Ocular motor dysfunction in stupor and coma.

Ocular motor disorders in stupor and coma are important clinical signs which are easily accessible with observation and a few bedside manoeuvres. Although the manifold signs of ocular motor dysfunction may be confusing to most clinicians, many of the signs can be attributed to clear pathophysiological mechanisms. This holds for conjugate eye deviations as well as for most spontaneous eye movements in coma. Using simple methods to elicit reflex eye movements, in most cases a lesion site within or outside the brain stem can be determined. It is stressed that an exact description and documentatio

The sit-up test: an alternate clinical test for evaluating pediatric torticollis.

The sit-up test, a new evaluation method for differentiating between ocular and orthopaedic torticollis, was evaluated against the monocular occlusion test, using results of three-step testing for standardization. The study group consisted of 31 patients with torticollis between the ages of 4 and 12.5 years. Subjects were selected based only on their ability to cooperate with three-step testing. Three-step testing identified 27 of the 31 patients as having ocular torticollis, with the remaining 4 having an orthopaedic etiology. Sit-up testing correctly identified all 27 ocular torticollis pati

Extraocular muscle cysticercosis presenting as Brown syndrome.

PURPOSE: Report of a case of acquired Brown syndrome caused by infestation of the superior oblique muscle by Cysticercus cellulosae. METHOD: Case seen in a referral practice. A 20-year-old man presented with recurrent attacks of conjunctivitis and diplopia in upgaze. Clinical examination of ocular motility established a diagnosis of acquired Brown syndrome of the right eye. Computed tomography of the right orbit unequivocally established the diagnosis of superior oblique muscle cysticercosis. The patient was started on systemic steroids and albendazole in the prescribed doses for a month. RE

Ocular manifestations of autoimmune disease.

Rheumatoid arthritis, juvenile rheumatoid arthritis, Sjögren's syndrome, the seronegative spondyloarthropathies, systemic lupus erythematosus, multiple sclerosis, giant cell arteritis, and Graves' disease are autoimmune disorders commonly encountered by family physicians. These autoimmune disorders can have devastating systemic and ocular effects. Ocular symptoms may include dry or red eyes, foreign-body sensation, pruritus, photophobia, pain, visual changes, and even complete loss of vision. Because a number of these diseases may initially present with ocular symptoms, physicians should maint

A Rhesus Monkey With a Naturally Occurring Impairment of Disparity Vergence. I. Behavioral Comparisons to Vergence in a Normal Animal.

PURPOSE: Human children with disorders affecting vergence eye movements have difficulty during close work, such as reading. Patients with convergence insufficiency show a receded near point and an exophoria that is greater at near than at far. Neurologic abnormalities may underlie these symptoms, but it is difficult to test this idea directly because there is no animal model for this disorder. In the present case report, we describe behavioral testing in a rhesus monkey with a naturally occurring impairment of vergence eye movements (monkey CI). METHODS: Three monkeys were trained to perform

Non-strabismic Binocular Vision Dysfunction among the Medical Students of a Teaching Hospital: A Descriptive Cross-sectional Study.

INTRODUCTION: Non-strabismic binocular vision dysfunctions are visual disorders that affect the person's binocular vision and visual outcome while performing near tasks and are very common among medical students. This study aimed to find out the prevalence of non-strabismic binocular vision dysfunction among the medical students of a teaching hospital. METHODS: A descriptive cross-sectional study was conducted among medical students of a teaching hospital from 25 April 2022 to 25 May 2022. Ethical approval was obtained from the Institutional Review Committee of the same institute (Reference n

Objective estimation of fusional reserves using infrared eye tracking: the digital fusion-range test.

CLINICAL RELEVANCE: Horizontal fusional reserves are used in the diagnosis and monitoring of common vergence disorders, such as convergence insufficiency, which can cause asthenopia and impact near work. Infrared eyetracking technology shows promise for obtaining automated and objective measurements of fusional reserves, expanding options for screening, clinical testing, and at-home monitoring/vision training. BACKGROUND: Current clinical tests for fusional reserves rely on subjective judgements made by patients (for diplopia) and clinicians (for eye movements). This paper describes an object

Convergence substitution in pontine tegmental hemorrhage: an observation with Frenzel goggles.

