Anomalous origin of the left coronary artery from the pulmonary artery is an exceedingly rare and potentially fatal congenital coronary anomaly that typically presents early in infancy. We report an unusual case of anomalous origin of the left coronary artery from the pulmonary artery in a 4-year-old child who presented later in life with vague respiratory and gastrointestinal symptoms and was found to have severe global cardiac dysfunction with evidence myocardial ischaemia.

Two cases of anomalous origin of the left coronary artery (LCA) from the pulmonary artery (PA) are reported in which early surgical correction was achieved by an end-to-end anastomosis of the left subclavian artery (LSA) to the LCA, with cardiopulmonary bypass and deep hypothermia. The clinical improvement was rapid and the left ventricular function recovered almost completely. We recommend this type of surgical correction, early in life, for infants in severe cardiac failure and anomalous origin of the LCA.

Fever of unknown origin can be challenging to the emergency medicine physician and the array of both benign and potentially life-threatening diagnoses on the differential can lead to a costly, time-consuming work-up. In this article, we present a case of potentially fatal tick-borne illness with a focus on history, physical exam, and laboratory findings that would differentiate this diagnosis from other causes of fever of unknown origin and aid in early and efficient initiation of treatment for patients.

Ewing sarcoma is a locally aggressive, highly malignant tumor most commonly seen in the skeletal system. The "Ewing family of tumors" also includes other tissue types that are not common, such as soft tissue origin classified as extraosseous Ewing sarcoma (EES) or primitive neuroendocrine origin. Age of onset most often occurs within the first 2 decades of life. Congenital presentation of EES is exceedingly rare. We report the first described case to our knowledge of congenital EES originating from the scalp.
