[Living related liver transplantation for patients with ornithine transcarbamylase deficiency].

Ornithine transcarbamylase deficiency (OTCD) is an inborn error of urea synthesis inherited as an X-linked trait, a clinical manifestation of which is a repeated episodes of hyperammonemic coma. Recently, liver transplantations have been performed in these patients in the USA and Europe. We experienced the anesthetic managements of liver transplantations in two OTCD patients, who had been suffering from several episodes of hyperammonemic decompensation despite a restricted protein diet with administration of sodium benzoate. Anesthesia was induced and maintained with a combination of fentanyl …

Feeding rhythm and ornithine decarboxylase activity in hereditary microphthalmic rats.

The effects of congenital absence of visual input on daily rhythms of feeding behavior and tissue ornithine decarboxylase activity were examined in hereditary blind rats. Food intake of normal and hereditary microphthalmic rats was determined every 3 h over 48 h under a 12 h-12 h light-dark cycle. Normal rats took their daily food mainly during the dark period, with two clear peaks in the evening and morning, but hereditary microphthalmic rats ate continuously, suggesting that the feeding rhythm of the hereditary microphthalmic rats is not entrained by the light-dark cycle due to a lack of the…

Orthotopic liver transplantation for ornithine transcarbamylase deficiency with hyperammonemic encephalopathy.

Ornithine transcarbamylase (OTC) deficiency is an X chromosome-linked disorder causing hyperammonemic encephalopathy with a very poor prognosis. We describe here two patients with OTC deficiency, one a late on-set female patient (case 1) and the other a neonatal-onset male patient (case 2), who were successfully treated with orthotopic liver transplantation (OLTx). The OTC activity in the excised liver was 10% and 0% of control, respectively. Hyperammonemic encephalopathy was controlled with medical therapy in case 1 until the of 5 years, but the complicated course in case 2 in which hyperammo…

Prenatal diagnosis of ornithine transcarbamylase deficiency: results in Spfash mice.

Ornithine transcarbamylase (OTC) deficiency is a human X-chromosome-linked disease (McKusick 31125). The presence of OTC activity in the human placenta encouraged us to examine the possible diagnosis of the disease in an animal model (Spfash mice) by enzymatic assay on placental samples. A significant positive correlation (p < 0.02) was found between placental and hepatic activities; the pH dependence of OTC was similar in the placenta and liver when compared within normal or homozygous mutant mice. The apparent Km (ornithine) and Km (carbamoylphosphate) values of the enzyme did not show any s…

Molecular analysis of a human liver mitochondrial ornithine transcarbamylase deficiency.

The liver of a young girl which had been successfully transplanted was investigated at the ornithine transcarbamylase (OTC, EC 2.1.3.3) gene expression level. Northern blot hybridization using a human OTC cDNA probe showed a greater than 80% decrease in specific OTC mRNA although having the same molecular size as a normal control. OTC polypeptide was simultaneously synthesized with a normal molecular size but at a low level (20%) as shown by immunoblotting. The OTC enzyme from the deficient liver exhibited very little catalytic activity (7.2% as compared to the normal subject). These results m…

Gastric and duodenal mucosal ornithine decarboxylase and damage after corticosterone.

We recently found that stress increases gastric and duodenal ornithine decarboxylase (ODC) activity and damages both tissues. The current study investigated whether corticosterone induces ODC activity in gastric and duodenal mucosa in rats and compared plasma corticosterone levels after stress and treatment with corticosterone to determine whether this hormone mediated the effects of stress on the mucosa. Rats were fasted 22 h, placed in a restraint cage, and immersed in water to the xiphoid process for 6 h. Stress markedly increased plasma corticosterone levels; the increase was 10.5 times co…

Ornithine carbamoyltransferase deficiency. A new variant with subnormal enzyme activity.

A new ornithine carbamoyltransferase variant is described with a nearly normal specific enzymatic activity. This new variant is characterized by a ten-fold increased Km value for L-ornithine (3.75 vs. 0.33 mmol/l in control) and by a normal Km value for carbamoylphosphate (0.42 vs. 0.29 mmol/l). Its pH optimum is shifted towards the alkaline side. This enzyme is highly unstable at 55 degrees C.

Secondary carnitine deficiency in hyperammonemic attacks of ornithine transcarbamylase deficiency.

Carnitine status was evaluated in 12 patients with hyperammonemic attacks caused by a deficiency in ornithine transcarbamylase. We found decreased free carnitine and increased acylcarnitine levels in the serum, a decreased free carnitine content and an elevated acyl/free carnitine ratio in the liver, and increased excretion of free and acylcarnitine in the urine. Analyses of urinary acylcarnitine using the secondary ion mass spectrometry technique revealed increased amounts of acetylcarnitine and dicarboxylic acid derivatives. These data suggest that the patients had a secondary carnitine defi…

Hyper-induction of skin ornithine decarboxylase activity in psoriatic patients.

