Head pain.

Head pain can be a challenging, difficult, and interesting diagnostic problem. A detailed history and carefully performed and directed physical and laboratory examination will aid in the complex differential diagnosis--which includes structural disease of the eye, ear, nose, throat, teeth; trauma; central and peripheral nervous system infections and tumors; and various neuropathies of unknown cause. Systemic abnormalities such as hypertension are a frequent cause, as are emotional upset and tension. Other isolated and extremely rare causes, such as the anatomic abnormality of an elongated styl…

Pain Overexposure Affects Self-Pain and Empathy in Athletes.

Experiencing pain repeatedly or over prolonged periods has been shown to affect the pain experience itself. Similarly, people overexposed to others' pain respond differently to expressions of it. Given that pain experience and empathy toward others' pain share cognitive and cerebral mechanisms, examining how both are affected by overexposure to self-pain and others' pain within the same population is of particular interest. Athletes represent one such population. This study thus aimed to compare pain perception and empathy in athletes and non-athletes. Thirty athletes and 28 non-athletes were …

Intraindividual pain variability in chronic pain: A systematic review.

OBJECTIVE: To present the first systematic review on the empirical evidence for variations in intraindividual pain intensity in patients with long-term pain conditions. METHODS: The search was conducted on Medline and included prospective longitudinal non-interventional studies done on adult human subjects with chronic pain conditions linked to nociplastic pain mechanisms. Abstract screening and full-text review were performed by two independent reviewers. A third reviewer was consulted in case of disagreement. RESULTS: Of the 1195 results, 13 studies fulfilled the eligibility criteria as de…

Spatial distribution of pain in complex regional pain syndrome.

Pain often spreads away from the injured site in complex regional pain syndrome (CRPS), but what drives this process is unclear. To explore this, pain drawings were investigated in 136 patients in relation to the type and location of injury, pain intensity and quality, CRPS duration, subtypes, and hyperalgesia to thermal and mechanical stimuli. Areas of pain were quantified using the ImageJ polygon selection tool. The pain area in the affected limb increased in line with pain intensity and with indices of hyperalgesia in the affected limb and ipsilateral forehead ( P 's < 0.01). The pain area …

Neurologic Approach to Radiculopathy, Back Pain, and Neck Pain.

Back pain and neck pain are common in clinical practice, but significant challenges and pitfalls exist in their diagnosis, treatment, and management. From the neurologic standpoint, cervical radiculopathy and lumbosacral radiculopathy are characterized by neck pain or back pain accompanied by sensory and motor symptoms in an arm or leg. The basic neurologic examination is vital, but testing like electromyography and MRI is often needed especially in cases that fail conservative management. Oral medications, injection-based therapies, physical therapy, and surgical evaluation all have a place i…

PAIN IN POST-POLIO SYNDROME: A SEPARATE PAIN ENTITY?

BACKGROUND: Most patients with polio recover from the initial infection, but develop muscle weakness, pain and fatigue after 15-40 years, a condition called post-polio syndrome. Although poliovirus has been almost eliminated, 12-20 million people worldwide still have polio sequelae. The pain is described mainly as nociceptive, but some patients experience neuropathic pain. The aim of this study was to further characterize post-polio pain. PATIENTS AND METHODS: A total of 20 patients with post-polio syndrome participated in the study. Physical examination was performed, and questionnaires cont…

Pain Catastrophizing and Pain Persistence in Temporomandibular Disorder Patients.

AIMS: To describe pain catastrophizing in temporomandibular disorder (TMD) patients in relation to disability and pain persistence. METHODS: A total of 163 TMD patients underwent a complete TMD evaluation according to the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD), including the Pain Catastrophizing Scale (PCS). Patients were divided into subgroups according to their PCS, Graded Chronic Pain Scale (GCPS), and pain persistence scores. The GCPS and pain persistence subgroups were assigned as dependent variables in a stepwise multiple logistic regression model. The ability of t…
