Otoacoustic emissions in an aberrant internal carotid artery: a case report.

A 52-year-old Korean woman who complained of impaired hearing and tinnitus of the left ear was diagnosed with an aberrant internal carotid artery (ICA) in the middle ear using a temporal bone high resolution CT (HRCT) scan. Upon otoscopic examination, her left eardrum was found to be in contact with a mass in the middle ear cavity. Upon audiometric evaluation, a mild conductive hearing impairment was documented, and a discrepancy in the results of otoacoustic emissions (OAEs) was found for the left ear. This case report studies the aberrant ICA in the middle ear that revealed abnormal OAE find

Optical Coherence Tomography for Quantitative Point-of-Care Ear Imaging.

OBJECTIVE: To determine whether an optical coherence tomography (OCT) otoscope can provide quantitative morphological measurements of the tympanic membrane (TM) that distinguish abnormal ears from normal controls. METHODS: Patients with ear disease and normal volunteers were imaged using an OCT otoscope. Integrated video otoscopy with simultaneous OCT imaging guided probe positioning. OCT volumes (< 5 s each) were acquired and fused to generate wide-field volumetric images of the TM and middle ear (ME). Datasets were analyzed using semi-automated segmentation to quantify TM thickness and surf

Investigating the use of a two-stage attention-aware convolutional neural network for the automated diagnosis of otitis media from tympanic membrane images: a prediction model development and validation study.

OBJECTIVES: This study investigated the usefulness and performance of a two-stage attention-aware convolutional neural network (CNN) for the automated diagnosis of otitis media from tympanic membrane (TM) images. DESIGN: A classification model development and validation study in ears with otitis media based on otoscopic TM images. Two commonly used CNNs were trained and evaluated on the dataset. On the basis of a Class Activation Map (CAM), a two-stage classification pipeline was developed to improve accuracy and reliability, and simulate an expert reading the TM images. SETTING AND PARTICIP

Hearing loss and other ear problems among schoolboys in Abha, Saudi Arabia.

The aim of this work was to determine the prevalence of hearing loss (HL) and its association with different ear problems among schoolboys in Abha City, Asir Region, Saudi Arabia. A representative sample of 974 schoolboys ages six to 12 years were subjected to both audiometric and otoscopic examinations. Audiometric examination identified 43 children with HL (40 conductive and three sensorineural), making a prevalence of 4.4% (1.7% mild, 2% moderate and 0.7% severe HL). Otoscopic examination revealed ear problems among 26.9% of schoolboys with the following prevalences: wax (21.3%), acute otit

A role for otoacoustic emissions in screening for hearing impairment and middle ear disorders in school-age children.

OBJECTIVE: The primary purpose of this study was to investigate the potential role of transient-evoked otoacoustic emissions (TEOAEs) for screening for hearing impairment and middle ear disorders in school-age children. Because TEOAEs are present in ears with normal cochlear and middle ear function and typically are absent or reduced in ears with cochlear and/or middle ear disorders of even mild degree, TEOAE screening could serve as a first-stage screening to separate from the general population of school-age children those at greater risk for hearing impairment and/or middle ear disorder. Respiratory Epithelial Adenomatoid Hamartoma: A Very Rare Entity Originating from the External Auditory Canal.

Respiratory epithelial adenomatoid hamartoma (REAH) is an uncommon lesion of the upper aerodigestive tract. It is characterized by abnormal glandular formations with ciliated epithelium arising from the nasal cavity, paranasal sinuses and nasopharynx, but other locations have also been described. We did not find any other cases of this hamartomas in the external auditory canal in the literature. A 40-year-old man was referred to the otorhinolaryngology department due to otorrhea and decreased hearing from 1 year ago. Otoscopic examination showed a reddish hemorrhagic mass in the left external

Aberrant sylvian vein: A newly described cause of pulsatile tinnitus.

We herein report a newly described cause of venous pulsatile tinnitus: protrusion of an aberrant sylvian vein into the tympanum. A 60-year-old woman presented with a 4-month history of objective persistent pulsatile tinnitus in the right ear with no other complaints. The pulsatile tinnitus diminished with rotation of the head to the right side or by compression of the right cervical vascular structures. The frequency and intensity of the tinnitus were 125 Hz and 20 dB HL, respectively. Audiometry and otoscopic examination findings were normal. Radiologic examination showed that the right sylvi

Superior canal dehiscence with tegmen defect revealed by otoscopy: Video clip demonstration of pulsatile tympanic membrane.

Superior canal dehiscence is a pathologic condition of the otic capsule acting as aberrant window of the inner ear. It results in reduction of inner ear impedance and in abnormal exposure of the labyrinthine neuroepithelium to the action of the surrounding structures. The sum of these phenomena leads to the onset of typical cochleo-vestibular symptoms and signs. Among them, pulsatile tinnitus has been attributed to a direct transmission of intracranial vascular activities to labyrinthine fluids. We present the first video-otoscopic documentation of spontaneous pulse-synchronous movements of th

Evaluation and Management of Otalgia.

