Learning outcomes and practice changes after a postgraduate course in office orthopedics.

Although many of studies of continuing medical education have demonstrated information transfer and some have resulted in practice changes, little is known about the relationships between physician knowledge, intentions, and actual changes in clinical behavior. This paper presents the results of a study of immediate and delayed learning outcomes, including a parallel exploration of physician intentions and practice changes for three tracer procedures, among 83 physicians attending a post-graduate course in office orthopedics. A high level of knowledge gain and retention was found, and consider…

Step count as a digital mobility outcome in orthopedics and orthopedic trauma surgery: a scoping review.

The need to collect objective outcome parameters digitally is increasing in both clinical practice and research. Step count is a frequently utilized digital mobility outcome (DMO) in orthopedic traumatology; however, its usefulness to monitor the patient recovery process remains unclear. The aim of this scoping review is to investigate the application and utility of daily patient step count as a DMO in musculoskeletal injuries. PubMed and consensus.app were queried. Eligibility criteria included the following: articles published within 20 years including patients with orthopedic trauma conditi

A Review of Federal and Statewide Guidelines and Their Effects on Orthopedics.

In the past three decades, the use of opioids has risen tremendously. Pain was named the "fifth patient vital sign" in the 1990s, and from that point, opioid usage has continued to grow throughout the 2010s leading to its recognition as a crisis. The United States is responsible for 80% of the global opioid usage while only accounting for less than 5% of the global population. Previously opioids were mostly used to treat acute pain, however, opioids have been most recently used to manage chronic pain as well. The opioid crisis has presented new challenges in treating pain while preventing the

Advances in Minimally Invasive Techniques in Pediatric Orthopedics: Percutaneous Spine Fracture Fixation.

Pediatric spine trauma presents unique management challenges. These injuries are often the result of high-energy mechanisms and are associated with other serious injuries that can complicate surgical and nonsurgical approaches. The pediatric population presents a host of challenges related to patient compliance, healing challenges, and patient tolerance of therapy. Percutaneous pedicle screw instrumentation, temporary fixation without fusion, continues to expand in its role of pediatric spine fracture treatment. Compared with open instrumentation and fusion, this technique addresses many of th

II Italian intersociety consensus statement on antithrombotic prophylaxis in orthopedics and traumatology.

Pharmacological prophylaxis for preventing venous thromboembolism (VTE) is a worldwide established procedure in hip and knee replacement surgery, as well as in the treatment of femoral neck fractures, but few data exist to its regard in other fields of orthopedics and traumatology. Indeed, no guidelines or recommendations are available in the literature, except for a limited number of weak statements about knee arthroscopy and lower limb fractures. The present paper represents the first multidisciplinary effort to provide suggestions on the prophylaxis of VTE in the remaining fields of orthope…

Extracorporeal shock wave therapy in orthopedics. Assessment of an emerging health technology.

OBJECTIVE: Extracorporeal shock wave therapy (ESWT) as an emerging technology in orthopedics has been assessed in Austria with the objective to establish a scientific basis for pending and pressing health policy decisions. Despite encouraging results within some indications and the promising prospect of a noninvasive treatment for some orthopedic diseases, it seemed crucial to assess this new field of application in the light of evidence-based standards, without forgetting the reality of healthcare decisions. This article presents the results of the above mentioned assessment in the context of…

The Opioid Crisis and the Orthopedic Surgeon.

Opioid use and abuse has become a national crisis in the United States. Many opioid abusers become addicted through an initial course of legal, physician-prescribed medications. Consequently, there has been increased pressure on medical care providers to be better stewards of these medications. In orthopedic surgery and total joint arthroplasty, pain control after surgery is critical for restoring mobility and maintaining patient satisfaction in the early postoperative period. Before the opioid misuse epidemic, orthopedic surgeons were frequently influenced to "treat pain with pain medications

Orthopedic Aspects of the Diabetic Foot.

BACKGROUND: Diabetes Mellitus (DM) is a common medical condition affecting more than 6% of the world population. Besides well-known pathology like diabetic retinopathy and renal insufficiency, it carries important orthopedic implications, especially around the foot and ankle (diabetic foot). FOCUS: This article focusses on the three major areas confronting the orthopedist: 1) changes in foot statics and biology leading to ulceration with or without osteomyelitis, 2) ankle fractures, particularly complications after a fracture and 3) Charcot foot. Their pathophysiology, clinical features, clas

Pericytes for the treatment of orthopedic conditions.

Pericytes and other perivascular stem cells are of growing interest in orthopedics and tissue engineering. Long regarded as simple regulators of angiogenesis and blood pressure, pericytes are now recognized to have MSC (mesenchymal stem cell) characteristics, including multipotentiality, self-renewal, immunoregulatory functions, and diverse roles in tissue repair. Pericytes are typified by characteristic cell surface marker expression (including αSMA, CD146, PDGFRβ, NG2, RGS5, among others). Although alone no marker is absolutely specific for pericytes, collectively these markers appear to sel…
