improve drinking outcomes. Limited compliance with oral naltrexone treatment is a known drawback. Several naltrexone implant and injectable depot formulations are being investigated and p…

Long-acting depot formulations of naltrexone for heroin dependence: a review.

PURPOSE OF REVIEW: The major problem with the oral formulation of naltrexone for heroin dependence is poor compliance (adherence). Long-acting sustained release formulations of naltrexone (implantable and injectable) might help to improve compliance and, thus, increase the efficacy of abstinence-oriented treatment of heroin dependence with naltrexone. RECENT FINDINGS: There have been several implantable and injectable formulations of naltrexone developed within the last decade. It was demonstrated that some of them are effective and relatively well tolerated medications for relapse prevention…

P-glycoprotein is not involved in the differential oral potency of naloxone and naltrexone.

The poor oral bioavailability of the opioid receptor antagonist naloxone (NA) when compared with naltrexone (NX) may be related to a greater interaction of naloxone with the efflux drug transporter P-glycoprotein (P-gp). We studied the involvement of P-gp in the transepithelial transport of the two opioid receptor antagonists, using a validated human in vitro Caco-2 cell monolayer model. The bidirectional transport of NA and NX (1, 50 and 100 microm) across the monolayers was investigated in the presence and absence of the specific P-gp inhibitor GF120918 (4 microm). NA and NX showed equal tra…

Topical application of naltrexone facilitates reepithelialization of the cornea in diabetic rabbits.

Delayed corneal reepithelialization is a complication of diabetes, and may lead to ulcers and erosions, which cause ocular morbidity and visual loss. This study examined the efficacy of naltrexone (NTX), a long-acting, potent opioid antagonist, applied topically, to facilitate the repair of standardized corneal abrasions in diabetic (alloxan-induced) New Zealand White rabbits (glucose levels>450 mg/dL). NTX at a concentration of 10(-4)M, or sterile vehicle (SV), was administered topically 4 times per day for 7 days to the abraded eye of uncontrolled Type 1 diabetic (DB), insulin-controlled Typ…

Naltrexone effect on physiological and subjective response to a cold pressor task.

In this double-blind, cross-over study physiological (i.e. blood pressure, heart rate, plasma catecholamine concentrations, plasma cortisol concentrations) and subjective (i.e. McGill Pain Questionnaire, positive affect, distress) response to a cold pressor task was assessed in 19 subjects 1 h after the administration of 50 mg naltrexone and after placebo. Significant differences in plasma catecholamine concentrations were found. Plasma epinephrine concentrations increased during the 1 h period after naltrexone administration but remained largely unchanged after placebo administration. A signi…

Naltrexone attenuates the subjective effects of amphetamine in patients with amphetamine dependence.

Amphetamine abuse and dependence is a global health concern with a collateral increase in medical and social problems. Although some of the neurobiological mechanisms underlying amphetamine dependence and its devastating effects in humans are known, the development of rational and evidence-based treatment is lagging. There is evidence from preclinical studies suggesting that the endogenous opioid system plays a role in mediating some of the behavioral and neurochemical effects of amphetamine in a variety of controlled settings. In the present study we assessed the effects of naltrexone, an opi…

Outcome study of kleptomania patients treated with naltrexone: a chart review.

PURPOSE: This study examines the response to treatment in 17 patients with kleptomania who were treated with naltrexone as monotherapy for up to 3 years. METHODS: Patients were treated with naltrexone (range, 50-200 mg/day) and were assessed at each clinic visit for stealing behavior, urges to steal, and for overall symptom severity. Liver function tests were examined regularly during treatment. RESULTS: Comparison of baseline symptoms with the most recent follow-up visit showed that 13 patients (76.5%) reported reduction in their urges to steal, and 7 (41.1%) reported no stealing behavior. …

Severe uncontrolled pain in buttock in a patient on naltrexone: a diagnostic challenge.

A 26-year-old woman with a history of intravenous drug use was admitted to hospital with worsening pain in the right buttock radiating to the lateral part of the thigh and to the calf with no suspicion of cauda equina compression. Eventually, a diagnosis of sacroiliitis was made and appropriate antibiotics were administered. Provision of analgesia for this patient was difficult. On admission her medications included naltrexone, venlafaxine and tramadol. Initially naltrexone was continued and analgesia provided by epidural local anaesthetic and clonidine, intravenous ketamine and oral agents. A…

[Addiction characteristics as prognostic factors in opiate dependence treated with naltrexone].

INTRODUCTION: Addiction characteristics as prognostic factors in opiate dependence treatment were studied. Thus, several factors related to previous opiate consumption record were considered (current heroin route, amount of heroin, onset age of heroin consumption, time of heroin consumption, other drug consumption record). OBJECTIVE: To establish the prognostic value of addiction characteristics in a naltrexone program for opiate dependence. METHODS: To achieve this objective, an observational, retrospective study was designed with a design of a treatment group with no control group. 945 sub…

Naltrexone and brief counseling to reduce heavy drinking in hazardous drinkers.

The present study examined the utility of daily naltrexone for decreasing alcohol drinking in hazardous drinkers. Forty-one participants participated in a 10-week trial and received 30 min of brief counseling on the first and second week of treatment, as well as a daily dose of 50 mg of naltrexone throughout the trial. Overall, naltrexone-treated participants did not show the same degree of improvement on drinking outcomes as placebo-treated participants. The placebo group drank fewer drinks per drinking day and achieved more abstinence days than the naltrexone group. Craving was also lower fo…

Treatment of cocaine-alcohol dependence with naltrexone and relapse prevention therapy.

This study evaluates whether patients with cocaine-alcohol dependence might benefit from naltrexone (NTX) pharmacotherapy when delivered in conjunction with psychotherapy. Eighty outpatients meeting DSM-IV criteria for alcohol and cocaine dependence were randomly assigned to receive NTX (placebo or 50 mg/d) combined with psychotherapy (Relapse Prevention [RP] or Drug Counseling [DC]) for twelve weeks. It was hypothesized that the skills training focus of RP therapy, in combination with NTX 50 mg/d, would produce greater reductions in cocaine and alcohol use. Outcome measures included self- and…

Naltrexone, an opioid antagonist, facilitates reepithelialization of the cornea in diabetic rat.

Ulcers and erosions of the corneal epithelium, as well as delays in resurfacing of the cornea after wounding, are major causes of ocular morbidity and visual loss in diabetes. To study whether intervention by the opioid antagonist naltrexone (NTX; 30 mg/kg, twice daily) can restore reepithelialization in diabetic cornea, we induced diabetes in rats by intravenous injection of 65 mg/kg streptozotocin. After confirmation of diabetes, 5-mm-diameter epithelial defects that did not include the limbus were created by mechanical scraping of the cornea. At 4 and 8 weeks, corneal reepithelialization wa…

Naltrexone potentiates both amnestic and anxiolytic effects of chlordiazepoxide in mice.

Studies have suggested that opioid antagonists potentiate the anxiolytic effect but not the amnestic action of chlordiazepoxide (CDZ). We investigated the effects of naltrexone (NAL) on the anxiolytic and amnestic effects of CDZ in mice tested in the plus-maze discriminative avoidance task (DAT). Mice are conditioned to choose between two enclosed arms (one of which aversive) while avoiding the two open arms of the apparatus. This task measures memory (time spent in the aversive vs. time in the non-aversive enclosed arms) and anxiety (time spent in the open arms). Mice treated with saline (SAL…
