Hypoxemia during hyperbaric oxygen therapy.

This report describes a patient with mucormycosis treated with hyperbaric oxygen therapy (HBO2). Arterial O2 tension (PaO2) during HBO2 was considerably lower (346 mm Hg) than expected (1,500 mm Hg) due to an inadvertent right main-stem intubation, which was not otherwise clinically evident. This case illustrates the influence of a large right-to-left shunt (Qs/Qt) on O2 loading during HBO2. We raise the question if routine hyperbaric treatment protocols are adequate to raise PaO2 in patients with large Qs/Qts.

Oxygen sensing in airway chemoreceptors.

Pulmonary neuroepithelial bodies, composed of innervated clusters of amine- and peptide-containing cells, are widely distributed throughout the airway mucosa of human and animal lungs. Structurally, neuroepithelial bodies resemble chemoreceptors (such as carotid body, taste buds) and are thought to function as hypoxia sensitive airway sensors. Evidence for this is indirect, however, and the mechanism of oxygen sensing by these cells is unknown. Here we culture neuroepithelial bodies isolated from rabbit fetal lungs and identify voltage-activated potassium, calcium and sodium currents using the…

Oxygen radicals in cerebral ischemia.

Superoxide production was measured as the superoxide dismutase (SOD)-inhibitable portion of nitro blue tetrazolium (NBT) reduction after cerebral ischemia-reperfusion in anesthetized cats equipped with cranial windows. Significant superoxide production was found in the early reperfusion period and continued for more than 1 h after ischemia. Superoxide was not detected in control animals not subjected to ischemia, during ischemia, and at 120 min of reperfusion. After ischemia, the vasoconstrictor response to arterial hypocapnia was reduced. This effect was prevented by pretreatment with SOD plu

Oxygen radicals in ulcerative colitis.

This article reviews the pathophysiologic concept that superoxide and hydrogen peroxide, generated by activated leukocytes, together with low-molecular-weight chelate iron derived from fecal sources and from denatured hemoglobin, amplify the inflammatory response and subsequent mucosal damage in patients with active episodes of ulcerative colitis. The putative pathogenic mechanisms reviewed are as follows: (1) Dietary iron is concentrated in fecal material owing to normally limited iron absorption. (2) Mucosal bleeding, characteristic of ulcerative colitis, as well as supplemental oral iron th

[Gas gangrene and hyperbaric oxygen].

From 1964-1987 we have examined 31 patients suspected to have gaseous gangrene. The diagnosis was confirmed 13 times on clinical and microbiologic grounds. 61% of the patients underwent amputation, 28% died. On the basis of this experience we have tried to define a therapeutic attitude. We recommend an adequate debridement, i.e. the removal of necrotic tissue and drainage of the wound, microbiological investigations and the administration of antibiotics before hyperbaric oxygen treatment is begun (2-3 treatments over 3 days).

Long-term home oxygen therapy.

Oxygen therapy improves mortality and morbidity in hypoxemic patients with COPD. For best results, oxygen should be administered continuously. The general guidelines that qualify a patient for long-term home oxygen therapy are rather explicit, although there are some gray areas, such as increasing exercise performance in normoxemic COPD patients and the treatment of dyspnea in certain patients. The physiologic goals of oxygen therapy are to reverse or prevent tissue hypoxia under the various conditions of life. Patients often experience a fall in oxygen saturation during REM sleep and during e…

Oxygen saturation during electroconvulsive therapy.

Oxygen saturation was measured during anaesthesia and electroconvulsive therapy (ECT) using ear oximetry. Significant hypoxia occurred during some treatments; desaturation related to the number of ventilations performed after muscle relaxation and prior to the shock. Desaturation did not correlate significantly with seizure duration. These findings highlight the need for adequate oxygenation during ECT.

Hyperbaric oxygen use. Update 1984.

The fact that as many as 60 new hyperbaric oxygen chambers are being established each year in the United States indicates the extent of interest in this relatively new therapy. In 1983 the Hyperbaric Medicine Committee of the Undersea Medical Society revised its classification of conditions being treated by hyperbaric oxygen (HBO), dividing them into two categories: category 1, conditions for which HBO treatment is known to be effective and is third-party reimbursable, and category 2, conditions for which such treatment is unproved experimentally or clinically and is not reimbursable. In the f…

Free radicals and activated oxygen.

Superoxide anion (0(-2)), hydrogen peroxide (H2O2) and hydroxyl radical (OH.) are products of the biological reduction of 0(2). They are very reactive and poorly tolerated within living systems and enzymes that catalytically scavenge these products have been evolved as defense mechanisms. These include superoxide dismutases (SOD), catalase and peroxidases. Large amounts of O-2 are produced by different enzymatic and non enzymatic biological processes. Large amounts of activated oxygens are produced by phagocytosing cells such as macrophages and polymorphonuclear cells. This production is assoc…
