Management of Retinopathy of Prematurity in the United Arab Emirates: A Consensus Statement From the Emirates Society of Ophthalmology.

Retinopathy of prematurity (ROP) is a leading cause of preventable childhood blindness worldwide and affects a substantial proportion of ver … of ROP in the UAE. A nationally representative panel of UAE-based consultant ophthalmologists, with neonatologists as non-voting advisors, developed recommendations for the International Classification of Retinopathy of Prematurity, Third Edition (ICROP-III) classification, screening, and management. Key management strategies include timely laser photocoagulatio …

Influence of maternal and perinatal complications on therapeutic hypothermia in newborns with low Apgar scores.

OBJECTIVE: The aim of this study was to evaluate the impact of therapeutic hypothermia on maternal and perinatal outcomes in newborns with Apgar score<7 at the 5th min. METHODS: A retrospective cohort study was carried out with 55 newborns who had an Apgar score<7 at the 5th min (35 without and 20 with therapeutic hypothermia) from low-risk pregnancies between 33 and 41 weeks gestation. The Apgar score was calculated through an objective assessment by a neonatologist in the delivery room. Therapeutic hypothermia was indicated by a neonatologist in the delivery room, according to the protocol

Neonatal functional echocardiography.

PURPOSE OF REVIEW: The role of echocardiography has dramatically changed over the past decade and use of functional echocardiography has become increasingly popular among neonatologists and pediatric intensivists in making clinical decisions in sick infants and children. The purpose of this review is to outline the current capabilities and limitations of functional echocardiography, best practices for its clinical application, and evidence for its utility. RECENT FINDINGS: Functional echocardiography can provide direct assessment of hemodynamics at bedside and can be used as a modern hemodyna…

[Retinopathy of prematurity: screening and therapeutic indications].

The retinopathy of prematurity is a proliferative disease of the retinal blood vessels, occurring in the prematurely born babies. As the life support conditions for the prematurely born babies have improved, the retinopathy of prematurity has became a problem, both for the neonatologists and the ophthalmologists. This paper discusses the pathogeny, the risk factors, the classification criteria and especially the methods for the screening and therapy of this disease.

Postoperative chylous ascites: increased scrotal volume as "alarm bell".

Chylous ascites has been reported only rarely as a possible consequence of congenital diaphragmatic hernia (CDH) surgical treatment. The present report regards a case of chylous ascites that developed after surgical treatment of CDH and was interestingly anticipated by increased scrotal volume. The aim was to alert neonatologists and pediatric surgeons about the potential usefulness of this clinical sign as a precocious "alarm bell" for chylous ascites development.

Quintuplet pregnancy.

A quintuplet gestation after ovulation induction with clomiphene and HCG is described. This is the first quintuplet birth reported in Israel. The pregnancy was associated with ovarian hyperstimulation syndrome, toxemia of pregnancy, and pruritus gravidarum. Diagnosis and management of these cases pose a special problem for the obstetrician and neonatologist. The problem of multiple pregnancies and their control in the treatment of infertility with ovulation-inducing drugs is discussed.

Survival at nine neonatal intensive care units in São Paulo, Brazil. Paulista Collaborative Group on Neonatal Care.

A collaborative effort to assess factors affecting newborn survival at neonatal intensive care units (NICUs) was made by studying 1948 newborns admitted to nine NICUs in the city of São Paulo between 1 June and 30 November 1991. Data on the study subjects were obtained using a standardized form. This was the first activity undertaken by a network of neonatologists (the Paulista Collaborative Group on Neonatal Care) dedicated to jointly evaluating and improving neonatal care in that city. The study results showed an overall mortality of 59 deaths per 1,000 neonates, with survival improving as g…

Chondrodysplasia punctata: case report and review of audiological and ENT features.

Chondrodysplasia punctata is a term referring to a clinically heterogeneous group of bone and cartilage dysplasias which cause characteristic epiphyseal stippling. The condition can involve the ear, nose and throat in diverse ways at many levels. We present a case of X-linked brachytelephalangic chondrodysplasia punctata, which illustrates the features of this condition particularly relevant to the audiological physician, otolaryngologist and neonatologist.

Expanding implementation of pediatric whole-genome sequencing: Insights from SeqFirst providers to inform equitable access to a precise genetic diagnosis.

Whole-genome sequencing (WGS) as a diagnostic test offers children suspected of having a rare genetic condition and their families the best direct path toward securing a precise genetic diagnosis (PrGD). Yet, a limited supply and inequitable access to genetic services are impediments to realizing the benefits of a PrGD. Such access disparities might be due to a range of structural and social determinants that manifest in interactions, or the lack thereof, between families, providers, and institutions. Semi-structured key informant interviews (n = 19) were conducted with neonatologists and neur…

Cranial Ultrasonography-Standards in Diagnosis of Intraventricular Hemorrhage and Ventricular Dilatation in Premature Neonates.