Otalgia can be broadly categorized into primary otologic causes and secondary nonotologic causes. Isolated otalgia in the absence of hearing loss, otorrhea, or abnormal otoscopic findings is typically secondary to referred pain from nonotologic causes, as the sensory nerve supply to the ear arises from 4 cranial nerves and the cervical plexus. The most common causes of primary otalgia are acute otitis media and otitis externa, whereas the most common causes of secondary otalgia are temporomandibular joint disorders and dental pathology. Persistent unilateral ear pain and other alarm symptoms w

Impedance screening. An integral part of hearing screening.

Rationale is presented for including tympanometry as an integral part of all hearing screening programs. Impedance screening will yield more accurate screening results in the identification of middle ear disease than traditional hearing testing or otoscopic examination. Medical complications often result from unidentified otitis media which also contributes to speech, language, and educational delays in children. Although additional research information is needed on the significance of early identification of children with otitis media, it is argued that tympanometry will foster such research

The Natural Course of Tympanic Membrane Retractions in the Posterosuperior Quadrant of Pars Tensa: A Watchful Waiting Policy.

INTRODUCTION: Tympanic membrane retraction (TMR) is a relatively common otologic finding. Currently, there is no consensus on the optimal treatment of TMR. Some ENT-surgeons advocate surgical correction while others opt for a watchful-waiting policy. Our aim was to investigate the natural course of retraction pockets in the posterosuperior quadrant of the pars tensa in a large cohort of patients. METHODS: An observational retrospective cohort study was conducted including patients of all ages with a posterosuperior pars tensa retraction. Primary outcome measure was difference between audiomet

An audiological profile of a cohort of school-aged children with HIV and AIDS attending an antiretroviral clinic in South Africa.

BACKGROUND: Recent estimates reveal that there are approximately 280 000 children between the ages of birth and 14 years who are living with the human immunodeficiency virus (HIV) and acquired immunodeficiency syndrome (AIDS) in South Africa. These children are living with a compromised immune system, are vulnerable to opportunistic infections and subsequent hearing loss. However, there is limited research on the nature and extent of this sensory impairment amongst school-aged children. OBJECTIVE: This study aimed to determine an audiological profile of a cohort of school-aged children attend

Intranasal beclomethasone as an adjunct to treatment of chronic middle ear effusion.

BACKGROUND: Following otitis media, 10% to 50% of children develop residual middle ear effusion with concurrent hearing loss and potential cognitive, behavioral, and language impairment. Prophylactic antibiotics and tympanostomy tubes are currently recommended treatments for chronic middle ear effusion. OBJECTIVE: In a double-blind, placebo-controlled, randomized study of chronic middle ear effusion, we assessed the effectiveness of topical intranasal beclomethasone as an adjunct to prophylactic antibiotic therapy. METHODS: Sixty-one children, aged 3 to 11 years with persistent middle ear ef

The O'Neill grading system for evaluation of the tympanic membrane: A practical approach for clinical hyperbaric patients.

Eustachian tube dysfunction (ETD) and middle ear barotrauma (MEB) are the two most common complications of clinical hyperbaric oxygen (HBO2) treatment. The current grading system, the Teed's Classification, was first described in 1944 with modifications to this system over the years, but none are specific for the evaluation and treatment of patients undergoing clinical HBO2 therapy. Currently, the standard of care is a baseline otoscopic examination performed prior to starting HBO2 therapy. Repeat otoscopy is required for patients having ETD, pain or other symptoms during the compression and/o

[Bilateral external auditory canal cholesteatoma - a case report].

The external auditory canal cholesteatoma is a rare and difficult to diagnose disease. Symptoms are similar to the external ear canal inflammation and can be masked by retained cerumen. In the article we described a case of 22 year old, mentally impairment women with bilateral otorrhea, in the past treated for external ear canal inflammation. The otoscopic examination showed bilateral congenital auditory canal stenosis, with masses obstructing ear canals. Imaging and histopathologic studies revealed bilateral external ear canal cholesteatoma. The patient was qualified for surgery, which, toget

[Imaging of conductive hearing loss].

CT study of the middle ear and ossicular chain plays a major role in assessing the etiology of conductive hearing loss. A normal otoscopic examination is compatible with a fenestral otosclerosis, a minor ossicular chain aplasia or a traumatic dislocation of the ossicular chain. Fenestral otosclerosis diagnosis does not usually require a CT examination except in children without familial history and mixed hearing loss with a transmissional component. In retraction pockets and middle ear cholesteatomas CT allows precise location, extension and defines surgical landmarks. In middle ear tumors CT
