Nuclear protein in testis (NUT) carcinoma is a rare and highly aggressive cancer, characterized by rearrangements involving the NUT gene located on chromosome 15q14. In this report, we present the case of a 52-year-old female diagnosed with primary parotid NUT carcinoma. Despite undergoing surgery, adjuvant chemotherapy, and incomplete regional radiotherapy, the patient succumbed to the disease after an overall survival duration of 7 months. We retrospectively discuss patient clinical and pathological features, as well as therapeutic approaches of NUT carcinoma of the head and neck.

NUT midline carcinoma is a rare malignancy most commonly seen in adolescents and young adults. The disease presents most often in the lung or head and neck area but can be seen occasionally elsewhere. The diagnosis can be difficult and requires a high degree of suspicion with demonstration of the classic fusion rearrangement mutation of the NUTM1 gene with one of a variety of partners by immunohistochemistry, fluorescent in situ hybridization, or genomic analysis. An 18-year-old man presented with sudden vision loss in his left eye. Magnetic resonance imaging revealed a tumor that had invaded the left optic nerve, originating from the left posterior ethmoid sinus. Immunohistochemical analyses identified positive staining for NUT protein in the nuclei of tumor cells. AIMS: NUT midline carcinoma (NMC) is a rare undifferentiated and aggressive carcinoma that locates characteristically to the midline of the head and neck, and mediastinum. NMC is characterized by chromosomal rearrangements of the gene NUT, at 15q14. The BRD4 gene on 19q13 is the most common translocation partner forming a fusion oncogene, BRD4-NUT. AIMS: To assess the extent of the problem of nut allergy in schoolchildren within the Severn NHS Trust. To determine how well informed schools are about the condition, their policies and attitudes, and the action that would be taken in the event of an acute reaction.
