Renal cell carcinoma: risks and benefits of nephron-sparing surgery for T1 tumors.

Renal cell carcinoma is the most common cancer of the kidneys that is primarily treated with surgery, including removal of part or all the involved kidney depending on size and tumor, complexity, and patient characteristics. Partial nephrectomy historically was restricted to cases of solitary kidney or bilateral tumors. It was then started for masses smaller than 4 cm and currently is even studied and justified in tumors smaller than 7 cm if surgically feasible. Although partial nephrectomy preserves kidney tissue and, therefore, delays or prevents the new onset of CKD and ESRD, radical nephre

Transgenerational programming of fetal nephron deficits and sex-specific adult hypertension in rats.

A developmental insult that restricts growth in the first generation has the potential to program disease in subsequent generations. The aim of this study was to ascertain transgenerational growth and cardio-renal effects, via the maternal line, in a rat model of utero-placental insufficiency. Bilateral uterine vessel ligation or sham surgery (offspring termed first generation; F1 Restricted and Control, respectively) was performed in WKY rats. F1 Restricted and Control females were mated with normal males to produce second generation (F2) offspring (Restricted and Control) studied from fetal

Nephron-sparing resection of angiomyolipoma after sirolimus pretreatment in patients with tuberous sclerosis.

PURPOSE: Renal angiomyolipoma in patients with tuberous sclerosis can cause life-threatening bleeding. Embolization or resection is recommended, but either intervention may result in substantial loss of renal function. Recently, regression of angiomyolipoma size has also been achieved with mTOR inhibitor therapy, but recurrent lesion growth after treatment cessation has to be expected. This is the first report on a multimodal therapeutic approach facilitating a nephron-sparing, definitive resection. METHODS: Three patients with renal angiomyolipoma not amenable to nephron-sparing surgery were

Predictors and mechanisms of oncological failure following nephron-sparing surgery for renal cancer.

BACKGROUND: One of the major concerns in performing nephron-sparing surgery (NSS) for renal cell carcinoma (RCC) is the risk of tumor recurrence. OBJECTIVES: To assess the rate, predictors and mechanisms of oncological failure in patients after NSS for renal cancer. METHODS: Between 1993 and 2008 NSS was performed in 229 patients via flank incision. Only patients without metastases at diagnosis and minimal 12 months follow-up were included in the outcome analysis. RESULTS: During a mean follow-up of 45 +/- 34 months (range 6-168 months) tumor recurrence was observed in 13 patients (5.6%). M

Novel molecular pathways in renal Mg2+ transport: a guided tour along the nephron.

PURPOSE OF REVIEW: This review highlights recent advances in renal magnesium (Mg) handling. The understanding of the molecular processes of epithelial Mg transport has expanded considerably due to the identification of novel genes involved in hypomagnesemic disorders. RECENT FINDINGS: Mg deficiency remains one of the most common electrolyte disorders. Detailed genetic analysis of families with inherited forms of hypomagnesemia has led to the identification of new genes involved in Mg homeostasis. As such, familial hypomagnesemia has been linked to mutations in the claudin-16/19 complex locate

Link between reduced nephron number and hypertension: studies in a mutant mouse model.

Low birth weight (LBW) infants with reduced nephron numbers have significantly increased risk for hypertension later in life, which is a devastating health problem. The risk from a reduction in nephron number alone is not clear. Recently, using conditional knock-out approach, we have developed a mutant mouse with reduced nephron number in utero and no change in birth weight, by deleting fibroblast growth factor receptor 2 (fgfr2) in the ureteric bud. Our purpose was to investigate the role of in utero reduced nephron number alone in absence of LBW as a risk for developing hypertension in adult

Differential nephron HO-1 expression following glomerular epithelial cell injury.

BACKGROUND: In proteinuria of glomerular origin there is upregulation of heme-oxygenase (HO), the rate-limiting enzyme of heme degradation, in the nephron in a segment-specific manner. To better characterize this phenomenon, we employed a model of proteinuria resulting from disruption of the glomerular capillary permeability barrier to protein by administration of the glomerular epithelial cell toxin puromycin aminonucleoside (PAN) to rats. In this model, we assessed nephron distribution of the expression of the inducible HO isoform, HO-1, and the role of free radicals in modulating HO-1 expre

Retroperitoneal laparoscopic nephron-sparing surgery for renal tumors: report of 32 cases.

OBJECTIVES: To evaluate the feasibility and clinical efficacy of retroperitoneal laparoscopic nephron-sparing surgery for renal tumors. METHODS: Between June 2002 and February 2004, 11 cases of renal benign tumor and 21 cases of renal malignant tumor underwent enucleation of the tumor and wedge resection of the tumor through retroperitoneal laparoscopy, respectively. Tumor resection and hemostasis were mainly achieved by harmonic scalpel. Follow-up studies were performed with an evaluation using renal spiral computed tomography. RESULTS: All procedures were technically successful. The mean o

Salt handling in the distal nephron: lessons learned from inherited human disorders.

