Measuring participation in patients with chronic back pain-the 5-Item Pain Disability Index.

BACKGROUND CONTEXT: Of the three broad outcome domains of body functions and structures, activities, and participation (eg, engaging in valued social roles) outlined in the World Health Organization's (WHO) International Classification of Functioning, Disability and Health (ICF), it has been argued that participation is the most important to individuals, particularly those with chronic health problems. Yet, participation is not commonly measured in back pain research. PURPOSE: The aim of this study was to investigate the construct validity of a modified 5-Item Pain Disability Index (PDI) scor

Individual analysis of patients with HoFH participating in a phase 3 trial with lomitapide: The Italian cohort.

BACKGROUND AND AIMS: The efficacy and safety of lomitapide as adjunct treatment for adults with homozygous familial hypercholesterolaemia (HoFH) have been confirmed in a phase 3 trial. Given the small number of patients (N = 29), and variations in patient characteristics, examining individual cases provides additional details regarding patient management with lomitapide. Here, we examine the details of the Italian patient cohort in the phase 3 trial. METHODS AND RESULTS: The methodology of the multinational, single-arm, open-label, 78-week, dose-escalation, phase 3 trial has been previously r

A study on variability of quantitative sensory testing in healthy participants and painful temporomandibular disorder patients.

OBJECTIVES: Quantitative sensory testing has mainly used thresholds to evaluate somatosensory sensitivity so far. The variability of different measures from session to session has also been investigated, but the variability of the single individual measures of a threshold or subject-based reports has not been considered. This study aimed to investigate the potential value of threshold variability in one session as a measure of internal consistency in somatosensory function. METHODS: The standardized quantitative sensory testing battery developed by the German Research Network on Neuropathic P

Communicating genetics research results to families: problems arising when the patient participant is deceased.

OBJECTIVES: This study explores communication within families of clinically significant genetics research results, after the death of the patient participant. BRCA2 mutations were found in several men after their death from prostate cancer. Spouses were given the results in a genetic counselling session and asked to inform relatives. METHODS: Cross-sectional, qualitative exploratory study. Interviews with 13 relatives, including informers and recipients of the information, were analysed using interpretative phenomenological analysis. RESULTS: Dissemination was hampered when communication cha

Recommendations for physical activity and recreational sports participation for young patients with genetic cardiovascular diseases.

A group of relatively uncommon but important genetic cardiovascular diseases (GCVDs) are associated with increased risk for sudden cardiac death during exercise, including hypertrophic cardiomyopathy, long-QT syndrome, Marfan syndrome, and arrhythmogenic right ventricular cardiomyopathy. These conditions, characterized by diverse phenotypic expression and genetic substrates, account for a substantial proportion of unexpected and usually arrhythmia-based fatal events during adolescence and young adulthood. Guidelines are in place governing eligibility and disqualification criteria for competiti

'The illness isn't the end of the road'-Patient perspectives on the initiation of and early participation in a multi-disease, community-based exercise programme.

BACKGROUND: Exercise is the cornerstone of cardiac rehabilitation (CR). Hospital-based CR exercise programmes are a routine part of clinical care and are typically 6-12 weeks in duration. Following completion, physical activity levels of patients decline. Multi-disease, community-based exercise programmes (MCEP) are an efficient model that could play an important role in the long-term maintenance of positive health behaviours in individuals with cardiovascular disease (CVD) following their medically supervised programme. AIM: To explore patients experiences of the initiation and early partici

Changes in the International Classification of Functioning, Disability, and Health Components "Activity/Participation" as Predicted Through Patient-Reported Outcomes Along With Comprehensive Back Pain Rehabilitation.

OBJECTIVES: The World Health Organization (WHO) recommended the International Classification of Functioning, Disability and Health (ICF) but its use in clinical practice is sparse. This study investigated the limitations and restrictions in the most relevant brief ICF core set categories for chronic low back pain (cLBP) as automatically predicted from routinely measured outcomes using a novel, validated mapping algorithm. MATERIALS AND METHODS: Of 2718 cLBP patients recruited, data from 1541 (64% females) were available from before and at the end of 6 months comprehensive outpatient rehabilit

Physician- and Practice-Level Variation in Clinical Trial Participation Among Patients With Advanced Cancer.

IMPORTANCE: Clinical trial enrollment is essential to advancing cancer care, yet participation among patients with advanced cancer remains low. While patient-level barriers are frequently cited, the role of health care delivery factors-specifically physician- and practice-level variation-has been poorly quantified. OBJECTIVE: To quantify variation in clinical trial participation and identify associated delivery system factors among patients with advanced cancer. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included adults with advanced or metastatic cancers diagnosed and treated betw
