OBJECTIVE: To explore the therapeutic efficacy and complications of endoscope-assisted surgical resection of acoustic neuroma.
METHOD: Assisted by hard-tube ear endoscope, 11 patients with acoustic neuroma were operated via labyrinthine approach and retrosigmoid approach.
RESULT: Total removal was achieved in 9 (81.8%) cases, while subtotal removal was achieved in 2 (18.2%) cases. Facial nerves and cochlear nerves were preserved completely during operation in all cases. After operation 9 (81.8%) cases had no facial paralysis, while the other 2 cases (18.2%) had mild peripheral facial paralys…

OBJECTIVES/HYPOTHESIS: Previous studies on hearing loss (HL) after acoustic neuroma removal concentrate mainly on pure-tone hearing results rather than hearing disability. Our objectives were to use the Speech, Spatial and Qualities of Hearing scale (SSQ), a comprehensively validated questionnaire, to characterize and quantify the auditory disabilities that patients experience with a profound unilateral HL after acoustic neuroma removal.
STUDY DESIGN: Forty-four patients with profound unilateral HL after acoustic neuroma surgery completed the SSQ. Their findings were compared with those of a …

OBJECTIVE: To evaluate the complications and sequelae of acoustic neuroma surgery, according to tumour size.
PATIENTS AND METHOD: A retrospective analysis of 120 patients who underwent microsurgical resection of vestibular schwannomas between November 1994 and September 2006 was undertaken. Tumour size, extent of removal, preservation of facial and cochlear nerves, complications, and sequelae were considered. The degree of hearing preservation after surgery was determined by the Gardner-Robertson classification.
RESULTS: There were 39 small (<1.5 cm), 59 medium (1.5-3 cm), and 22 large tumou…

We present two subjects with previously undiagnosed acoustic neuromas who complained of vertigo whenever they ran. One had normal hearing while the other already had a unilateral sensorineural deafness. Hyperventilation for 30 seconds provoked an ipsilateral beating nystagmus and reproduced the vertiginous sensation in both subjects. Hyperventilation is a simple bedside test that should be performed when assessing a subject with vertigo or when there is a clinical suspicion of an acoustic neuroma.

A vestibular schwannoma (acoustic neurinoma) is a benign tumour localized in the cerebellopontine angle; it can give rise to cranial nerve symptoms. In recent years stereotactic irradiation has become an alternative to radical surgery. Stereotactic irradiation is administered with a gamma knife unit or with an adapted linear accelerator, as a single fraction (radiosurgery) or fractionated (stereotactic radiation therapy). Stereotactic irradiation gives local control rates of over 90%. Post treatment hearing preservation rate is over 60% and treatment related toxicity is low. Comparable treatme

Most amputation neuromas of the biliary tract occur in the cystic duct stump after cholecystectomy and are asymptomatic. However, when they arise in the main hepatic duct and are associated with obstructive jaundice, it is difficult to distinguish them from carcinoma. We describe a case in which preoperative differential diagnosis was difficult. A 60-year-old man was admitted to the Institute of Clinical Medicine, University of Tsukuba, with a chief complaint of jaundice. Cholangiography showed an irregularly elevated nodular lesion on the lateral wall of the common hepatic duct and multiple f…

Intra-operative neurophysiologic monitoring (IOM) is rapidly evolving as an important adjunct during acoustic neuroma surgery to reduce the incidence of neurologic deficits. Monitoring alerts the surgeon to ongoing changes in neural function. The benefit of facial nerve monitoring in reducing the incidence of facial palsy during acoustic tumor resection appears clear and is now recommended by the National Institutes of Health (Consensus Development Conference on Acoustic Neuroma, 1991). Auditory monitoring is not as effective as facial monitoring but hearing preservation can be enhanced partic…

We report an unusual case of nonscarring alopecia clinically resembling patchy alopecia areata around central solitary circumscribed neuroma. Prompt and spontaneous complete hair regrowth was observed after removal of the tumor. Further knowledge of growth factor and receptor interaction in regulating the hair follicle cycle, as well as on the production of growth factors and cytokines by tumor cells and/or cells within the tumor microenvironment may contribute to better understand the pathologic mechanisms underlying certain curious phenomena such as peritumoral nonscarring alopecia.

Surgical series of hearing preservation in acoustic neuroma usually emphasize a team's results with one particular technique. This report reviews acoustic neuroma outcomes of individualizing the surgical approach to patient and tumor characteristics. This study reviews 60 consecutive hearing preservation acoustic neuroma surgeries in a total series of 330 acoustic neuromas. Tumor sizes ranged from 0.3 to 4 cm in patients ranging from 23 to 74 years of age. Middle fossa surgery was performed in 57%, retrosigmoid in 43%. Overall, measurable hearing was preserved in 77%, and useful hearing in 67%…

To improve the therapeutic effect of acoustic neuroma, 158 patients with acoustic neuroma treated in our hospital were retrospectively reviewed. Among them, 11 recurrent acoustic neuroma were found. The clinical analysis demonstrated that the recurrence of acoustic neuroma had close relationship with the size of the tumor, operative approach and extent of excision. All recurrent acoustic neuroma was large-sized when first operated. The more the tumor was excised, the lower the recurrent rate was. The recurrent rates were 19.4% for partial resection, 13.2% for near-total resection, 0% for total…

Pre-, per- and post-operative ECoG findings obtained from a 43-year-old woman with an acoustic neuroma were studied. Although CAP presented a normal waveform in response to click during the whole process of the operation, it was transformed into an abnormally broadened negative waveform in shape with a mild hearing exacerbation, and has remained unchanged up to the present. This broad response was considered to be a receptor potential, because it showed no adaptational amplitude reduction. Additionally, postoperative ECoG to tone burst stimuli demonstrated an increase in DC potential following…

We report our early experience using stereotactic radiotherapy (radiosurgery) in the treatment of cranial neuroma, by the linear accelerator method. We report the first 13 neuromas treated in 12 patients (follow-up 6-60 months). Radiologically, seven of 10 patients demonstrated signs of central tumour necrosis on follow-up scanning and four of these also demonstrated shrinkage. Of seven assessable acoustic neuroma patients treated, hearing was stabilized in three and improved in two. An abducent neuroma patient treated by this method is reported and demonstrated good response. Our preliminary …
