Nephrotic syndrome (NS) is characterized by high proteinuria (over 3,5g/24 hrs), hypalbuminaemia, general edemas and hypercoagulation. Besid … bitors) with positive nephroprotecive effect. It leads do increasing of glycosuria with concomitant natriuresis and osmotic diuresis. The effect is proportional to glomerulal filtration, but the effect on natriuresis stay in all stages of renal insufficiency.

Cytoplasmic granules have been demonstrated in epithelial cells from the endolymphatic sac, an extraosseus part of the inner ear located in the posterior cranial fossa. Intravenously infused extracts from endolymphatic sacs in anesthetized rats elicited a potent natriuresis and diuresis without effects on blood pressure, glomerular filtration rate, or lithium clearance. Only a minor kaliuresis was observed. Extracts of dural tissue adjacent to the endolymphatic sacs had no effect. It is concluded that the endolymphatic sac contains as endogenous inhibitor of sodium reabsorption and could be th…

The role of the kallikrein system in the natriuresis of SIADH was studied in 3 patients. Following free water intake, serum Na decreased. Urinary Na excretion, urinary aldosterone excretion and urinary kallikrein-like activity (UKaV) increased. Significant relationships were observed between UKaV and urinary aldosterone excretion during the study. Further, there were significant relationships between serum Na and UKaV, and between uric acid clearance and UKaV. In 1 patient, who was observed for a longer period, a significant relationship was observed between Na clearance and UKaV. We conclude …

BACKGROUND: It was demonstrated in streptozotocin (STZ)-induced diabetic rats that the D(1) receptor agonist failed to promote sodium excretion as a result of reduced renal D(1) receptor expression and decreased receptor G protein coupling. The present study examined the influence of glycaemic control with insulin on the renal D(1) receptor dysfunction in STZ-induced type 1 diabetes.
METHODS: Renal function, blood pressure, the natriuretic response to 5% volume expansion (VE) and the effects of the D(1) receptor agonist fenoldopam on natriuresis and on Na(+)/K(+)-ATPase activity in renal tubu…

gamma-Glutamyl L-dopa, a renal pro-drug for dopamine, was administered to rats before and after injection of glycerol, and to a control group which received water in place of glycerol. A third group of rats was given glycerol but no gamma-glutamyl L-dopa. The plasma creatinine in rats given gamma-glutamyl L-dopa and glycerol was significantly lower than in rats receiving glycerol alone. The fall in urine creatinine excretion, and polyuria, after glycerol was reduced by gamma-glutamyl L-dopa and the natriuresis abolished. gamma-Glutamyl L-dopa given alone caused a 4000-fold increase in urine do…

BACKGROUND: Hyponatremia is a frequently observed electrolyte abnormality in patients with central nervous system disease. Several mechanisms, such as SIADH, hypopituitarism, and CSWS, have been proposed with varied incidences among several studies. We attempted to clarify the incidence and mechanism of hyponatremia for each type of TBI. We also assessed the efficacy of sodium supplementation and retention therapy. For sodium retention therapy, hydrocortisone was administered, expecting its mineralocorticoid effect, when the hyponatremia was associated with excess natriuresis.
METHODS: Retros…

AIMS: Recent randomized controlled trials (RCTs) showed improved decongestive response with intensified treatment during hospitalization for acute decompensated heart failure (ADHF). Nevertheless, none demonstrated long-term benefit, possibly due to lack of statistical power. We therefore pooled individual data from three recent RCTs to assess the clinical impact of intensified decongestive therapy.
METHODS: Individual patient data from three RCTs evaluating decongestive strategies in ADHF-acetazolamide (ADVOR), hydrochlorothiazide (CLOROTIC), and natriuresis-guided therapy (PUSH-AHF)-were po…

OBJECTIVES: Thiazide and thiazide-like diuretics remain first-line antihypertensive agents, exerting their primary renal effect through inhibition of the Na+-Cl- cotransporter (NCC) in the distal convoluted tubule, producing natriuresis and saluresis with relative K+ and Ca2+ sparing. Although numerous medicinal plants possess diuretic properties, most preclinical studies assess only urine volume, preventing mechanistic classification and obscuring whether observed diuresis reflects NCC-mediated tubular activity.
METHODS: This focused review applied an electrolyte-based pharmacological framew…

In patients with central nervous system disease, life-threatening hyponatremia can result from either the syndrome of inappropriate secretion of antidiuretic hormone or cerebral salt wasting. Clinical manifestations of the two conditions may be similar, but their pathogeneses and management protocols are different. Cerebral salt wasting syndrome is a disorder in which excessive natriuresis and hyponatremia occurs in patients with intracranial diseases. We report a 6-month-old girl with CSWS associated with tuberculous meningoencephalitis. She was diagnosed as having CSWS on the basis of hypovo…

