We describe three children with panniculitis and associated systemic manifestations including fever. Histopathologic features, such as the presence of lobular or septal inflammation, presence of vasculitis, character of the cellular infiltrate, and presence of erythrophagocytosis, were useful in classifying this group of panniculitides. We present a patient in whom histoplasmosis panniculitis developed during steroid therapy for pancytopenia secondary to myelodysplasia. Although the cutaneous manifestations of disseminated histoplasmosis are rare, we review them because of the increasing numbers of organ-transplant patients, as well as other patients with immunodeficiency, including acquired immune deficiency syndrome, in whom the risk of this unusual presentation of histoplasmosis must be considered.

Panniculitis occurred in a 34-year-old man. The clinical features of the panniculitis were not consistent with erythema nodosum, Weber-Christian disease, or the other previously described panniculitides. Subsequent investigation showed concurrent hepatitis possibly caused by type A hepatitis virus. The skin lesions resolved following treatment with oral indomethacin. We believe this is the first report of a case of nonerythema nodosum panniculitis associated with viral hepatitis.

Pancreatic panniculitis is a rare cutaneous manifestation of pancreatic disease, most commonly occurring in patients with acute or chronic pancreatitis or pancreatic neoplasms. In most reported cases, pancreatic pathology is recognized before or concurrent with the onset of skin lesions; less commonly, panniculitis precedes the diagnosis. Histology demonstrates a lobular panniculitis with saponified fat necrosis and anucleate "ghost" adipocytes, findings that are pathognomonic.
