Ultrasonography of the eye and orbit.

Ultrasonography (US) is, quite often, the first imaging modality used in eye and orbit assessment. The indications of ophthalmic US cover a wide range of disease where direct clinical assessment is impossible or of little value. Doppler US enhances the ability to assess blood flow in the main arteries and veins. In order to take full advantage of all the possibilities US has to offer the examiner thorough knowledge of the examination technique and normal US anatomy of the eye and orbit is required. This paper reviews the basics of the examination technique and ultrasound anatomy of the eye and…

Surgical anatomy of the Chinese orbit.

PURPOSE: To describe the metric measurements and normal anatomic variants in Chinese bony orbits. METHODS: Orbital osteology was studied in 194 orbits from 97 Chinese skulls. Morphologic observations and metric measurements were made to elucidate the variations of the orbital bony anatomy and to localize important fissures and foramina within the orbit. Sexual dimorphism and side asymmetry of orbital features were analyzed. Results were then compared with data from other populations. RESULTS: The details of the orbital osteology in Chinese are described. Most orbital features show anatomic v…

Magnetic resonance imaging of the orbit.

Magnetic resonance imaging (MRI) employs the nuclear resonance phenomenon to produce images of living tissues. Unlike computed tomography (CT), which relies solely on differences in the propensity of tissues to absorb X-rays to produce various contrasts, MRI offers a wide variety of pulse sequences, each of which exploits differences in the magnetic properties of protons in living tissue to produce contrast resolution. When optimal protocols are used, MRI contrast resolution in the orbital soft tissues is superior to that provided by any other imaging modality. Other advantages of MRI over CT …

Ossifying fibromyxoid tumor of the orbit.

PURPOSE: This report describes the clinical, radiographic, and histopathologic features of the first known case of ossifying fibromyxoid tumor involving the orbit. METHODS: Case report and literature review. RESULTS: An 81-year-old woman presented with a 3-month history of diplopia, pain, and right upper eyelid swelling. CT and MRI of the orbit disclosed a 2.5-cm extra-conal mass superomedially in the right orbit, with displacement of the globe inferiorly and remodeling of the orbital roof. During surgery, the tumor was noted to be firm, gritty, and extra-periosteal in location, and the bone…

Primary intraosseous hemangiomas of the orbit.

INTRODUCTION: Primary intraosseus orbital hemangiomas are rare vascular anomalies, of which only 23 cases have been previously reported in the literature. We here describe a further two cases. MATERIALS AND METHODS: An anterior approach was used to remove a mass located in the inferior orbital rim in one case, and in the orbital floor in the other. RESULTS: Good restoration of the orbital walls with no signs of recurrence. DISCUSSION: Intraosseous hemangiomas of the orbit are usually located anteriorly, and can be removed by anterior surgical approaches. The main surgical problem is due to …

Aneurysmal bone cyst of the orbit.

We report the case of an aneurysmal bone cyst of the left orbital roof in a 12-year-old boy who presented proptosis of the left eye and painless swelling of the left orbital rim. A 3-cm-large tumor developed in less than 3 months, with first visible signs about 2 months after a minor head injury. Radiologically, the lesion was osteolytic and multicystic. The cyst, filled with hematic fluid, was surgically removed via left frontal craniotomy and a simple curettage with high-speed bur. The patient recovered well and has been in good health throughout 14 months of follow-up.

Adrenocortical carcinoma metastatic to the orbit.

PURPOSE: To describe the clinical course and histopathologic features of a patient with adrenocortical carcinoma metastatic to the orbit. METHODS: Case report and literature review. RESULTS: A 24-year-old man first came to medical attention because of symptoms referable to a 4.47-kg, nonfunctioning carcinoma of the left adrenal cortex. Several metastases ensued, including a large tumor to the right superior lateral bony orbit with extension to the brain, temporalis fossa, and orbit proper. The tumor was resected with the use of a combined neurosurgical, ophthalmic, and craniofacial approach.…

Meningiomas of the orbit: contemporary considerations.

