Neurophysiological mechanisms of dentin hypersensitivity.

In hypersensitive teeth pain is typically induced by cold and hot fluids or foods brought to contact with the sensitive dentin or by osmotic and mechanical stimuli. The results of animal experiments indicate that intradental A-type nerve fibres are responsible for the sensitivity of dentin. They are most probably activated by the hydrodynamic mechanism. Thus, their activation is to a great extent dependent on the condition of dentin with either open or blocked dentinal tubules. Blocking of the tubules effectively prevents the nerve activation. Correspondingly, in human experiments, the conditi

Neurophysiological study of Horner's syndrome.

Sympathetic skin response from the forehead and from the hand obtained by using electric, acoustic stimuli and during deep breathing was recorded in normal subjects and in patients suffering from Horner's syndrome in order to characterize this clinical picture neurophysiologically. The reliability of the method points to its possible usefulness in the pre-postganglionic differential diagnosis also.

Rett's syndrome: a neurophysiological study.

Thirteen girls with Rett's syndrome were evaluated with wakefulness and sleep polygraphic records and EMG studies. The main EEG findings were paroxysmal activity during sleep, multifocal in younger and unifocal in older girls, and "pseudorhythmic flattening". In the opinion of the authors, the changes with age are related to evolution of the disease or maturational changes of CNS. The affectation of the peripheral nervous system was demonstrated in almost half the patients.

Ataxia in myxoedema: a neurophysiological reassessment.

In a long-standing case of myxoedema with ataxia and dysarthria, neurophysiological investigations were carried out to assess how much of the ataxic dysbasic syndrome depended on the slowness of mechanical contraction and how much resulted from primary cerebellar involvement. It was observed that the Achilles reflexogram showed a marked prolongation of contraction and relaxation time and that in both quadriceps and triceps surae mechanical percussion induced a marked myxoedema and prolonged relaxation time. The EMG of these muscles during voluntary contraction and stopping reaction detected an

With neurophysiologic investigations we can distinguish many aspects of functional lesions in the nervus trigeminus, for example localization, prognosis and effects of treatment. Interdisciplinary cooperation is necessary.

[Neurophysiological mechanisms of the reflex lacrimation].

Neurophysiological mechanisms of the reflex lacrimation were analyzed in anesthetized rabbits. The secretory pattern of the lacrimation elicited by stimulation of the cornea consisted of two phases, that is, a rapid flow phase during stimulation and the subsequent slow flow phase in post-stimulus time. Parasympathetic nerve activities are closely related to the secretory volume in the rapid flow phase of the reflex lacrimation. On the other hand, excitation of the sympathetic nerve depressed the secretion in the rapid flow phase, while it facilitated slightly the secretion in the slow flow pha

Segmental neurophysiological mechanisms in scoliosis.

Segmental spinal reflexes (stretch reflexes) were studied in patients with scoliosis. The proprioceptive responses to the phasic stretch of the paraspinal muscles were asymmetric in all patients, and were increased on the convex side. The asymmetry was more pronounced when the patients were standing. The observed asymmetry of the reflex responses was taken to indicate asymmetry in the tone and postural activity of the superficial layer of the paraspinal muscles. A reciprocal relationship was found in the segmental reflex organisation between the superficial and deep layers of the paraspinal mu

Neurophysiological observations in diseased newborns.

The early statements that the EEG alone could correctly be used for the assessment of gestational age (cf., among others, Dreyfus-Brisac, 1964; Parmelee et al., 1968; and Nolte et al., 1969), even in pathological babies, need some restrictive qualifications. Likewise, the more recent statements that certain neurophysiological parameters, including nerve conduction velocity and basic EEG patterns are independent not only of gestational age and body weight, but also of various abnormal factors in the pre- and perinatal periods, appear also too rigid, at least as regards the EEG and the other par

Technologies in clinical neurophysiology for brain-body interfacing: IFCN handbook chapter.

Interfacing the brain with the body represents a central challenge in contemporary clinical neurophysiology. Neurological injuries such as stroke, spinal cord injury, and limb loss disrupt the bidirectional flow of information between central and peripheral circuits, impairing voluntary movement, sensation, and embodiment. Clinical neurophysiology provides the quantitative and methodological foundation necessary to understand and modulate this communication. By characterizing neural signals across cortical, spinal, and peripheral levels, it establishes the principles required to decode and sti

Clinical and neurophysiology study of 17 children with epileptic negative myoclonus.

BACKGROUND: Epileptic negative myoclonus (ENM) is a clinically underrecognized seizure type, and systematic data on its clinical manifestations and optimal management strategies in pediatric populations remain scarce. This case series aimed to describe the clinical manifestations, neurophysiological patterns, and therapeutic outcomes in pediatric ENM. METHODS: A retrospective descriptive analysis was conducted on 17 ENM patients diagnosed at Wuxi Children's Hospital between October 2017 and June 2024. Clinical data, electroencephalogram (EEG) findings, and treatment responses were systematica

Semiology and neurophysiology of clonic seizures in children: a retrospective study.

OBJECTIVES: To identify and quantify clonic seizures in children, we retrospectively reviewed the clinical symptoms and neurophysiology of them. METHODS: Data were obtained from 24 patients presenting with 34 clonic seizures, and their video-electroencephalography (EEG) recordings were examined for symptomatology and ictal EEG characteristics. Additionally, synchronous electromyography (EMG) data from 17 patients were analyzed. RESULTS: Our quantitative analysis demonstrated high diagnostic precision in lateralizing focal clonic seizures, with 90.9% exhibiting contralateral hemispheric conco

Diagnostic utility of clinical neurophysiology in Wilson's disease with hyperkinetic movements.

OBJECTIVE: Movement disorders (MD) are frequent in Wilson's disease (WD). Surface electromyography (sEMG) with accelerometry is commonly used to analyze hyperkinetic MD, as clinical examination may be insufficiently sensitive. We present here the first neurophysiological description of MD in WD patients to: (1) assess the sensitivity of neurophysiology relative to clinical evaluation; (2) correlate MD diagnosed by neurophysiology with the biological and radiological data of patients. METHODS: WD patients with hyperkinetic MD not fully characterized by clinical examination alone were included

Hacking the functions of sleep: noninvasive approaches to stimulate sleep neurophysiology.

Sleep is essentially contributing to human health and well-being through multiple biological functions, including restoration and biosynthesis, brain clearance, energy metabolism, immunological and endocrine processing, synaptic plasticity, memory consolidation, and regulation of cognitive and emotional processes. Sleep disturbances are highly prevalent and are both a symptom and a contributing risk factor for psychiatric, neurological, and somatic disorders. Given the limitations of pharmacological interventions, noninvasive neuromodulation techniques ranging from noninvasive transcranial [tr

Amyotrophic lateral sclerosis with demyelinating neurophysiology and a motor band sign.

We describe an unusual case of clinical amyotrophic lateral sclerosis (ALS) with initial neurophysiological studies suggesting demyelination, along with neuroimaging findings that helped to support the eventual diagnosis. An otherwise well 68-year-old man had 8 weeks of left upper limb weakness. On examination, there were widespread lower and upper motor neurone findings suggesting ALS. However, nerve conduction studies identified sensorimotor demyelinating changes suggesting chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), a diagnosis further supported by cerebrospinal fluid
