Intracoronary papaverine induced myocardial lactate production in patients with angiographically normal coronary arteries.

Although intracoronary papaverine has been widely used for the measurement of coronary flow reserve, little is known concerning whether papaverine may produce deleterious metabolic changes in humans. We investigated the effect of papaverine on lactate metabolism in 28 patients with normal coronary arteries. We continuously monitored phasic coronary flow velocity in the proximal left anterior descending coronary artery using Doppler guidewire. We also obtained paired samples of arterial and coronary sinus blood for the measurement of lactate at control and at 1 min after administration of 10 mg…

Decreased blood flow in a papaverine-induced passive vascular bed in sclerodermal skin.

Blood flow was measured in the form of 133Xenon washout rate constants in a cutaneous vascular bed made passive by intracutaneous injection of a 133Xenon-papaverine mixture. Experiments were performed using injection volumes of 0.005, 0.02, 0.1 and 0.2 ml in 7 normals and 15 patients suffering from generalized scleroderma. Blood flow was closely related to the volume of fluid injected, probably reflecting dilution of tissues and increased diffusion distances when injection volumes were increased. When using injection volumes of 0.005 and 0.02 ml, blood flow was significantly reduced in the pat…

Safety and Facial Nerve Outcomes of Intracisternal Papaverine Irrigation for Vestibular Schwannoma Resection.

BACKGROUND: Papaverine, a vasodilator approved for use by the U.S. Food and Drug Administration, has shown efficacy in treating vasospasm in cardiology, urology, and nephrology. The vasodilatory effect of papaverine is also hoped to protect the facial nerve from ischemic damage and nerve manipulation during vestibular schwannoma surgery. Our institution uses intracisternal papaverine irrigation during vestibular schwannoma resection to protect the facial nerve in patients with neuromonitoring changes. Our objective was to investigate the safety and facial nerve outcomes of intracisternal papav…

Periarterial papaverine to treat renal artery vasospasm during robot-assisted laparoscopic partial nephrectomy.

Renal artery vasospasm can be a troublesome complication during robot-assisted laparoscopic partial nephrectomy. Urologists performing this procedure, especially if utilizing selective arterial vascular microdissection, should be aware of using papaverine for both prevention and treatment of renal artery vasospasm. We present a 33-year-old male who developed severe renal artery vasospasm just with hilar dissection causing the kidney to become ischemic. Papaverine was topically applied on the renal arteries resulting in vasodilation and reperfusion of the kidney. Our objective of this report is…

Intracavernous papaverine/phentolamine-induced priapism can be accurately predicted with color Doppler ultrasonography.

OBJECTIVES: To determine the value of penile duplex ultrasonography in predicting the occurrence of intracavernous drug-induced ischemic priapism directly after the rigid erection phase. METHODS: We evaluated 250 men with erectile dysfunction by color Doppler ultrasonography after the diagnostic injection of papaverine hydrochloride and phentolamine mesylate. Cavernosal artery waveforms were recorded, and peak systolic velocity (PSV) and end diastolic velocity (EDV) of the recorded waveforms were measured. In cases of priapism, color Doppler ultrasonography findings were evaluated to identify…

Paradoxical aggravation of vasospasm with papaverine infusion following aneurysmal subarachnoid hemorrhage. Case report.

Reports of intraarterial papaverine infusion as treatment for cerebral vasospasm are few and documented complications are uncommon. The authors report the case of a patient with paradoxical aggravation of cerebral arterial narrowing during selective intraarterial papaverine infusion intended to treat vasospasm following aneurysmal subarachnoid hemorrhage (SAH). A 48-year-old man presented to the authors' service with symptomatic vasospasm 10 days after experiencing an SAH. The ruptured anterior communicating artery aneurysm was surgically obliterated the following day, and thereafter maximum h…

Papaverine-sensitive vasospasm and arterial contractility and compliance after subarachnoid hemorrhage in dogs.

This study examined the relationship between papaverine-sensitive and -insensitive components of vasospasm, arterial contractility and compliance, and time after subarachnoid hemorrhage (SAH) in dogs. Eighteen dogs underwent angiography and then two intracisternal injections of blood. Angiography was repeated 4 (n = 5), 7 (n = 4), 10 (n = 4), or 14 (n = 5) days later. Papaverine, 100 to 200 mg, was infused into the basilar artery, and angiography was repeated. Four additional dogs had cerebral angiography only and served as controls. The basilar arteries were removed and studied pharmacologica…

Diagnosis of vasculogenic impotence: combination of penile xenon-133 washout and papaverine tests.

