The clinical use of a temporal periosteal bone flap for the reconstruction of a malar bone in a patient with the Treacher Collins syndrome is presented. The temporal muscle functions as an axial carrier of the periosteum that induces osteogenesis in young children, whereas the bone segments may serve as a nucleus for further bone formation from the periosteum. Correction of the eyelid coloboma was obtained by the rotation and advancement of a temporopalpebral flap.

An experimental in vivo model has been developed on the rabbit mandible to study reparative regeneration of alveolar bone after intercortical split osteotomy and the role of the periosteum in the process of reparative osteogenesis. The peculiarity of this model is fixation of the periosteum over the reconstruction zone, which allows preserving the bone block displaced during surgery and leads to the formation of an organotypic structure of the regenerate with cortical and spongy bone.

A lesion similar to that described by Bankart in recurrent dislocation of the shoulder was seen in seven patients with recurrent dislocation of the peroneal tendons. Detachment of the periosteum had resulted in the formation of a false pouch on the surface of the lower end of the fibula; into this pouch the peroneal tendons could easily dislocate. Reattachment of the periosteum to drill holes in the fibula prevented dislocation, and this anatomical method of repair is described. This lesion is one of the causes of recurrent dislocation of the peroneal tendons.

The authors present 2 children in whom a hard palate mucosal graft was used for a defect of the nail bed after resecting subungual exostosis. After the tumor was resected with the overriding nail bed, hard palate mucosa without periosteum was grafted to the nail bed defect. In both patients the graft took completely, and within 2 weeks after the operation the patients were able to enjoy activities of daily life, including athletic movement, without any symptoms. Superior oblique muscle palsy is a frequently encountered acquired muscle deficit accounting for vertical and torsional diplopia. There were two cases of residual isolated superior oblique palsy following anterior ethmoidal artery ligation for epistaxis, a finding not previously reported. The mechanism is suggested to be due either to direct trochlear damage by separation from the periosteum or to localized hematoma formation beneath the periosteum with resultant fibrosis and displacement of the trochlea from its normal apposition to the frontal bone.

Resection of the distal-third of the fibula for tumor may improve important static troubles of the ankle. The authors report an original technique which permits to benefit to osteogenic ability of periosteum to produce new bone in children and teenagers. This technique avoids to take out autograft and permits a complete regeneration of the fibula, using a "lift" procedure. It can be also used when a large bone graft has to be taken in the medial-third of the fibula and when the periosteum is separable enough to be completely preserved.
