The Impact of Ethics and Resources on the Allocation of Traumatic Brain Injury Surgery: A Survey of Neurosurgeons Within Low- and Middle-Income Countries and High-Income Countries.

BACKGROUND: Scarce resource allocation in traumatic brain injury (TBI) care is ethically challenging. We surveyed neurosurgeons about their views on surgical decision-making in TBI in resource-limited settings. METHODS: Neurosurgeons and trainees completed an online survey about surgical decision-making for TBI when neurosurgical capacity is limited. The survey included questions about the impact of patient-level factors (such as the patient's need for surgery or potential for improvement in mortality) and system-level factors (such as operating theater and bed space availability) on decision…

Aneurysm treatment practice patterns for newly appointed dual-trained cerebrovascular/endovascular neurosurgeons: Comparison of open surgical to neuroendovascular procedures in the first 2 years of academic practice.

BACKGROUND: The practice patterns of a hybrid open cerebrovascular/neuroendovascular (CVNV) neurosurgeon in early academic practice is unknown. METHODS: We performed a multi-institutional retrospective cohort study of patients with cerebral aneurysms that were treated within the first 24 months of the neurosurgeon's practice. RESULTS: A total of 533 aneurysms were treated by the three senior authors within the first 24 months of their academic practice. Of these aneurysms, 172 were treated with microsurgical clipping, 191 with coiling, and 170 with flow diversion. Treatment in the setting of

Comparative assessment of surgical outcomes and cost-efficiency between orthopedic surgeons and neurosurgeons in degenerative lumbar spine surgery: a systematic review with pairwise and proportional meta-analysis.

This study combines pairwise and proportional meta-analyses to evaluate differences in outcomes and cost-efficiency between neurosurgeons (NS) and orthopedic surgeons (OS) for lumbar degenerative disease. Following PRISMA guidelines, PubMed, ScienceDirect, Ovid, Web of Science, and Springer Link were searched for studies comparing NS and OS outcomes. Results for pairwise meta-analysis were generated with the "metabin" function using standardized mean differences (SMD) for continuous variables and odds ratios (OR) for binary variables, while the "metaprop" function was used to calculate pooled …

Cumulative Sum and Risk-Adjusted Cumulative Sum Analysis of the First-Year Learning Curve for Unilateral Biportal Endoscopy in a Neurosurgeon with Endoscopic Skull Base Experience.

OBJECTIVE: Unilateral biportal endoscopy (UBE) has emerged as a minimally invasive alternative for lumbar spine surgery; however, its learning curve still presents certain challenges. This study aimed to evaluate the learning curve of UBE during its first year of adoption by a neurosurgeon with extensive endoscopic skull base surgery experience, using both cumulative sum (CUSUM) and risk-adjusted cumulative sum (RA-CUSUM) analyses. METHODS: Between May 2024 and May 2025, a total of 108 patients who underwent UBE were retrospectively analyzed. Procedures included UBE lumbar discectomy (n = 89)

Pedicle Galeo-pericranial Flap Augmentation in Salvage Frontotemporal Cranioplasty: Additional 'Neurosurgeon-friendly' Reconstruction Technique of Aesthetic Neurosurgery in Superficial Temporal Artery Branch Compromised Host.

This technical note aims to demonstrate the usefulness, indications and its limitations of augmentation technique by bipedicle galeo-pericra … lp, onto the titanium plates or beneath the fragile fibrous scar. We do not apply this technique by neurosurgeons alone where infection is active or if the host is irradiated. This technique is recommended as a reconstructive aesthetic neurosurgical procedure. It is a 'neurosurgeon-friendly' simple procedure, as it does not require any special tools or complica …

Global neurosurgery: current and potential impact of neurosurgeons at the World Health Organization and the World Health Assembly. Executive summary of the World Federation of Neurosurgical Societies-World Health Organization Liaison Committee at the 71st World Health Assembly.

Since the creation of the World Health Organization (WHO) in 1948, the annual World Health Assembly (WHA) has been the major forum for discussion, debate, and approval of the global health agenda. As such, it informs the framework for the policies and budgets of many of its Member States. For most of its history, a significant portion of the attention of health ministers and Member States has been given to issues of clean water, vaccination, and communicable diseases. For neurosurgeons, the adoption of WHA Resolution 68.15 changed the global health landscape because the importance of surgical …

Hydrocephalus: A Historical Perspective Down the Memory Lane.

