Of 16 patients with anterior spinal tuberculosis treated in this unit during the period 1979-1984, five presented with neurological complications following laminectomy at another hospital. After the decompressive laminectomy all five patients developed an increasing kyphosis, and four of the five patients had deteriorated neurologically. They were transferred to our unit where the four with neurological deterioration underwent surgical stabilisation. This paper illustrates the dangers of posterior spinal surgery in the presence of anterior disease.

Subarachnoid-pleural fistulas caused persistent pleural effusion in 2 patients with traumatic paraplegia. The fistulas were diagnosed by myelography and radioisotope scan. Most of the previously reported cases of this rare entity have been treated by operative closure of the fistula. Both of the reported patients recovered: one underwent insertion of chest tubes to promote drainage and the other had no specific therapy. The authors believe conservative treatment should be attempted before surgery is considered.

Hereditary spastic paraplegia (HSP) comprises a clinically and genetically diverse group of inherited neurodegenerative disorders unified by slowly progressive, usually symmetrical lower-limb spasticity secondary to corticospinal tract degeneration. For practising neurologists, the first step is recognising when a presentation is consistent with HSP and when it is not, and then carefully excluding treatable causes. BACKGROUND: Spinal cord ischemia is a rare but severe complication of thoracic endovascular aortic repair (TEVAR). Prompt diagnosis and cerebrospinal fluid (CSF) drainage are critical for neurologic recovery.

Epidural steroid injections are a common procedure performed by pain physicians. The American Society of Regional Anesthesia along with several other groups recently provided guidelines for performing epidural injections in the setting of anticoagulants. We present a case of a patient who developed an epidural hematoma and subsequent paraplegia despite strict adherence to these guidelines. Although new guidelines serve to direct practice, risks of devastating neurologic complications remain as evidenced by our case.