We have recently demonstrated that telomerase activity is increased and telomere length shortened in lymphocytes from peripheral blood of patients with cutaneous T-cell lymphoma. In order to determine which cell type has increased telomerase activity and shortened telomere length, CD4+, CD8+, CLA+ CD3+ and CLA- CD3+ T cells were isolated from peripheral blood of 25 patients, including 15 patients with mycosis fungoides and 10 patients with parapsoriasis. Eleven healthy individuals were used as controls; CD19+ B cells were separated from each individual as an internal control. The results showe

Malignant rhabdoid tumor of the orbit.

A 36-month-old girl had a 3-week history of proptosis of the right eye. Computed tomography showed an ill-defined homogeneous mass filling the intraconal space. Histopathologic examination and immunohistochemistry findings of an incisional biopsy specimen were consistent with malignant undifferentiated tumor with rhabdoid features. Despite chemotherapy (a combination of vincristine sulfate and dactinomycin) and radiotherapy, massive orbital recurrence occurred 6 months later and orbital exenteration was performed. The recurrent tumor was composed entirely of pleomorphic epithelial cells with p…

Solitary fibrous tumor of the orbit.

Solitary fibrous tumor (SFT) of the orbit is a very rare lesion that may be misdiagnosed as fibrous histiocytoma, hemangiopericytoma, or other orbital tumors. We present a 62-year-old man who presented with painless proptosis, 20 years following left eye enucleation for a presumed neurofibroma. On T2-weighted magnetic resonance imaging (MRI), a hypointense tumor almost filled his entire left orbit. There was no intracranial extension. The specimen obtained at orbital exenteration was consistent with the histologic, immunohistochemical, and electron microscopic findings of SFT. The tumor was po…

Primary malignant ectomesenchymoma of the orbit.

Malignant ectomesenchymoma is a rare soft tissue tumor of the childhood believed to arise from a remnant of pluripotential migratory neural crest cell (ectomesenchym) and composed of both a mesenchymal element (most often embryonal rhabdomyosarcoma) and a neuroectodermal element (ganglioneuroma, schwanomma neuroblastoma or melanocytic cells). Reported sites of origin are the abdomen, perineum or scrotum, the extremities, the middle ear, nasopharynx, face, and neck. Herein we report a new case of an orbital ectomesenchymoma studied by means of histochemistry and immunohistochemistry in order to…

Atypical choristomatous cysts of the orbit.

BACKGROUND: The commonset choristomatous cysts of the orbit are dermoid and epidermoid cysts, which are lined by kerantinizing squamous epithelium. They typically occur in the superior orbit, most commonly superotemporally. Other types, lined by different epithelia, and other orbital sites are much less common. METHODS: Five cases of atypical orbita cystic choristomas that presented over the past 6 years are reported. The clinical and radiological features, surgical approach and findings, and histology, are described. Similar cases from the literature are reviewed and possible aetiology discu…

[Rhinogenous inflammatory diseases of the orbit].

Patients of young age predominated in the group of 113 patients (69 males, 44 females) with orbital rhinogenic inflammation observed by the authors. 57.5% of the cases arose due to acute sinusitis, 42.5%-due to exacerbation of chronic sinusitis. The inflammation focus developed more frequently in the frontal sinuses and ethmoid labyrinth. Periostitis was diagnosed in 17 patients, subperiosteal orbital abscess in 9 patients. Inflammation of the orbital soft tissues presented as eyelid and orbital fat edema in 66 patients, as retrobulbar abscess in 11 and phlegmon in 10 patients. The treatment p…

Color Doppler US of the orbit.

Color Doppler imaging of the orbit shows promise in evaluation of several types of orbital abnormalities. The most promising use is for evaluation of vascular disorders. Screening for orbital varices and cavernous-carotid fistulas is easily accomplished. Color Doppler imaging can also be useful in diagnosis of ocular ischemia, central retinal vessel occlusion, and anterior ischemic optic neuropathy. In cases of inflammatory disease, it may be helpful in searching for complications of infection and in monitoring the size of fluid collections. In cases of tumors, color Doppler imaging can be use…
