Abductor Digiti Minimi and Anterior Interosseous to Ulnar Motor Nerve Transfer: The Super Turbocharge End-to-Side Transfer.

Anterior interosseous nerve to ulnar motor nerve supercharged end-to-side (SETS) nerve transfer to restore intrinsic function is a recently adopted nerve transfer in severe ulnar neuropathy. Its success is predicated on the critical threshold number of axons innervating the intrinsic muscles. Given the relative expendability of the abductor digiti minimi (ADM) muscle and the critical function of the other intrinsic muscles, the authors modified their SETS transfer to redirect axons from the ADM to turbocharge the ulnar motor nerve to innervate the more critical intrinsic muscles. They refer to…

Tendon versus nerve transfers - balancing hand function in upper extremity high nerve injuries.

IntroductionHigh peripheral nerve injuries disrupt the balance between flexor and extensor muscle groups and result in significant impairmen … ements. However, tendon transfers may compromise muscle balance, excursion, and fine motor control. Nerve transfers have emerged as an alternative strategy, aiming to prevent motor loss by reinnervating native muscles. While nerve transfers may better preserve physiological patterns of movement, they require prolonged periods for reinnervation and depend on pati …

Nerve transfers for axillary nerve repair in brachial plexus injuries: results from 206 patients.

OBJECTIVE: Restoration of axillary nerve function is one of the main priorities of brachial plexus surgery. Neurotization, the transfer of a … e long thoracic nerve (36%) and intercostal nerves (29.6%). CONCLUSIONS: The authors conclude that nerve transfers can be effective treatment options for axillary nerve injuries. Knowing the potential success rates of the less used donor nerves, i.e., from different segmental levels of the spinal cord, is of utmost importance, especially for extensive brachial …

Transfer of Motor Fascicle From the Ulnar Nerve to the Axillary Nerve by Posterior Access. New Approach.

ABSTRACT: We describe a new technique of transferring the motor branch ulnar nerve (UN) to the axillary nerve (AN) by posterior approach. Three patients with C5, C6, and C7 brachial plexus injury were operated. By supraclavicular approach, the spinal accessory was transferred to the suprascapular nerve. By posterior approach in the arm, the AN was identified within the quadrilateral space, and the UN was identified medially with intrafascicular dissection of a motor fascicle, which is lifted to 4 cm in length and transferred to AN. By medial approach, a motor branch from the median nerve is tr

Results of spinal accessory nerve to suprascapular nerve transfers in children with brachial plexus birth injury.

Shoulder external rotation after brachial plexus birth injury can be restored by transfer of the spinal accessory nerve to the suprascapular nerve, or more distally to its infraspinatus branch. We studied the outcome of these nerve transfers in 52 patients with a minimum postoperative follow-up of 12 months (mean 7.3 years). The median postoperative improvement in shoulder external rotation was 120° (interquartile range [IQR] 45-135) after anterior and 110° (IQR 83-120) after dorsal spinal accessory nerve transfer to the suprascapular nerve main trunk, and 110° (IQR 80-125) after transfer to t…

Intercostal Nerve to Long Thoracic Nerve Transfer for the Treatment of Winged Scapula: A Cadaveric Feasibility Study.

There are very few surgical options available for treating a patient with winged scapula caused by a long thoracic nerve (LTN) injury. Therefore, we devised a novel technique based on a cadaveric dissection whereby regional intercostal nerves (ICN) were harvested and transposed to the adjacent LTN in 10 embalmed cadavers (20 sides). The LTN was identified along the lateral border of the serratus anterior and ICNs were identified at the mid-axillary line inferior to the lower edge of the pectoralis major muscle. Along the mid-clavicular line, each ICN was transected and transposed to the adjace

Nerve Transfers for the Restoration of Wrist, Finger, and Thumb Extension After High Radial Nerve Injury.

High radial nerve injury is a common pattern of peripheral nerve injury most often associated with orthopedic trauma. Nerve transfers to the wrist and finger extensors, often from the median nerve, offer several advantages when compared to nerve repair or grafting and tendon transfer. In this article, we discuss the forearm anatomy pertinent to performing these nerve transfers and review the literature surrounding nerve transfers for wrist, finger, and thumb extension. A suggested algorithm for management of acute traumatic high radial nerve palsy is offered, and our preferred surgical techniq…

Rhomboid nerve transfer to the suprascapular nerve for shoulder reanimation in brachial plexus palsy: A clinical report.

Recovery of shoulder function is a real challenge in cases of partial brachial plexus palsy. Currently, in C5-C6 root injuries, transfer of the long head of the triceps brachii branch is done to revive the deltoid muscle. Spinal accessory nerve transfer is typically used for reanimation of the suprascapular nerve. We propose an alternative technique in which the nerve of the rhomboid muscles is transferred to the suprascapular nerve. A 33-year-old male patient with a C5-C6 brachial plexus injury with shoulder and elbow flexion palsy underwent surgery 7 months after the injury. The rhomboid ner…

A part of the ulnar nerve as an alternative donor nerve for functioning free muscle transfer: a case report.

