Tuberculosis is a common disease in South Africa, but the diagnosis may be difficult when it presents in the miliary form. Two cases of miliary tuberculosis with pancytopenia are described.

A patient with gamma heavy chain disease (Franklin's disease) was discovered during evaluation for pancytopenia and splenomegaly. Lymphadenopathy, palatal edema, and infiltration of the bone marrow palatal edema, and infiltration of the bone marrow with abnormal cells were all absent. Serum and urine protein electrophoresis demonstrated a monoclonal protein migrating in the beta region. Immunoelectrophoresis showed that it reacted with antibodies against the Fc fragment of IgG heavy chains (gamma chains) but not with antibodies against kappa and lambda but not with antibodies against kappa and…

Autoinflammatory-pancytopenia syndrome (AIPCS) is a rare autosomal recessive disease caused by a mutation in the deoxyribonuclease II (DNASE2) gene. It has mainly haematological manifestations and recurrent fevers. The kidney could be involved in the form of proteinuria, membranoproliferative glomerulonephritis (MPGN) or immune complex deposition rather than acute kidney injury (AKI) or C3 glomerulopathy (C3G). A six-year-old male child presented with high-grade fever, AKI, generalized edema and hypertension. He had a history of recurrent hospital admission with fever, arthritis, skin rash and…

A 71-year-old woman was found to have pancytopenia during a routine health check-up and was referred to a local hospital. Bone marrow examination did not yield a definitive diagnosis, and the cytopenia gradually progressed over the following two years. She was subsequently referred to our institution for further evaluation, including assessment of eligibility for hematopoietic stem cell transplantation. 18F-Fluorodeoxyglucose positron emission tomography (18F-FDG PET/CT) revealed massive splenomegaly without abnormal FDG uptake. Given the concurrent infection suspected to be fungal pneumonia, …

Methotrexate (MTX) is a commonly used immunosuppressant and chemotherapeutic agent, widely prescribed for autoimmune diseases such as psoriasis, rheumatoid arthritis, and certain malignancies. It functions by inhibiting dihydrofolate reductase, leading to impaired DNA synthesis and cell proliferation. While generally well-tolerated, MTX has a narrow therapeutic index, and its adverse effects can be severe, including hepatotoxicity, pulmonary toxicity, and hematological complications such as pancytopenia. Pancytopenia involves the reduction of all three blood cell lines and can result in signif…

Chronic alcoholism is a well-documented cause of folate deficiency, with past studies revealing high prevalence rates among alcoholics. Despite mandatory folate fortification in the UK from 2021, individuals with chronic alcohol consumption remain susceptible to severe folate deficiencies. This case study explores the hematological impact of severe folate deficiency in a 38-year-old female chronic alcoholic who presented with pancytopenia. The patient's symptoms included cough, shortness of breath, lethargy, reduced appetite, constipation, and rectal bleeding. Her medical history included poly…

This case describes a 42-year-old man who underwent kidney transplantation and developed fevers, pancytopenia, and elevated liver function tests starting on post-operative day 9. An extensive microbiologic and molecular workup was performed, ultimately leading to a diagnosis of donor-derived toxoplasmosis with associated hemophagocytic lymphohistiocytosis in the recipient. This case highlights the potential for post-transplant toxoplasmosis in high-risk mismatch (D+/R-) recipients, as well as the role of Toxoplasma-targeted prophylaxis in such patients.

Hydralazine-induced lupus leading to pancytopenia is an uncommon presentation and can have systemic effects on the body. We present the case of a 73-year-old male with complaints of fever, night sweats, and non-intentional weight loss. Complete blood count pathology review showed pancytopenia with no blasts. Detailed infectious disease workup, including Coccidioidomycosis and tuberculosis, was negative. Rheumatological workup including rheumatic factor, anti-smith, anti-double-stranded DNA antibodies, and anti-ribonuclear proteins, was negative. Anti-nuclear antibodies and anti-histone antibod…

A 21-year-old man presented to the emergency department with generalised weakness, weight loss and decreased appetite for few weeks. He had evidence of severe pancytopenia and haemolysis. His peripheral smear with many schistocytes was suspicious for thrombotic thrombocytopenic purpura (TTP). He was supported with blood transfusions and daily plasmapheresis. His platelet counts worsened despite 4 days of therapy. Bone marrow biopsy was significant for hypercellular bone marrow with megaloblastic changes. Further workup revealed normal ADAMTS13 level, low vitamin B12, positive intrinsic factor …

BACKGROUND: Graves disease or other causes of thyrotoxicosis are frequently associated with cytopenia. Although anemia is the most common, other cell lineage can be affected. Pancytopenia is a rare complication of thyrotoxicosis.
CASE PRESENTATION: We report a case of a 33-year-old Chinese man who presented a nonsevere pancytopenia in the context of a newly diagnosed Graves disease. Restauration of euthyroid state led to progressive correction of pancytopenia.
CONCLUSIONS: Literature review shows other rare cases of pancytopenia. It is usually nonsevere with just extremely rare cases of tran…

OBJECTIVE AND IMPORTANCE: We present a case of hemophagocytic lymphohistiocytosis (HLH), giving insight in how to establish diagnosis and start appropiate treatment.
CLINICAL PRESENTATION: A 45-year-old male presented at the emergency ward with high fever and pancytopenia. Repeat bone marrow aspirates showed hemophagocytosis. Extensive work-up with exclusion of other infectious and malignant diseases, eventually lead us to the diagnosis of hemophagocytic lymphohistiocytosis.
INTERVENTION: The patient was treated with Etoposide, Cyclosporine A, and systemic steroids based on the HLH-2004 prot…

Three patients developed pancytopenia in the second month post-allogeneic bone marrow transplantation in association with progressive splenomegaly and a normocellular marrow. Splenectomy resulted in a prompt and sustained improvement in all haematological parameters in all cases. None of the spleens had morphological or cytogenetic evidence of tumour. Hypersplenism should be considered in patients with persistent or recurrent pancytopenia and splenomegaly post-transplantation.

OBJECTIVES: Perinatal illness and preterm birth carry lifelong multiorgan complications and are associated with hematologic derangements dur … : Among 13073 infants, critical illness or prematurity was found to increase the odds of developing pancytopenia post-NICU discharge. Subsequent analyses stratified on prematurity demonstrate that a diagnosis of shock or sepsis was associated with pancytopenia.
CONCLUSIONS: Our findings suggest that perinatal insults are associated with hematopoietic system d …
