A cohort study of sciatic pain and measures of internal spinal load in professional drivers.

UNLABELLED: In a prospective cohort study of 537 male professional drivers, the occurrence of sciatic pain showed stronger associations with measures of internal lumbar load expressed in terms of daily compressive dose, S(ed) (MPa), and risk factor, R (non-dimensional), according to ISO/WD 2631-5 (2013), than with measures of daily vibration exposure calculated as either 8-h energy-equivalent frequency-weighted acceleration (ms(-2) r.m.s.) or vibration dose value (ms(-1.75)) according to the EU Directive on mechanical vibration (2002). Herniated lumbar disc, previous lumbar trauma and physical

Hypocapnia related changes in pain-induced brain activation as measured by functional MRI.

Stress, acute pain and chronic pain may often result in hyperventilation (HV) which produces hypocapnia. The aim of this fMRI-study was to investigate the influence of hypocapnia on cortical activation during noxious stimulation in 14 healthy volunteers. The intensity of voluntary HV was controlled by capnometry Three tasks were performed in the fMRI sessions: (I) three 3-min HV periods with 7-min periods of recovery in between; (II) mechanically induced phasic pain stimulation--pain task (PT); (III) tapping--motor task (MT). The last two of these protocols were performed under normocapnic and

Developing a valid and reliable measure of health outcome for patients with low back pain.

OBJECTIVE: The authors developed and validated a measure of health outcome for patients with low back pain. RESULTS: A questionnaire was developed incorporating the type of questions asked when taking a clinical history. After testing on a sample of 568 patients, three questions were discarded from the questionnaire. The final questionnaire was found to be reliable and valid in the sense that patients' scores correlated highly with their scores on a general health profile and with GP perceptions of severity. Health status in patients with low back pain was significantly impaired when compared

Period Prevalence Surveys: A New Approach to Measuring Phantom Limb Pain in Amputees.

BACKGROUND: The quantification of phantom limb pain (PLP) is challenging due to the heterogeneity of patient experiences. This study evaluated the use period prevalence questionnaires in assessing PLP in patients with lower limb osseointegrated implant (OI) prostheses. METHODS: The study included patients who received single-stage lower limb OI prostheses at our institution. Period prevalence questionnaires included questions regarding frequency and duration of PLP prior to OI implantation and in the last year postoperatively. The frequency of PLP was assessed by asking participants how often

Neuropathic pain in multiple sclerosis: discrepancies between DN4 screening and neurophysiological measures.

BACKGROUND: Pain is a common and disabling symptom in multiple sclerosis (MS), yet its pathophysiological mechanisms remain poorly understood. Clinical assessment of neuropathic pain relies largely on subjective screening tools such as the DN4 questionnaire. This study aimed to characterize pain phenotype in MS using the DN4 score and to determine its relationship with objective neurophysiological markers of nociceptive and non-nociceptive pathway function assessed by laser-evoked (LEPs) and somatosensory evoked potentials (SEPs). METHODS: Fifty MS patients reporting pain underwent standardiz

Advancing outcome measure development and analytical approaches: Pain in Animals Workshop 2023.

Annually, millions of humans and animals suffer from chronic and acute pain, creating welfare and quality of life concerns for both humans and animals who suffer this pain. In developing new therapeutic approaches, the challenge is to accurately measure this pain to ascertain the efficacy of novel therapeutics. Additionally, there is a need to develop new and effective analgesic options that may offer alternatives to using opioids that contribute to the opioid epidemic. The Pain in Animals Workshop (PAW) meetings are held every other year in partnership with the National Institutes of Health (

Effect of Foot Anthropometric Measurements on Pain and Functional Disability of Pregnant and Nonpregnant Women.

BACKGROUND: Weight gain during pregnancy, loosening of the foot ligaments with relaxin hormone release, and anthropometric and biomechanical changes can be seen in the foot with the growing fetus. We sought to determine whether these pregnancy-related changes cause painful musculoskeletal problems by altering the proximal structures of the lower extremities and the structure of the spine. METHODS: Both feet of 210 women (half in the third trimester of pregnancy) aged 18 to 40 years with similar demographic characteristics were evaluated by measuring navicular drop, hallux valgus angle, foot w

Hair cortisol sampling as a measure of physiological stress in youth with acute musculoskeletal pain.