A patient with no active elbow flexion because of poliomyelitis-like syndrome underwent functioning free muscle transfer for elbow flexion reconstruction in which a part of the ulnar nerve was used as a donor motor nerve. Fourteen months after surgery the patient had achieved 120 degrees of active elbow flexion against gravity without functional deficit of the donor ulnar nerve. A part of the ulnar nerve can be used as an alternative donor motor nerve for reconstruction of chronic C5-C6 (or C5-7) brachial plexus injury.

Rethinking Recovery: Beyond the 18-month Milestone in Nerve Transfer Surgery.

PURPOSE: Conventional dogma is that recovery following nerve transfer surgery plateaus ∼18 months post operation. This widely accepted notion has shaped patient counselling, patient expectations, and clinical follow-up protocols. This study aims to re-examine the long-term recovery beyond 18 months and present novel findings that challenge the conventional recovery timeline. MATERIALS AND METHODS: A retrospective review of adult patients who underwent nerve transfer surgery more than 5 years ago was conducted. Once identified, these patients were contacted to perform a telephone survey, a qua…

Somatosensory Misalignment: Persistent Referred Sensation After Intercostal-to-Musculocutaneous Nerve Transfer.

INTRODUCTION: Referred sensation (RS) is described as the sensation evoked in the skin areas distinct from the site of stimulation. The study sought to identify and map RS in participants with traumatic brachial plexus injury (TBPI) through a standardised assessment. METHODS: Conducted as an observational study, Experiment 1 involved 19 participants who underwent RS screening by stimulating 22 specific skin areas across both upper limbs, the neck and the face with a cotton swab. In Experiment 2, a comprehensive RS mapping was performed using Semmes-Weinstein monofilaments on the reinnervated

Brachial Plexus Nerve Transfer Under Wide Awake Local Anesthesia No Tourniquet.

Brachial plexus injuries commonly require nerve transfers, traditionally performed under general anesthesia. We propose the wide awake local anesthesia no tourniquet (WALANT) technique, utilizing lidocaine with epinephrine to achieve anesthesia and maintain a bloodless surgical field without general anesthesia. Our initial experience with nerve transfers (eg, ulnar-to-musculocutaneous for elbow flexion) entirely under WALANT demonstrates excellent patient tolerance, minimal pain, and high satisfaction. Intraoperative nerve identification was enhanced by patient feedback and nerve stimulation w…

Adoptive cell transfer of macrophages following peripheral nerve injury in mice.

Macrophages are key innate immune cells involved in multiple biological processes, including peripheral nerve regeneration. Here, we describe a protocol for the adoptive cell transfer of bone-marrow-derived macrophages (BMDMs) following sciatic nerve crush injury (SNCI). This procedure involves isolating BMDMs from a donor mouse, potentially manipulating them ex vivo, and reintroducing them into an animal following SNCI. Preclinical studies show that BMDMs can infiltrate injured nerves and impact functional recovery, potentially providing a novel therapy for nerve injuries. For complete detail

Extradural contralateral S1 nerve root transfer for spastic lower limb paralysis.

The current study aims to ascertain the anatomical feasibility of transferring the contralateral S1 ventral root (VR) to the ipsilateral L5 VR for treating unilateral spastic lower limb paralysis. Six formalin-fixed (three males and three females) cadavers were used. The VR of the contralateral S1 was transferred to the VR of the ipsilateral L5. The sural nerve was selected as a bridge between the donor and recipient nerve. The number of axons, the cross-sectional areas and the pertinent distances between the donor and recipient nerves were measured. The extradural S1 VR and L5 VR could be sep

Nerve transfer in the spastic equino varus foot: Anatomical feasibility study.

PURPOSE: We want to evaluate the feasibility of transferring a motor branch of the anterior tibial muscle (ATM) to the extensor digitorum longus (EDL) to evaluate this procedure in patients with spastic equinovarus foot (EVF) following post-stroke hemiplegia. METHODS: Ten cadaveric dissections from five fresh frozen human cadavers were performed to establish the anatomic feasibility of transferring a motor branch of the deep peroneal nerve, usually destinated to the ATM, to the branch of the EDL to manage spastic EVF. RESULTS: Six cases (60%) presented three branches destinated to the ATM, o

Supercharging the Smile: A Novel Dual Nerve Transfer for Facial Reanimation.

UNLABELLED: Facial paralysis has profound effects on the functional and psychosocial well-being of patients. Various surgical facial reanimation techniques have been described to address this devastating condition. While traditional surgical approaches have proved successful in restoring either facial tone or facial movement, newer combination nerve transfer techniques are addressing the limitations of the traditional single nerve transfer approaches. METHODS: This study aimed to describe a promising new surgical approach to facial reanimation utilizing a dual nerve transfer to maximize both …

Oculo-zygomatic nerve transfer for facial synkinesis: An anatomical feasibility study.

BACKGROUND: Patients with severe oro-ocular synkinesis often present with concomitant inefficient smile excursion on the affected site. In theory, oculo-zygomatic nerve transfer may decrease synkinesis and improve smile by redirecting nerve fibers to their target muscle. The aim of this study was to explore the feasibility of nerve transfer in human cadavers between a caudal branch innervating the orbicularis oculi to a cephalad branch innervating the zygomaticus major muscles. METHODS: Eighteen hemi-faces were dissected. Reach for direct coaptation of a caudal nerve branch innervating the or…