Ornithine decarboxylase activity was measured in skin from 14 normal and 24 psoriatic subjects (with stable plaque type psoriasis vulgaris) using punch biopsies carried out immediately (0 h) and 6 h after stimulation by cellotape stripping. At 0 h, no significant difference was found between normal and involved or uninvolved psoriatic skin, but a significant increase was measured 6 h after tape stripping. This increase was significantly greater in uninvolved and involved psoriatic skin compared with normal skin. Preliminary results indicated that these changes were localized in the epidermis.

Serum glutamate dehydrogenase and ornithine carbamyl transferase in Reye's syndrome.

Parallel increases in the ornithine carbamyl transferase and glutamate dehydrogenase activities were observed in the serum of 22 Reye's syndrome patients. The increase in the activity of glutamate dehydrogenase was masked by a dialyzable inhibitor. It is proposed that the measurement of serum ornithine carbamyl transferase and glutamate dehydrogenase activities may be useful as an indicator of liver mitochondrial damage in Reye's syndrome.

Prenatal diagnosis of ornithine transcarbamylase deficiency utilizing fetal liver biopsy.

The prenatal diagnosis of ornithine transcarbamylase deficiency (OTCD) was made by using radioactive microassays for ornithine transcarbamylase (OTC) and--as an internal control--carbamyl phosphate synthetase (CPS I) in liver biopsy material from two 19-week-old at-risk fetuses. In each case, no OTC activity could be detected, while CPS I activity was normal. Control fetuses of 17-21 weeks gestation had OTC activities in the range of 10.7 to 19.4 mumol/mg protein per hr. The prenatal results were confirmed post abortum by the radiochemical assays and by an enzyme-specific cytochemical staining…

Psoralen-containing sunscreen induces phototoxicity and epidermal ornithine decarboxylase activity.

Sunscreens containing 5-methoxypsoralen (5-MOP) are being promoted commercially to increase suntanning and sun protection. A recent study indicated that the 5-MOP concentration used in these sunscreens is too low to induce cutaneous phototoxicity with ultraviolet (UV) radiation. We investigated whether the sunscreen Sun System III (SS III), which contains 5-MOP, could induce skin erythema, edema, delayed pigmentation, and epidermal ornithine decarboxylase (ODC) activity when used in conjunction with UVA radiation (320-400 nm). ODC induction is an early event in the promotion of skin tumors. In…

[Hyperammonemia in the newborn through ornithine transcarbamylase deficiency (author's transl)].

This is a report of a family with hyperammonemia in the newborn, very probably due to ornithine transcarbamylase deficiency. Typically the male infant died in the newborn period within 74 hours. In the female newborn it was possible to provide a normal development with protein restricted diet. In mother's line there are unexplained death's of infants. The examination of amino acids and organic acids excluded other disturbances. The determination of orotic acid in the urines and oral loading with protein indicated to OTC-deficiency. The RDS-symptomatic and pulmonary bleeding in the newborns wit…

Abnormal ornithine carbamoyltransferase in mice having the sparse-fur mutation.

Mice with the X-chromosomal sparse-fur (spf) mutation frequently have urinary bladder stones composed mostly of orotic acid, which was identified by the following criteria: ultraviolet and infrared absorption, spectra, chromatographic behavior, melting point, and reactivity in a specific color test. This clue led to the discovery that spf-bearing mice have an abnormal form of liver ornithine carbamoyltransferase (carbamoylphosphate:L-ornithine carbamoyltransferase, EC 2.1.3.3). Normal ornithine carbamoyltransferase has maximum activity at pH 7.6-8.0 and 80% of maximum activity at pH 10.0.

Predicting the disease severity in male individuals with ornithine transcarbamylase deficiency.

OBJECTIVE: Ornithine transcarbamylase deficiency (OTC-D) is an X-linked metabolic disease and the most common urea cycle disorder. Due to high phenotypic heterogeneity, ranging from lethal neonatal hyperammonemic events to moderate symptoms and even asymptomatic individuals, the prediction of the disease course at an early disease stage is very important to individually adjust therapies such as medical treatment or liver transplantation. In this translational study, we developed a severity-adjusted classification system based on in vitro residual enzymatic OTC activity. METHODS: Applying a ce…

Evaluation of neurocognitive function of prefrontal cortex in ornithine transcarbamylase deficiency.

Hyperammonia due to ornithine transcarbamylase deficiency (OTCD) can cause a range of deficiencies in domains of executive function and working memory. Only a few fMRI studies have focused on neuroimaging data in a population with OTCD. Yet, there is a need for monitoring the disease progression and neurocognitive function in this population. In this study, we used a non-invasive neuroimaging technique, functional Near Infrared Spectroscopy (fNIRS), to examine the hemodynamics of prefrontal cortex (PFC) based on neural activation in an OTCD population. Using fNIRS, we measured the activation i…