The molecular basis of inherited salt-losing tubular disorders with secondary hypokalemia has become much clearer in the past two decades. Two distinct segments along the nephron turned out to be affected, the thick ascending limb of Henle's loop and the distal convoluted tubule, accounting for two major clinical phenotypes, hyperprostaglandin E syndrome and Bartter-Gitelman syndrome. To date, inactivating mutations have been detected in six different genes encoding for proteins involved in renal transepithelial salt transport. Careful examination of genetically defined patients ("human knocko

Mechanisms of distal-nephron Li(+) reabsorption during dietary K(+) restriction in rats.

OBJECTIVE: Coronavirus 2019 (COVID-19) has now been declared as a worldwide pandemic. Currently, no drugs have been endorsed for its treatment; in this manner, a pressing need has been developed for any antiviral drugs that will treat COVID-19. Coronaviruses require the SARS-CoV-2 3CL-Protease (3CL-protease) for cleavage of its polyprotein to yield a single useful protein and assume a basic role in the disease progression. In this study, we demonstrated that punicalagin, the fundamental active element of pomegranate in addition to the combination of punicalagin with zinc (Zn) II, appear to sho

NEM-sensitive ATPase activity in rat nephron: effect of metabolic acidosis and alkalosis.

The present study was designed to quantitate the amount and to map the localization of N-ethylmaleimide (NEM)-sensitive adenosinetriphosphatase (ATPase) activity in microdissected segments of the rat nephron. After complete nephron mapping the effect of chronic metabolic acidosis and alkalosis on enzyme activity was determined. In control animals the highest enzyme activity was found in the early proximal convoluted tubule of juxtamedullary nephrons; superficial early proximal tubule as well as medullary and cortical thick ascending limbs and collecting ducts also contained substantial activit…

Alpha2-adrenoceptors and cellular cAMP levels in single nephron segments from the rat.

A functional role for the numerically predominant renal alpha2-adrenoceptors, which in other tissues inhibit adenylate cyclase, remains undefined. We therefore examined the effect of alpha2-adrenoceptor stimulation with (-)-epinephrine (E) on cell cAMP content in the isolated proximal convoluted tubule (PCT), medullary and cortical thick ascending limb of Henle, and collecting tubule (MTAL, CTAL, MCT, and CCT, respectively). Parathyroid hormone (1-34 PTH), in PCT or CTAL, or arginine vasopressin (AVP), in MTAL, CTAL, MCT, or CCT, was used to activate adenylate cyclase in intact cells from thes

Effect of carbonic anhydrase inhibition on superficial and deep nephron bicarbonate reabsorption in the rat.

The nephron segment responsible for the acetazolamide-insensitive fraction of renal bicarbonate reabsorption has not been clearly delineated. This study compares superficial and deep nephron bicarbonate reabsorption before and after acetazolamide at two dose levels (20 and 50 mg/kg per h) in mutant Munich-Wistar rats employing both cortical and papillary micropuncture and microcalorimetry. Systemic acid-base balance and right whole kidney glomerular filtration rate were similar in all groups examined. The effects of the two doses of acetazolamide were indistinguishable and resulted in a signif

The excretion of renal cells following necrosis of the distal segment of the nephron by hexadimethrine bromide.

The rate of excretion of renal cells was determined in rats with necrosis of the distal convoluted tubules and broad ascending limbs of the loops of Henle caused by injection of hexadimethrine bromide. The magnitude and the duration of abnormal cell excretion were correlated both with the dose of hexadimethrine and with the degree of damage that was evident on histological examination of the kidney. In general cell excretion studies provided a satisfactory indication of the degree of renal damage but the smallest lesions did not cause a significant increase in cell excretion and occasionally a

KDM1A regulates nephron progenitor programs and is associated with human kidney disease risk.

BACKGROUND: Genome-wide association studies (GWAS) have identified numerous loci associated with kidney function, but linking non-coding variants to causal effector genes remains challenging. By integrating fine-mapped GWAS data from 2.27 million individuals with kidney expression and epigenomic datasets, we identified KDM1A as a top-prioritized gene at a kidney function locus. KDM1A is broadly expressed across nephron segments in mouse and human kidneys and is consistently upregulated in chronic kidney disease (CKD). METHODS: We generated lineage-specific Kdm1a knockout mice targeting nephro

Roles of WNK1 and mTORC2 in aldosterone-independent regulation of potassium secretion in the distal nephron.

INTRODUCTION: Precise regulation of kidney potassium excretion is essential for maintaining plasma potassium concentration within narrow limits in the face of wide variations in intake. Aldosterone has long been considered the key regulator of tubule potassium secretion. However, increasing evidence suggests that it plays a relatively limited role over a wide range of potassium ingestion. Here we investigated the roles of WNK1 kinase and the mTORC2 regulatory complex in potassium secretion in the distal nephron of mice. METHODS: We generated knockout (KO) mice where alternatively mTORC2 or th