The present study evaluates both penile xenon-133 washout (XWT) and papaverine tests (PT) in the diagnosis of vasculogenic impotence. XWT was accomplished by subcutaneous injection of xenon-133 (1-2 mCi in 0.1 mL saline solution) into the dorsal coronal prepuce. Abnormal XWT was suggested in patients whose clearance time (T1/2) was longer than 7.5 minutes and whose penile blood flow rate (Q) was less than 6 mL/100 g tissue/min. PT was done by intracavernous injection of papaverine (60 mg in 20 mL normal saline). Abnormal PT was indicated in patients whose onset of full erection was more than t…

Attenuation of neurobehavioural abnormalities by papaverine in prenatal valproic acid rat model of ASD.

Autism spectrum disorder (ASD) is a neurodevelopmental disorder with complex aetiology and phenotypes. Phosphodiesterase-10A (PDE10A) inhibition has shown to provide benefits in various brain conditions. We investigated the role of a PDE10A inhibitor, papaverine on core phenotypes in prenatal-valproic acid (Pre-VPA) model of ASD. In order to identify probable mechanisms involved, the effects on several protein markers of neuronal function such as, neurogenesis-DCX, neuronal survival-BDNF, synaptic transmission-synapsin-IIa, neuronal transcription factor-pCREB, neuronal inflammation (IL-6, IL-1…

Pharmacokinetics of intraluminally administered serum papaverine for spasm prophylaxis of the internal mammary artery.

BACKGROUND: Papaverine (Paveron N™ Linden Arzneimittel Vertrieb GmbH, Germany) is a widely used agent for preventing spasm in mammary artery preparations. The question addressed in this study is whether the intraluminal administration of papaverine can result in detectable absorption of the drug into the systemic arterial circulation. METHODS: In 15 patients (age 65 ± 6 years; body mass index 28.9 ± 3.7), an internal mammary artery (IMA) was prepared during coronary artery bypass grafting (CABG). A maximum of 3 mL of a 1 mg/1 mL diluted papaverine solution was injected intravascularly (intral…

Dilation of the internal mammary artery by external papaverine application to the pedicle--an improved method.

Spasm of the internal mammary artery (IMA) during coronary bypass grafting may cause inadequate graft flow and makes accurate placement of sutures difficult. In addition, IMAs with poor intraoperative flow rates are more likely to occlude. In this study three methods for spasm prevention were compared in 51 patients undergoing coronary bypass surgery. In group 1, IMA pedicles were covered with a sponge soaked with papaverine solution (0.8 mg/ml of papaverine in 0.9% saline) and left intact distally, thus allowing continuous blood flow until used for bypass. In group 2, the IMAs were divided an…

[Treatment of impotence with intracavernous auto-injections: moxisylyte diminishes the risks compared to papaverine].

The alpha-blocking agent Moxisylyte was tested in auto-intracavernous injections (auto-ICI) in 72 impotent patients. The side-effects were compared to those observed in a group of 34 impotent patients treated with auto-ICI of Papaverine during the same period of time. The Moxisylyte auto-ICI improved 83% of the patients including 68% reporting a complete and durable success. Moxisylyte proved to be safer due to a reduced rate of prolonged erections (1.3% of the patients versus 8.8% with Papaverine) and corporeal fibrosis (1.3% versus 32% with Papaverine). Though less potent than Papaverine, an…

[Comparative activity of ethaverine and papaverine on chronotropic and dromotropic cardiac functions in the anesthetized dog].

Ethaverine inhibits both atrioventricular conduction and heart rate, starting at a dosage of 1 mg/kg, injected i.v. into the anaesthetized dog. In the experimental conditions used, ethaverine is clearly different from papaverine, which stimulates both at a dosage between 1 and 10 mg/kg, and which only produces an inhibitory effect starting at a dosage of 20 mg/kg, with the appearance of cardiotoxic phenomena. After total cardiac denervation the inhibitory action of ethaverine persists partially, especially at the atrioventricular level, which is probably due to a direct effect on the heart mus…