Intraventricular hemorrhage (IVH) is a common complication encountered in extremely-low-birth-weight (ELBW) and very-low-birth-weight (VLBW) premature babies. The neurologic outcome of these patients is influenced by the magnitude of the hemorrhagic process that damages the involved anatomic structures but also by the impaired circulation of cerebrospinal fluid (CSF) through the ventricular system, leading to posthemorrhagic ventriculomegaly (PHVM). Cranial ultrasound (CUS) performed by neonatologists (point-of-care ultrasound-POCUS) facilitates the early diagnosis of IVH and PHVM and can obje…

Variation in the management of persistent pulmonary hypertension of the newborn: a survey of physicians in Canada, Australia, and New Zealand.

BACKGROUND: Despite advances in management of persistent pulmonary hypertension of the newborn (PPHN), the risk of mortality and adverse neurological sequelae remains high. Characterizing variation in practices is a crucial step toward improved patient outcome. OBJECTIVE: Evaluate intensive care practices in Canada and the Australia-New Zealand region (AUS-NZ). METHODS: A prospective cross-sectional online survey of neonatologists was conducted. A 35-item questionnaire was developed, validated, and piloted to collect information on diagnosis, inhaled nitric oxide (iNO) practices, alternative…

Challenges in neurologic prognostication after neonatal brain injury.

Parents often ask neonatologists and neurologists to determine neurologic prognosis in the preterm and term infant after neonatal brain injury. Prognostication in these populations remains rather full of uncertainties. Knowledge of available diagnostic tests and their limitations allows the clinician to synthesize the most likely outcomes after neurologic injury. In this review, we describe the diagnostic tools available to the clinician, active areas of research, and challenges in neurologic prognostication of the neonate.

Evaluation and diagnosis of the dysmorphic infant.

Neonatologists have a unique opportunity to be the first to identify abnormalities in a neonate. In this review, multiple anomalies and physical features are discussed along with the potential associated genetic syndromes. The anomalies and physical features that are discussed include birth parameters, aplasia cutis congenita, holoprosencephaly, asymmetric crying facies, preauricular ear tags and pits, cleft lip with or without cleft palate, esophageal atresia/tracheoesophageal fistula, congenital heart defects, ventral wall defects, and polydactyly.

A linear fracture and meningitis associated with non-infected cephalohematoma in a neonate.

We present a neonate with cephalohematoma complicated by a linear skull fracture and Staphylococcus epidermidis meningitis. Clinicians, especially neonatologists, should be aware that a cephalohematoma in the newborn infant with a history of vacuum-assisted delivery could be the origin or trigger point of the infection either as sepsis, meningitis or osteomyelitis. The utmost importance of screening studies should be emphasized in order to be aware of the pathogenic potential of cephalohematomas.

Fatal Vernix Caseosa Aspiration Associated With Persistent Pulmonary Hypertension of the Newborn.

Vernix caseosa aspiration is an extremely rare condition resulting in high mortality if complicated by persistent hypertension of the newborn (PPHN). Herein we offer the first case report of PPHN due to massive vernix caseosa aspiration documented by histopathological examination. This case report is presented to provide a synopsis of the pathoetiology of PPHN related to vernix caseosa aspiration syndrome as likely to be encountered by neonatologists and general pediatricians involved with neonatal care.

One hundred consecutive infants born at 23 weeks and resuscitated.

We sought to delineate the in-hospital outcome for infants born alive and universally resuscitated at an estimated gestational age (GA) of 23(0)/(7) to 23(6)/(7) weeks and to document when and why death occurred. We performed a cohort study of prospectively collected data on 100 consecutive infants born alive at 23 weeks GA from June 16, 1990 through August 6, 2006. All deliveries were attended by a neonatologist and resuscitation was universally attempted. At the time of death, a primary cause was determined by the attending neonatologist. Forty infants survived and 60 died prior to hospital

Medical management of extremely low-birth-weight infants in the first week of life: a survey of practices in the United States.

We sought to determine the current practices of neonatologists in their management of extremely low-birth-weight (< 1000 g) infants. We directly mailed an anonymous survey to the medical directors of 809 neonatal intensive care units in the United States. More than one-third of those surveyed responded, with a substantial majority from intensive care (level III) nurseries or extracorporeal membrane oxygenation centers. Academic centers and private practice environments were both well represented. Some traditional practices have changed, such as beginning resuscitation with 40% rather than 100%…

[AIDS and the gynecologist-obstetrician].

After the first case of HIV infection in a pregnant woman in our hospital, we thought on the need to review the medical literature about HIV infection and pregnancy and bring up to date the knowledge of this topic to the obstetricians and neonatologists. HIV infection is caused by a retrovirus. It selectively infects helper/inducer lymphocytes after binding to a glycoprotein receptor on their surface, named CD4. This depletion is slow and gradual, and by time is characterized by the presence of opportunistic infections and some class of tumors. Homosexual men are by now the group most affected…
