Rod Test in Percutaneous Nephrolithotomy: A Practical Guide to Selective Nephrostomy Placement.

INTRODUCTION: Percutaneous nephrolithotomy (PCNL) is the gold standard for treating renal calculi. With the advancement in technology, the recent trend has been toward tubeless procedures, but some cases still need placement of a nephrostomy. AIMS AND OBJECTIVES: The objective of our study was to assess the intraoperative role of the "metal dilator rod" in deciding whether to place a nephrostomy. MATERIALS AND METHODS: A prospective study was conducted from 15 November 2024 to 15 May 2025, and 83 consecutive patients undergoing PCNL were included in the study. All procedures were performed b

Nomogram for Predicting Postoperative Major Bleeding in Percutaneous Nephrolithotomy: A Retrospective Study.

BACKGROUND AND OBJECTIVE: The aim of this study was to develop a nomogram for estimating the risk of postoperative major bleeding in patients who underwent surgery for percutaneous nephrolithotomy. METHODS: This retrospective study included 493 patients who had undergone unilateral percutaneous nephrolithotomy for kidney calculi at Third Xiangya Hospital from January 2022 to June 2024. Patients were temporally divided into a training set (the first 70% of treated patients) and a validation set (the last 30%). The least absolute shrinkage and selection operator, and multivariable logistic regr

Is percutaneous nephrolithotomy in thrombocytosis patient safe? A retrospective case series analysis.

Percutaneous nephrolithotomy (PCNL) is the gold standard for large and complex renal stone despite other available modalities. PCNL techniques have been evolved with many modifications and innovation in the recent decade. It can be safely performed in elderly patient and children with highest stone-free rate (SFR) among all other modalities. However, as PCNL is a blind procedure, chance of bleeding is very high than other surgical modalities. If patient already had, hematological problem, then perioperative risk of bleeding increased significantly. We are presenting few cases of PCNL in thromb

Percutaneous Nephrolithotomy in an Intrathoracic Kidney: A Rare Anatomical Variant With a Successful Outcome.

Thoracic kidney is the rarest form of renal ectopia and is usually detected incidentally. The coexistence of nephrolithiasis within an intrathoracic kidney associated with a congenital diaphragmatic hernia is extremely uncommon. We report a 32-year-old woman with a left intrathoracic kidney containing a large staghorn calculus herniating through a congenital diaphragmatic defect. A percutaneous nephrostomy was placed under ultrasound and computed tomography (CT) guidance, and percutaneous nephrolithotomy (PCNL) was performed through the 9th intercostal space using a pneumatic lithotripter, ach

A Novel method of ensuring safe and accurate dilatation during percutaneous nephrolithotomy.

OBJECTIVE: To report our technique that helps locate the guidewire into the ureter enabling safe dilatation during PCNL. MATERIALS AND METHODS: Cases in which the guidewire failed to pass into the ureter following successful puncture of the desired calyx were subjected to this technique. A second guidewire was passed through the outer sheath of a 9 Fr. metallic dilator cannula, passed over the first guidewire. The cannula and outer sheath were removed, followed by percutaneous passage of a 6/7.5 Fr ureteroscope between the two guidewires, monitoring its progress through both the endoscopic an

Randomized trial comparing modified tubeless percutaneous nephrolithotomy with tailed stent with percutaneous nephrostomy with small-bore tube.

BACKGROUND AND PURPOSE: Recent trials using smaller percutaneous catheters as well as "tubeless" percutaneous technique have attempted to reduce postoperative analgesia requirements and the morbidity of a traditional large-bore nephrostomy tube after percutaneous nephrolithotomy (PCNL). We performed a randomized trial comparing tubeless procedures and use of small catheters to elucidate any differences in perioperative parameters, postoperative discomfort, complications, convalescence, and CT scan findings. PATIENTS AND METHODS: Twenty-four consecutive patients underwent randomization into tu

Use of Mini-Percutaneous Nephrolithotomy Technique to Remove Extra-Peritoneal Foreign Bodies.

BACKGROUND Foreign bodies (FBs) in the intra- or extra-peritoneal cavity are relatively rare. The aim of treatment is to remove the FB to prevent chronic inflammation and/or infection. Traditionally, surgical management is necessary, as the detection and localization of FBs can be difficult. Mini-percutaneous nephrolithotomy (Mini-PCNL) under the guidance of ultrasonography (US-guided) has recently been used as an alternative to conventional therapeutic options for FB removal. CASE REPORT In this article, we report the cases of 2 patients with an extra-peritoneal wooden toothpick FB treated us
