Azacitidine-induced cryptogenic organizing pneumonia: a case report and review of the literature.

BACKGROUND: Myelodysplasia syndrome is a heterogeneous group of hematological disorders that are characterized by abnormal morphology and cytopenias of bone marrow elements. Azacitidine is a hypomethylating agent that is commonly used in treatment of myelodysplasia syndrome. We present an extremely rare case of cryptogenic organizing pneumonia following therapy with azacitidine and a review of the relevant literature. This is the fifth case of azacitidine-induced interstitial lung disease and the sixth one due to hypomethylating drugs; of interest, this is the first reported case that has occu…

Reversed halo sign in invasive fungal infections: criteria for differentiation from organizing pneumonia.

BACKGROUND: The purpose of this study was to identify CT scan findings that differentiate the reversed halo sign (RHS) caused by invasive fungal infection (IFI) from the RHS caused by organizing pneumonia (OP). METHODS: We retrospectively reviewed CT scans of patients with RHS caused by IFI or OP. The study included 15 patients with proven or probable IFI (eight men and seven women) and 25 patients with biopsy-proven OP (13 women and 12 men). The CT images were reviewed individually by two chest radiologists who were blinded to the final diagnosis. RESULTS: Reticulation inside the RHS was ob…

Bronchiolitis obliterans organizing pneumonia associated with sulfasalazine in a patient with rheumatoid arthritis.

Pulmonary toxicity and blood dyscrasias are rare side effects of sulfasalazine. Pulmonary pathology is variable, the most common being eosinophilic pneumonia with peripheral eosinophilia, and interstitial inflammation with or without fibrosis. We here present the case of a 68-year-old female patient treated for 6 months with sulfasalazine for rheumatoid arthritis. On laboratory examination, eosinophil count was 97 x 10(3) mm(3). Thorocoscopic biopsy was performed. Histopathologic diagnosis was bronchiolitis obliterans organizing pneumonia (BOOP). This is the first case in the literature to pre…

A case of polymyositis complicated with organizing pneumonia: case report and literature review.

A 54-year-old man was admitted to hospital with fever, dyspnea, and polyarthralgia. A chest radiograph showed consolidations in the bilateral lungs, and histological examination of transbronchial lung biopsy samples revealed organizing pneumonia. He was also diagnosed with polymyositis because of muscle weakness, elevated muscle enzymes, myogenic findings on the electromyogram, and a positive test for the anti-Jo-1 antibody. Herein, we review 25 cases of organizing pneumonia with polymyositis/dermatomyositis with respect to their clinical features and treatment.

[Atelectasis of the right lower lobe in association with bronchiolitis obliterans organizing pneumonia].

A 58-year-old woman with underlying rheumatoid arthritis was admitted to the hospital because of a dry cough and the presence of an abnormal shadow in the right lower lung field. Consolidation and volume loss in the right lower lobe with air bronchogram were recognized on a chest tomogram. Bronchofiberscopic examination disclosed neither stenosis nor tumors in the large bronchi. Organizing pneumonia was recognized pathologically in transbronchial lung biopsy (TBLB) specimens. The volume of the right lower lobe decreased rapidly, and new infiltration shadows appeared in the right upper and midd…

Bronchiolitis obliterans organizing pneumonia after syngeneic bone marrow transplantation for acute lymphoblastic leukemia.

We report a patient who developed bronchiolitis obliterans organizing pneumonia (BOOP) after syngeneic BMT for ALL. The patient complained of persistent low-grade fever and non-productive cough after engraftment. Chest CT scan showed patchy infiltration bilaterally in the lower lung fields. Antibiotics were ineffective. Cultures, serological studies and polymerase chain reaction detected no infectious pathogens. We finally made a diagnosis of BOOP by thoracoscopical lung biopsy. The lung lesion disappeared in a month with corticosteroid therapy. While BOOP following allogeneic BMT has been rep…

Bronchiolitis obliterans organizing pneumonia (BOOP): the cytological and immunocytological profile of bronchoalveolar lavage.

The cytological and immunocytological profile of bronchoalveolar lavage (BAL) was studied in 10 patients with idiopathic bronchiolitis obliterans organizing pneumonia (BOOP) and compared with the data in idiopathic pulmonary fibrosis (IPF) (n = 22), chronic eosinophilic pneumonia (CEP) (n = 9), and extrinsic allergic alveolitis (EAA) (n = 24). Lymphocyte subsets were enumerated using an immunoperoxidase slide assay. The BAL pattern in BOOP patients was characterized by several features: 1) colorful cell differentials with an increase in all cell types, most markedly in lymphocytes, and more mo…

[Bronchiolitis obliterans organizing pneumonia (BOOP). Characteristics and boundaries of an anatomo-clinical entity].

