Despite major diagnostic and therapeutic advances, postoperative peritonitis appeared to be still associated with a severe prognosis. The failure to react to delayed hypersensitivity skin tests was recently shown to identify patients at increased risk for sepsis. In an attempt to clarify the mechanisms of this anergy, cellular and humoral immunity was studied with in vitro tests in 12 patients treated for postoperative peritonitis. Complement was decreased in 33.3% of cases and normal in the others. No significant change was found in IgG and IgM titres, but IgA concentrations were increased in…

A case of toxic shock syndrome (TSS) in a male continuous ambulatory peritoneal dialysis (CAPD) patient is described. The prodromal phase started with staphylococcal peritonitis and deteriorated to the malignant phase of TSS approximately 48 hours from the time he presented to the local emergency room. Desquamation first affected the extremities and likely involved his gastrointestinal tract. His course of peritonitis involved unusual pain, with progression into toxic shock in spite of appropriate antibiotic coverage.

The selection depends on several factors and in particular: -the means of transport by which the antibiotic reaches the peritoneum : the aminosides and the betalactamines attain the peritoneum easily while the polymyxins do not; -the causative agent: most often it is a gram negative aerobic or anaerobic bacteria. The entero-bacteria predominate but in the last few years Bacteroides fragilis has been frequently encountered giving rise to numerous studies. The flora is most often polymicrobic. The betalactamines act on gram positive bacteria whether they are aerobic or anaerobic. The aminosides

Cefamandole was evaluated as the sole antimicrobial agent used to treat bacterial peritonitis in 113 patients. Dosage varied between 1 and 2 g given intravenously every 6 hr. Laparotomy for excision of infected or gangrenous tissues, closure of gastrointestinal perforations, or drainage of an established abscess was required in 99 of the cases. A good clinical response was obtained in 107 patients, or 95% of the total group. Of the six deaths only one could be attributed to infection. No evidence of renal, hepatic, or hematopoietic toxicity was noted. There were no allergic reactions, although…

A case of actinomyces peritonitis occurring in a young girl undergoing regular dialytic treatment is reported. The peritoneal localization was the only one detectable and occurred in a patient who at the beginning of the dialytic treatment had received peritoneal dialysis on two occasions. During 4 months of treatment the patient underwent surgical drainage of the peritoneal fluid and medical treatment with various antibiotics. Super-infection with Escherichia coli and Candida, and an acute episode of bronchopneumonia, complicated the course of the treatment which was finally successful

Actinomycotic peritonitis in a 5-year-old female German Shorthaired Pointer was characterized by abdominal enlargement, a draining tract in the paralumbar fossa, and chronic debilitation. Clinical recovery resulted from excision of a granulomatous abdominal mass in combination with initial penicillin therapy and prolonged (3 months) sulfonamide therapy.

In a series of 20 patients with bile peritonitis seen during a 15-year period, 15 were men and 5 women, with an average of 72 years. They represented 1.8% of 1123 patients with acute cholecystitis admitted during the same period. Three patients were not operated upon and all died, while 5 of 17 operated patients died. The high mortality rate is due to a delay in diagnosis and treatment. Early operative treatment of acute cholecystitis should be resorted to whenever possible.

Contamination of surgical wounds by starch glove powders has produced peritoneal inflammation and adhesions in many patients. Some have required surgical operation because of possible infection, others because of obstruction of the intestine. Resolving granulomas and fibrosis have been observed as incidental asymptomatic lesions in some patients during reoperation for other conditions. Starch granules are probably capable of producing inflammation in any patient, and the degree is believed to be related to the amount of starch present. Care in removing glove powder from gloves before they are
