Cranial neuralgia (CN) can cause significant debilitating pain within a nerve dermatome. Accurate diagnosis requires detailed clinical history and examination, understanding of pathophysiology and appropriate neuroimaging to develop an optimal treatment plan. The objective of this article is to review and discuss some of the more common CNs including trigeminal neuralgia and its associated painful neuropathies, occipital neuralgia, and less common glossopharyngeal neuralgia (GPN). The neuroanatomy, pathophysiology, diagnostic imaging, and treatment of each of these pathologies are reviewed wit…

Postherpetic neuralgia, when defined as neuropathic pain persisting 1 month or longer after herpes zoster infection, affects about 10% of all patients who have contracted the disease. The incidence of postherpetic neuralgia increases with age; at age 60, about 50% of herpes zoster patients will suffer significant pain, and this proportion grows with subsequent decades. If therapy is carefully chosen and monitored, it is possible to give satisfactory relief, taking pain from severe to mild, to between 60 and 70% of patients. This article will review current treatment and focus on antidepressant…

One hundred and fifty-six patients with moderate to severe postherpetic neuralgia (PHN) were followed for up to 11 years. Nearly half of all patients were doing well at the final assessment (median 2 years) and more than half of these were on no therapy at this time. The most commonly used agents associated with a good outcome were antidepressants, topical capsaicin and analgesics of various kinds. Longer duration PHN appeared to have a worse prognosis. More of these patients were noted to be using some form of treatment at follow up. A group of patients seemed to follow a progressive course a…

Neuralgic pain during or following herpes zoster infection is a common problem in pain therapy. The current management of neuralgias due to zoster is discussed with reference to patients in a chronic pain clinic within an anesthesiology department. The courses of 80 patients followed up for at least 3 months from the pain clinic at the University Hospital in Kiel were analyzed. The mean age was 69 years. The predominant locations for zoster lesions were the thoracic segments (65%) and the first branch of the trigeminal nerve (19%). Diabetes mellitus was present in 20% of the patients and malig…

On the basis of analyzing 800 observations on patients with neuralgias and prosopalgias, the authors identify special or paroxysmal neuralgias of the tunnel origin that are marked by a pathognomic clinical picture and require specific therapy. Another group of facial pains--prosopalgias proper--includes pains varying in the origin, clinical course and therapeutic approach. Diagnostic terms for describing neuralgias of the trigeminal nerve are reviewed, and the concept of their primarily central or primarily peripheral origin is proved to be untenable.

A study of 21 patients has shown that clonazepam is an effective drug in preventing attacks of pain in trigeminal neuralgia and Sluder's syndrome. It has also been shown that no side effects result from long-term prescription of this drug.

BACKGROUND: Trigeminal neuralgia(TN), hemifacial spasm (HFS) and glossopharyngeal neuralgia (GPN) were referred to hyperactive dysfunction syndromes (HDSs) of the cranial nerves. These symptoms may occur synchronously or metachronously, but the combination of three diseases is extremely rare.
METHODS: From 2007 through 2013, six patients with coexistent GPN-HFS-TN were treated in our department. The combined symptoms occurred on the same side in three and on both sides in three. These patients underwent nine microvascular decompression (MVD) procedures in total. The clinical data including op…

BACKGROUND: Trigeminal neuralgia (TGN) secondary to hydrocephalus is relatively uncommon in clinical practice. This study aimed to investigate the correlation between hydrocephalus and TGN and evaluate the efficacy of surgical intervention in alleviating TGN.
METHODS: We conducted a retrospective analysis of three cases from our institution and performed a systematic literature review (PubMed search up to December 2024). The inclusion criteria were patients with concurrent hydrocephalus and TGN. Data were analyzed for demographic characteristics, treatment modalities, and outcomes.
RESULTS: …

PURPOSE OF REVIEW: To review current treatments for trigeminal neuralgia, with an emphasis on determining which patients may benefit from neurosurgical procedures.
RECENT FINDINGS: A detailed history is the most helpful tool for diagnosing trigeminal neuralgia (TN) and predicting response to neurosurgical treatments. Patients with classic trigeminal neuralgia will describe severe, unilateral, intermittent facial pain that is triggered by innocuous sensory stimuli. In most cases, pain is caused by compression of the trigeminal nerve by a blood vessel near the brainstem. Magnetic resonance imag…
