BACKGROUND: The authors propose an expanded technique of the temporoparietal facial flap of sufficient size to provide complete coverage in a single-stage procedure for the projected three-dimensional autogenous cartilage framework, including the tragus or lobule.
METHODS: A temporoparietal fascial flap measuring 9 x 9 cm was prepared and wrapped around an expander. The expander was gradually infused with saline solution. Approximately 6 months after the first operation, a projected three-dimensional auricular framework was covered by the expanded temporoparietal fascial flap. Nine patients u

PURPOSE: A spontaneous meningoencephalocele of the temporal bone may present with effusion in the middle ear, a cerebrospinal fluid leak, hearing loss, or rarely otitic meningitis. Repair of spontaneous encephaloceles in the temporal bone has been performed using transmastoid and transcranial middle fossa approaches or a combination of the two with varied results. The authors present a technical paper on the transmastoid extradural intracranial approach for the management of temporal lobe encephaloceles.
MATERIALS/METHODS: Case reports and cadaver dissections are used to provide a pictorial e

OBJECTIVE: To present in detail a reconstruction technique using the remnant ear in lobule-type microtia.
METHODS: By comparing the location of residual ear and the contralateral normal ear, we classified 3 different types of the remnant ear. Three techniques of transposition were applied to adjust the location of the reconstructed ear.
RESULTS: The reconstructed earlobes not only looked real but also were symmetrical with the contralateral normal ears.
CONCLUSION: The remnant ear is a key factor-in addition to the framework and covering skin-in ear reconstruction in microtia.

Temporomandibular joint herniation (TMJ) can be caused by inflammation, trauma, tumor, or otologic procedures. However, spontaneous TMJ herniation can also occur as a result of a congenital bony defect in the external auditory canal (EAC), known as the patent foramen of Huschke (PFH), and occurs in 0.4% of the population. Herein, the authors present a case of spontaneous TMJ herniation through the PFH with clicking tinnitus. The patient underwent the surgical repair of bony defect in the EAC with placement of titanium mesh, and the symptom disappeared after surgery. They also review the releva

A report on the stapedectomy procedure using the Robinson stainless steel stapes prosthesis over a 15-year period is presented in a study of 3,200 cases. Short-term results, long-term results, failures, and delayed complications are statistically analyzed. This implant is presently the most frequently used prefabricated stapes prosthesis in the country, and each year more otologic surgeons are adopting this surgical technique.

The surgical treatments for prominent ears are on the basis of the approach over the basics of the deformity decrease the concha-mastoid angle and create a new antihelical fold. The author describes a new retroauricular flap to be used in the recreation of the antihelical fold and critically analyzes the cosmetic results and complications. A skin ellipse is outlined on the posterior surface of the ear. After hydrodissection with anesthetic solution in the outlined area, the skin is removed as thin as possible over the superior mid ear, preserving the underlying connective tissue and perichondr

The surgical approach for ossiculoplasty, also known as ossicular chain reconstruction, is chosen based on the extent of concomitant middle ear or mastoid disease, such as chronic otitis media or cholesteatoma. The primary objective of the surgical approach is to adequately expose the middle ear ossicles, diagnose the ossicular chain pathology, and facilitate reconstruction. Surgeons may employ microscopes and/or endoscopes for visualization and access the ossicular chain through a variety of surgical corridors including the transcanal, endaural, transmeatal, postauricular, and transmastoid/tr

OBJECTIVE: Although congenital middle ear anomalies include various types of ossicular anomalies, all of these can be treated by ossiculoplasty or stapes surgery. Transcanal endoscopic ear surgery (TEES) is a minimally invasive surgical method for middle ear disease with an excellent surgical view that has been widely adopted worldwide. To determine the efficacy of TEES for middle ear anomalies, we describe the surgical results and compare the hearing outcomes between patients treated by TEES and microscopic ear surgery (MES).
METHODS: A total of 39 ears with congenital middle ear anomalies w

BACKGROUND: 30° and 45° endoscopes are commonly used, but there are few reports about the use of 70° endoscopes for ear surgery.
METHODS: The use of 70° endoscopes made it possible to resect cholesteatomas without removing extensive regions of the external ear canal, which facilitated reconstruction, as the areas that needed to be reconstructed were smaller.
CONCLUSION: None of the 34 patients who underwent this procedure for pars flaccida cholesteatoma suffered recurrence. Therefore, we consider that the procedure is useful. A further study of the postoperative outcomes of this procedure sh

PURPOSE: Differences in the definition and classification of cholesteatoma hinders comparing of surgical outcomes of cholesteatoma. Uniform registration is necessary to allow investigators to share and compare their findings. For many years surgical cholesteatoma procedures were divided into two main groups: canal wall up mastoidectomy (CWU) and canal wall down mastoidectomy (CWD). Recently, mastoid obliteration can be added to both procedures. Because of great variation within these main groups, the International Otology Outcome Group (IOOG) proposed the new SAMEO-ATO classification system to

A porencephalic cyst is an aberrant accumulation of cerebrospinal fluid within the brain parenchyma. Its occurrence is rare, with an incidence of 3.5 per 100,000 live births. Etiology is considered the result of perinatal cerebral ischemia or hemorrhage, leading to parenchymal loss. Porencephalic cysts are diagnosed radiologically, and management depends on the clinical manifestation. Our case depicts a porencephalic cyst presenting with nondominant parietal lobe symptoms in adulthood. We hypothesize that a membrane between the cyst and ventricle allowed formation of a 1-way valve that led to

Obstructive Eustachian tube dysfunction (OETD) is a common condition resulting from inadequate opening of the Eustachian tube (ET). A new surgical treatment involves high-pressure inflation of a balloon within the ET, with the aim of dilating the soft tissue structure. However, the mechanical effects of this intervention have not been established, nor the impact of changing device size or other technical parameters. A novel experimental technique allowed quantification of plastic and elastic tissue deformation in model materials and then human cadaver ETs during balloon dilation, based on the

: Superior canal dehiscence (SCD) is a bony defect of the superior semicircular canal (SCC). Patients with SCD Syndrome (SCDS) may experience symptoms such as aural fullness, pulsatile tinnitus, hyperacusis, autophony, or pressure or noise-induced vertigo . The defect can be repaired in various ways, but there is potential for loss of perilymphatic fluid during transmastoid approaches that could result in postoperative sensorineural hearing loss . We hypothesize that if the procedure were performed "underwater" in balanced salt solution (BSS), loss of perilymphatic fluid would be minimized. CA