This is a personal account of evolution of knowledge about the etio-pathogenesis, diagnosis and treatment of over three decades as a student and later a professional neurosurgeon. This reflects an historical perspective on the subject not withstanding the tremendous advances that have taken place in the field. It is regrettable that a successful management of hydrocephalus leave aside its cure, still eludes us.

Practice Pattern Variations in the Use of Neuromonitoring, Image Guidance, and Robotics for Lumbar Pedicle Screw Placement Based on a Nationwide Neurosurgery Survey.

OBJECTIVE: Lumbar pedicle screw placement surgery involves various assistive technologies, including fluoroscopic, stereotactic, or robotic navigation and intraoperative neuromonitoring (IONM). We aimed to discern neurosurgeons' preferences for screw placement techniques and IONM utility, while also considering the influence of experience. METHODS: A survey was distributed to members of the Congress of Neurological Surgeons using REDCap software, collecting demographic data and querying preferred techniques for screw placement and IONM modalities. Opinions on IONM use during stereotactic or r…

Thoracic outlet syndrome. Neurosurgical perspective.

Neurosurgeons have for the most part abdicated a role in thoracic outlet surgery and have left the diagnosis and treatment of these patients to thoracic, vascular, and general surgeons. We view this as unfortunate. Neurosurgeons are well-positioned to diagnose these conditions. The major source of confusion with regard to diagnosis is cervical spine disease or peripheral nerve entrapment diseases with which neurosurgeons are quite familiar. Orthopedic consultations with regard to shoulder pathology are encouraged. The supraclavicular approach to treatment is one with which most neurosurgeons w…

Definitions and treatments for chiari-1 malformations and its variants: Focused review.

BACKGROUND: Reviewing the neurosurgical literature demonstrated that spinal neurosurgeons rarely (0.78%) diagnose chiari-1 malformation (CM-1) in adults on magnetic resonance (MR) studies defined by tonsillar descent >5 mm below the foramen magnum (FM). Children, averaging 10 years of age, exhibit CM-1 in 96/100,000 cases. According to the literature, fewer spinal neurosurgeons additionally recognize and treat the low lying cerebellar tonsil (LLCT) syndrome. METHODS: The normal location of the cerebellar tonsils on cranial/cervical MR averages 2.9 mm ± 3.4 mm above or up to 3 mm below the FM.…

Comparison between pediatric and adult neurosurgery: management and future perspectives. Tethered cord syndrome, hydrocephalus, craniosynostosis.

With creation of the many subdivisions within the field of neurosurgery, neurosurgeons have taken on different roles. It is important for neurosurgeons to understand all the different subjects involved to enable them to build up long-term goals in patient management and research. As pediatric neurosurgeons, the authors give accounts of three characteristic diseases encountered in pediatric patients: spinal dysraphism with tethered cord syndrome, hydrocephalus, and craniosynostosis. Drawing on their personal experience of tethered cord syndrome, the authors stress the pathophysiological mechani…

Recognition and Practice of Hypopituitarism After Traumatic Brain Injury and Subarachnoid Hemorrhage in Japan: A Survey.

INTRODUCTION: Individuals with traumatic brain injury (TBI) or subarachnoid hemorrhage (SAH) are at a high risk of hypopituitarism, and should benefit from early diagnosis and management. However, data on awareness, attitudes, and practices of physicians treating hypopituitarism post-TBI/SAH are limited. The objectives of this study were to gain an understanding of the awareness of Japanese neurosurgeons and endocrinologists towards hypopituitarism post-TBI/SAH and the need for pituitary function assessments in these patients; and to assess the practices and perspectives of these specialists o…

Variation in medical management and neurosurgical treatment of patients with supratentorial spontaneous intracerebral haemorrhage.

INTRODUCTION: The role of surgery in spontaneous intracerebral haemorrhage (sICH) remains controversial. This leads to variation in the percentage of patients who are treated with surgery between countries. PATIENTS AND METHODS: We sent an online survey to all neurosurgeons (n = 140) and to a sample of neurologists (n = 378) in Dutch hospitals, with questions on management in supratentorial sICH in general, and on treatment in six patients, to explore current variation in medical and neurosurgical management. We assessed patient and haemorrhage characteristics influencing treatment decisions.…