Bronchiolitis obliterans organizing pneumonia (BOOP) is defined by endoluminal obstruction of distal airspaces by a fibrous granulation tissue consisting of inflammatory cells, fibroblasts, and connective tissue. The morphogenesis of the process is the following: diffuse alveolar injury leading to basement membrane denudation and intra-alveolar exudate of fibrinogen, immunoglobulins, coagulation factors, fibronectin; intra-alveolar migration of interstitial fibroblasts through gaps of the epithelial basement membrane; intra-alveolar secretion of loose connective tissue (codistribution of colla…

Organizing pneumonia potentially caused by infliximab in a pediatric patient with ulcerative colitis.

BACKGROUND: Organizing pneumonia (OP) is a rare interstitial lung disease characterized by intra-alveolar fibroblastic plugs and chronic inflammation. Although most cases are idiopathic, OP may occur secondary to infections, autoimmune disease, or drug exposure. Pulmonary complications of tumor necrosis factor-alpha (TNF-α) inhibitors are uncommon, and infliximab-associated OP has not yet been described in the pediatric population. CASE PRESENTATION: A 12-year-old girl with ulcerative colitis (UC) on infliximab presented with three weeks of dyspnea, fever, and pleuritic chest pain. Computed t…

A case report of relapsing organizing pneumonia in rheumatoid arthritis with literature review.

RATIONALE: Organizing pneumonia (OP) rarely occurs in patients with rheumatoid arthritis (RA). We report a case of concurrent onset of OP and RA, in which OP relapsed after 9 years, while RA remained in a state of low disease activity. PATIENT CONCERNS: A 65-year-old woman presented with pain in multiple joints accompanied by recurrent fever. Chest computed tomography (CT) showed inflammation in the right lung. After treatment for the infection proved ineffective, a right lung biopsy confirmed OP. Despite 9 years of antirheumatic treatment, the patient experienced recurrent fever and cough. C…

Perioperative Treatment of Metachronous Multiple Lung Cancer with Organizing Pneumonia: A Case Report.

We report a rare case of the independent coexistence of lung cancer and organizing pneumonia (OP) in different lobes of the right lung in a 67-year-old man with a history of left upper lobectomy. Computed tomography revealed a lesion consistent with OP in the right upper lobe and a suspicious shadow in the right lower lobe, which was diagnosed as squamous cell carcinoma via bronchoscopic biopsy. The patient underwent right lower lobectomy and partial resection of the right upper lobe under left one-lung ventilation. Empirical corticosteroids were administered preoperatively, followed by a brie…

Magnetic resonance imaging of focal organizing pneumonia: differential diagnosis with peripheral lung carcinoma.

BackgroundComputed tomography (CT) is the most common way to evaluate focal organizing pneumonia (FOP); however, sometimes it is difficult to differentiate FOP and peripheral lung carcinoma (PLC).PurposeTo clarify the MRI manifestation of FOP and the value of MR in the differential diagnosis of FOP and PLC in comparison to CT.Material and MethodsChest MR (3D T1WI, T2WI TSE, DWI) and CT images of 72 patients (50 men: mean age=64.7 years; 22 women: mean age=64.9 years; 36 FOPs and 36 PLCs) were retrospectively analyzed. Two experienced radiologists reviewed all CT and MR images and graded CT and…

Korean Guidelines for Diagnosis and Management of Interstitial Lung Disease: Cryptogenic Organizing Pneumonia.

Cryptogenic organizing pneumonia (COP), one of the idiopathic interstitial pneumonias (IIP), exhibits an acute or subacute course. It can be diagnosed after excluding secondary causes or diseases. COP accounts for approximately 5% to 10% of IIPs, with the average age of diagnosis ranging from 50 to 60 years. Patients primarily present with dry cough and dyspnea. They often experience fever, fatigue, and weight loss. Common radiologic findings on high-resolution computed tomography include localized consolidations, which are typically subpleural or located in the lower zones, though they can oc…

Secondary Organizing Pneumonia: A Case Report of a Noteworthy Complication of Breast Irradiation.

Organizing pneumonia (OP) is a pattern of pulmonary tissue repair injury. It can be cryptogenic or a response to a specific lung injury. Radiation therapy is a potential cause of secondary OP, especially after breast irradiation. The lesions may appear anywhere in the lung parenchyma with no relation to the irradiated side. Corticotherapy is the mainstay of treatment and prognosis is generally favorable. We describe a case of a female patient previously diagnosed with breast cancer who developed secondary OP within six months of postoperative radiotherapy. Initial diagnosis was challenging due…
