Voice range in superior laryngeal nerve paresis and paralysis.

Evaluation of Physiologic Frequency Range (PFR) and Musical Frequency Range (MRP) of Phonation was performed on 56 adults (singers and nonsingers) presenting with superior laryngeal nerve (SLN) paresis or paralysis confirmed by laryngeal electromyography. The most common etiology was neuritis (69.7%), followed by iatrogenic and unknown causes, each accounting for 10.2% of cases, and finally trauma (8.9%). Both female and male singers with SLN paresis or paralysis had significantly higher PFR and MPR than nonsingers. Female classical singers presented PFR and MPR of up to 10 semitones (ST) highe…

Diaphragmatic paresis in newborns due to phrenic nerve injury.

Phrenic nerve lesions as a result of birth trauma have been reported as a cause of acute respiratory distress infrequently. We report recent diagnostic and therapeutic experiences in four newborns with birth-traumatic phrenic nerve injury: one bilaterally, and three unilaterally, all right-sided. In each case, mechanical ventilation was required for at least 16 days. Ultrasound examination of the diaphragm and phrenic nerve conduction studies turned out to be the diagnostic methods of choice. Spontaneous recovery occurred in two children and two became asymptomatic after operative treatment. O

Familial childhood primary lateral sclerosis with associated gaze paresis.

Three children from consanguineous parents began losing the ability to walk in late infancy. Despite chronically progressive weakness leading to wheelchair dependence by adolescence and later loss of motor speech production, intellect remained preserved. Examination revealed upper motor neuron findings of pseudobulbar palsy and spastic quadriplegia, without dementia, cerebellar, extrapyramidal or sensory signs. In addition they exhibited a diffuse conjugate saccadic gaze paresis, especially severe on down-gaze. CT and MRI scans of brain and spinal cord, EEGs, visual and brainstem auditory evok…

[Vocal folds paresis -- our experience in diagnostics and treatment].

In the years 1989-1992 we diagnosed 148 cases of recurrent nerve paresis. There were 78 cases of iatrogenic paresis, mainly caused by thyroid surgery and cardiosurgery. In 48 cases we found the diseases which caused paresis (mainly cancers and lesions of central nervous system). In 22 cases we have not found the cause of the paresis. We used videostroboscopy for diagnostic and monitoring therapy. This method seems to be very valuable because provides very good magnified image of the larynx and serves perfect documentation. The return of mucosal wave is the first sign of reinnervation of vocal …

Paresis of a vertical rectus muscle after cataract extraction.

Nine patients had a permanent paresis of a vertical rectus muscle after cataract extraction. We sought to determine the common factors associated with and their contribution to vertical muscle paresis after cataract extraction. The study design included a survey that was conducted among the referring cataract surgeons and anesthesiologists with particular attention to surgical technique and anesthetic administered, as well as a medical history and examination and appropriate laboratory tests. Patients had a complete ophthalmic examination including prism cover testing in all nine diagnostic po…

Facial sensibility in patients with unilateral facial nerve paresis.

This study evaluated facial sensibility in 29 patients with unilateral lower motoneuron facial nerve paresis using standard clinical tests of sensory evaluation used at other anatomic sites, most commonly the hand. Vibratory and cutaneous pressure thresholds and moving and static two-point discrimination were measured. Statistically significant differences were found between the affected and unaffected sides of the face, with vibration threshold, cutaneous pressure threshold, and static two-point discrimination being greater on the affected side. Vibration thresholds and two-point discriminati…

Iron mydriasis. Pupillary paresis from occult intraocular foreign body.

Two patients presented with unilateral dilated and poorly reactive pupils and were found to have a previously unsuspected intraocular iron foreign body. Both of them had normal vision and one had normal color of the iris so that the condition was missed by several physicians. The pupils reacted normally to pilocarpine 1% and also showed response to dilute (0.1%) pilocarpine. There was no response to phospholine iodide. These findings indicate that the mydriasis was due to a local siderotic parasympathetic neuropathy of the pupil.

Congenital middle ear encephalocele initially seen with facial paresis.

Congenital middle ear encephalocele (CMEE) is a rare entity, previously reported in only 29 patients. It might originate from dehiscence of the tegmen tympani or antri or the bony plate of the posterior fossa. The common presenting symptoms are spontaneous cerebrospinal fluid (CSF) otorrhea and/or rhinorrhea, persistent "serious otitis media," conductive hearing loss, and, occasionally, recurrent meningitis. We report a case of CMEE initially seen with progressive facial paresis, review the previously reported cases of CMEE, and discuss the pathogenesis and surgical management.