The effect of indomethacin on the renal function has been surveyed by the literature and our experimental data. The primary purpose of the present paper is to discuss the various mechanisms of the prostaglandins which participate in the overall operation of the kidney. Prostaglandin has been proposed as a "natriuretic hormone" effecting the natriuresis observed with extracellular fluid volume expansion. Dogs and rabbits were volume expanded under Nembutal anaesthesia. The massive natriuresis associated with expansion ws significantly reduced in dogs by preloading injection of indomethacin (4 m…

Hypertension hyponatremia syndrome occurred in a 32-week male neonate following septicemic shock on Day 9. The systolic blood pressure rose from 60 to 85 mmHg as the serum sodium dropped from 136 to 121 mmol/L associated with natriuresis, polyuria, and dehydration. Convulsions occurred at a systolic blood pressure of 102 mmHg. Investigations for hypertension revealed hyper-reninemia without cardio/renovascular or neuroendocrine abnormalities. Salt supplementation and antihypertensive therapy with captopril led to resolution of natriuresis and hyponatremia. Review of literature revealed associa…

Hyponatremia, natriuresis, and a decrease in plasma volume of greater than 10% occurs in approximately 50% of the patients with aneurysmal SAH, perhaps due to SIADH and CSWS. However, fluid restriction, as indicated in SIADH, might result in vasospasm and cerebral infarction in these patients. Maintaining intravascular volume seems to be important in SAH; several reports suggest that cerebral ischemia can be reversed by use of volume expanders. Fludrocortisone has been shown to reduce natriuresis, which may help maintain plasma volume in patients with SAH. Adequate oral salt intake also appear…

The effect of almitrine on urinary sodium excretion was investigated in conscious carotid body-, sino-aortic- and sham-denervated SHR. The animals were kept singly in metabolic cages and had free access to food and tap water. Oral application of almitrine (0.5 mg/kg bwt) during five days induced a twofold natriuresis in sham-operated rats. The first natriuretic response on the fourth day of almitrine treatment was observed in animals of all groups indicating that this effect is not chemoreceptor mediated. The second natriuresis seen on the sixth day of the post-treatment period in both sino-ao…

Diuretics, such as furosemide, are routinely administered to dogs with congestive heart failure (CHF). Traditionally, dose and determination … plication rates, future rehospitalization, and mortality. At their core, loop diuretics act through natriuresis, and attention to body sodium (Na) stores and handling offers insight into the pathophysiology of CHF and pharmacology of diuretics beyond what is achievable from clinical signs alone. Human patients with low diuretic responsiveness or diuretic resi …

It has been suggested that "normal" levels of urine albumin excretion rate (AER) may be predictive of an increased risk for progression of hypertension, cardiovascular morbidity, and mortality. No data are available on the effect of race and gender on AER in normal youth. We evaluated AER in timed urine samples in subjects participating in a study of stress-induced pressure natriuresis. A total of 317 healthy, normotensive adolescents aged 15 to 18 years (155 males and 162 females; 216 blacks and 101 whites) participated in a 5-hour testing protocol, which included a 1-hour period of mental st…

High-dose trimethoprim-sulfamethoxazole (TMP-SMX) causes hyperkalemia, thought to result from TMP-induced blockade of amiloride-sensitive Na(+)-channels in the distal nephron. The present study was performed in anesthetized dogs to determine if increasing distal sodium delivery affects this antikaliuretic effect. In Group 1, intrarenal infusion of vehicle did not alter renal function. In Group 2, i.v. infusion of amiloride led to diuresis, natriuresis and antikaliuresis associated with a reduction of the transtubular potassium gradient (TTKG) in both kidneys. Intrarenal infusion of TMP (0.2 mg…

The renal mechanisms of hypertension were reviewed, and new way to classify hypertension based on renal mechanisms was proposed. Theoretically, the pressurenatriuresis relationship can be affected in two different ways; either a parallel shift along the blood pressure axis toward a higher blood pressure level or a depression in the slope. The parallel shift of the pressure-natriuresis relationship induces non-sodium sensitive hypertension, while the depressed slope induces sodium sensitive hypertension. Since the degree of the shift of the pressure-natriuresis relationship is determined by the…

Marinobufagenin (MBG) is a cardiotonic steroid of the bufadienolide class of compounds which has the ability to inhibit the ubiquitous enzyme, Na+/K+-ATPase, resulting in natriuresis. The involvement of MBG in the pathogenesis of volume expansion-mediated forms of hypertension has been suggested for some time, and we have proposed that MBG participates in the hypertension noted in preeclampsia. We examined the hypothesis that MBG might contribute to these forms of hypertension by promoting the activity of the mineralocorticoid receptor (MR). However, our data demonstrate that instead, MBG inte…

The relationship between the kidney and hypertension is complex. The kidney is both culprit and victim. Renal disease and even renal structural abnormalities (nephron underdosing) lead to hypertension. On the other hand, blood pressure (even blood pressure values in the range of normotension) accelerates a progressive loss of renal function in patients with primary renal disease. This review discusses some recent work in this field, emphasizing that multiple mechanisms are operative in renal hypertension, particularly a shift in the natriuresis-blood pressure relationship (blood pressure natri…