Stress physiology contributes to health outcomes. Hair cortisol concentration (HCC) is an objective measure of cumulative cortisol secretion associated with health, including pain. The aim of the current study was to describe associations between pre-injury stress physiology (as measured by HCC), acute pain characteristics and relevant demographic factors (i.e., BMI, age, sex, days since injury) in youth with an acute musculoskeletal (MSK) injury. Participants were 58 youth aged 11 to 17 with acute MSK pain. Participants completed self-report measures assessing pain intensity, pain catastrophi

Quality Indicators for Pain in Adults: A Review of Performance Measures by the American College of Physicians.

Pain is a debilitating symptom generally caused by injuries or various conditions. It can be acute, subacute, or chronic and can have a significant impact on a patient's quality of life. The goal of managing pain is to relieve or reduce suffering and improve patient functioning. Several performance measures that address the treatment of pain are used in payment, public reporting, or accountability programs. The American College of Physicians (ACP) embraces performance measurement as a means to improve quality of care. ACP believes that a performance measure must be methodologically sound and e

Identifying risk factors for chronic postsurgical pain and preventive measures: a comprehensive update.

INTRODUCTION: Chronic postsurgical pain (CPSP) is a prevalent condition that can diminish health-related quality of life, cause functional deficits, and lead to patient distress. Rates of CPSP are higher for certain types of surgeries than others (thoracic, breast, or lower extremity amputations) but can occur after even uncomplicated minimally invasive procedures. CPSP has multiple mechanisms, but always starts as acute postsurgical pain, which involves inflammatory processes and may encompass direct or indirect neural injury. Risk factors for CPSP are largely known but many, such as female s

Development and preliminary evaluation of a short self-report measure of generalized pain hypersensitivity.

PURPOSE: Generalized pain hypersensitivity is frequently observed in chronic pain conditions. Currently, identification is based on expert clinical opinion, and in very few cases combined with quantitative sensory testing. The objectives of this study were to develop and evaluate a short self-report measure of generalized pain hypersensitivity: a generalized pain questionnaire (GPQ). METHODS: Items for the GPQ were developed based on a literature review, followed by an interview study with ten rheumatic patients with suspected pain hypersensitivity. We examined the psychometric properties of

Anodyne by Design; Measuring the Prevalence of Esoteric Designer Opioids in Pain Management Patients.

The recent increase in illicit opioids sold on the black market, cut into heroin and masqueraded as prescription pills prompts a significant public health concern. Most designer opioids possess unknown potencies and unknown pharmacokinetics and their unregulated, variable dosages lead to rashes of overdoses. Additionally, many of the designer opioids, especially the fentanyl analogs are significantly more potent than heroin. High-profile cases involving overdoses of U-47700 and carfentanil have been reported in the media; however, the true prevalence of these and other designer opioids is unkn

Q-angle static or dynamic measurements, which is the best choice for patellofemoral pain?

BACKGROUND: The elevated Q-angle seems to be one of the most suggested factors contributing to patellofemoral pain. Females with patellofemoral pain are often evaluated through static clinical tests in clinical practice. However, the adaptations seem to appear more frequently in dynamic conditions. Performing static vs. dynamic evaluations of widely used measures would add to the knowledge in this area. Therefore, the aim of this study was to determine the reliability and discriminatory capability of three Q-angle measurements: a static clinical test, peak dynamic knee valgus during stair asce

A new knee surgery model in rats to evaluate functional measures of postoperative pain.

INTRODUCTION: With the increase in the number of total knee surgeries being performed, postoperative analgesic management remains a challenge. We used a new animal knee surgery model to characterize pain-related behavior in the rat, and its therapeutic modulation with systemic and intrathecal drug treatment. METHODS: Rats were anesthetized with isoflurane and an incision was made over the left knee to expose the patella tendon. The tendon was reflected aside and a 1.4-mm diameter, 0.5 mm deep hole was drilled in both the femur and tibia at 2 mm above and below the knee joint, respectively. Th