We report a case in which convergence movement was substituted for bilateral horizontal gaze palsy due to a pontine tegmental hemorrhage. When visual fixation was suppressed with Frenzel goggles (FG), substituted convergence movements were prominent in our patient. The observation suggests that both vergence eye movements and horizontal gaze movements act synergistically and lesioning of the version system results in overactivity of the convergence system after a pontine hemorrhage. Furthermore, it may be worth using FG in the observation of substituted eye movements in patients with horizonta

[Predominantly ipsilateral smooth pursuit impairment associated with a lesion in the basal pons].

In contrast to the well-defined brainstem centers for saccades and eye reflexes, the brainstem centers mediating smooth pursuit eye movements are poorly understood. In monkeys, unilateral damage to the basal pons impairs horizontal smooth pursuit in the direction of the side of the lesion. The major source of afferents to the basal pons is the middle temporal area (MT). The dorsolateral pontine nucleus (DLPN) in the basal pons is considered to be the major gateway for smooth pursuit related signals to the contralateral flocculus. In humans, only one patient was found to have a unilateral lesio

Virtual reality-based vision therapy versus OBVAT in the treatment of convergence insufficiency, accommodative dysfunction: a pilot randomized controlled trial.

BACKGROUND: Virtual reality is being increasingly applied in vision therapy. However, the differences in effectiveness, optimal treatment cycle, and prognosis between virtual reality-based vision therapy and traditional therapies remain unknown. The purpose of this study was to compare the effectiveness of virtual reality-based vision therapy and office-based vergence/accommodative therapy in young adults with convergence insufficiency or accommodative dysfunction. METHODS: The patients were randomly assigned to either the virtual reality-based vision therapy group or the office-based vergenc

Correlations between clinical measures and symptoms: Report 2: Accommodative and vergence measures with symptoms.

AIM: The aim of this study was to explore associations among clinical measures of accommodation and vergences with symptoms, which could facilitate the validation of the Convergence Insufficiency Symptoms Survey to screen for deficient clinical measures. METHODS: Using a multi-stage random cluster sampling, 1211 high school students (481 males and 730 females) between 13 and 18 years of age were selected and examined. Visual acuity, stereoacuity and suppression, refractive errors, near point of convergence, heterophoria and fusional vergences, as well as, amplitude of accommodation, accommoda

Oculomotor and Vestibular Deficits in Friedreich Ataxia - Systematic Review and Meta-Analysis of Quantitative Measurements.

Disease-specific oculomotor assessments play a crucial role in the early diagnosis of hereditary cerebellar ataxias. Whereas several studies have reported on quantitative oculomotor and vestibular measurements in Friedreich's Ataxia (FRDA), the value of specific oculomotor paradigms remains unclear. We aimed to address this knowledge gap through a systematic literature review and providing disease-specific recommendations for a tailored set of eye-movement recordings in FRDA. MEDLINE and Embase were searched for studies reporting on quantitative oculomotor and/or vestibular measurements in FRD

Isolated unilateral infarction of the cerebellar tonsil: ocular motor findings.

The oculomotor abnormalities with isolated infarction of the cerebellar tonsil are unknown. In a patient with acute infarction of the right tonsil, we found (1) nearly completely abolished ipsilateral smooth pursuit and impaired contralateral pursuit, (2) a low-amplitude ipsilesional right-beating nystagmus without fixation, (3) gaze-holding deficits, and (4) normal vestibulo-ocular reflex. These findings contrast with striking vestibular abnormalities reported with unilateral flocculus and anterior tonsil infarction. Taken together, these findings allow more diagnostic certainty in cerebellar

Stability of gaze control in dyslexia.

The neurobiological basis of saccade control has at least three components: fixation, reflexes, voluntary control. It was found in earlier studies that the voluntary component of saccade is specifically impaired in dyslexics as compared with controls of the same age. In this study, we searched for evidence of fixation instability by analyzing the eye movements of 99 control subjects and 262 dyslexics (age 7-17 years) performing an overlap prosaccade and a gap antisaccade task. The percentage of intrusive saccades was counted for each subject during a period of the trial where stationary fixati

[Ocular motor disorders in multiple sclerosis].

Multiple sclerosis is the most common disabling neurological disease in young people. Many neuro-ophthalmological manifestations can occur during the course of the illness, and optic neuritis is both the most frequent and the best known. However, some ocular motility disorders can also occur, but since there may be no symptoms, they often remain underdiagnosed. These eye movement disorders are mostly related to brain-stem and cerebellum lesions. They can be acute, indicating a spatial dissemination of the demyelinating disease, and consequently may indicate the need for systemic treatment of m…