[Two symptoms of elective paresis of individual hand muscles].

The author describes 2 symptoms of elective lesions of the hand muscles: one of the central genesis, the other--peripheral, vegetative. The first symptom is characterized by a weakening of volar and dorsal interossal muscles of the hand in an increased electrical activity, which is beyond the weakened muscles. It is due to a lesion of filogenetically new cortico-spinal path, adjacent to the pyramidal and participating in the innervation of the muscles of the distal parts of extremities. The second symptom is "the symptom of the fifth finger", which is characterized by a selective weakening and

Nonconvulsive status epilepticus featuring ictal paresis mimicking a stroke.

PURPOSE: Wilson (1928) and Lennox (1960) described ictal paresis (IP) occurring during prolonged seizures that correspond to the modern definition of nonconvulsive status epilepticus (NCSE). This study aims to characterize this NCSE variant and clearly distinguish it from an acute stroke. METHODS: Three male and three female patients were evaluated from 2015 to 2023. Analysis was restricted to the ictal phase by including only those with neurological deficits at the time of MRI/CT perfusion. RESULTS: Among the six patients, four had hemicorporeal deficits and two had isolated arm deficits. O…

Segmental zoster paresis of the lower extremity: Case report.

Herpes zoster (HZ) is a segmental vesicular eruption, pain, and sensorial symptoms. Segmental motor weakness can rarely be seen as a complication of HZ. Here, we present two cases of motor paresis associated with HZ, case 1 was L2 and L3 segmental motor paresis with femoral neuropathy and case 2 was L5 and S1 segmental motor paresis with sensorial ganglion involvement. In both cases after electrotherapy, exercise program, and medication for pain, there were no motor weakness and pain. Zoster motor paresis is a rare complication that responds to treatment and physicians should be careful about …

Vincristine-induced vocal cord paresis and paralysis in children.

OBJECTIVES: To describe three new cases of vincristine-induced vocal cord paresis or paralysis (VIVCPP) in children and to review the diagnosis and management of this neuropathy. METHODS: Retrospective case series. Diagnosis of VIVCPP was confirmed by laryngoscopy in all children. RESULTS: Less than 20 cases of VIVCPP in children have been previously documented in the literature. Of the three children in our case series, one had unilateral vincristine-induced vocal cord paresis and two had bilateral VIVCPP. The first two patients each had two separate episodes of paresis, lasting 4 months an…

Diagnosis of vocal fold paresis: current opinion and practice.

OBJECTIVES/HYPOTHESIS: No accepted standard exists for the diagnosis of vocal fold paresis (VFP). Laryngeal specialists are surveyed to establish expert opinion on diagnostic methodology and criteria. STUDY DESIGN: Cross-sectional survey. METHODS: Questionnaires were distributed at laryngology conferences in fall 2013. Responses were collated anonymously and subjected to cross-tabulated data analysis. RESULTS: Fifty-eight responses completed by posttraining physicians whose practice focused in laryngology ≥ 75% were analyzed. One (1.7%) relied principally on laryngeal electromyography, one …

Bell's palsy: symptoms preceding and accompanying the facial paresis.

This individual prospective cohort study aims to report and analyze the symptoms preceding and accompanying the facial paresis in Bell's palsy (BP). Two hundred sixty-nine patients affected by BP with a maximum delay of 48 hours from the onset were enrolled in the study. The evolution of the facial paresis expressed as House-Brackmann grade in the first 10 days and its correlation with symptoms were analyzed. At the onset, 136 patients presented postauricular pain, 114 were affected by dry eye, and 94 reported dysgeusia. Dry mouth was present in 54 patients (19.7%), facial pain, hyperlacrimati…

Intracranial tuberculoma presenting as an isolated oculomotor nerve paresis.

BACKGROUND: The differential diagnosis of isolated oculomotor nerve paresis ranges from benign to potentially lethal pathologies. Intracranial tuberculosis (TB), as in the case of this patient, carries a high morbidity and mortality. Early diagnosis is crucial to improve patient outcomes. CASE REPORT: We present the case of a 46-year-old man with a chief complaint of 5 days of diplopia. His examination was remarkable for right inferolateral exotropia and weakness of the right medial rectus. Due to the neurologic findings, we obtained a computed tomography brain scan, which revealed a ring-enh…
