Some patients with panic attacks are extremely sensitive to tricyclic antidepressants and develop jitteriness, shakiness, and insomnia when given very low doses. Two patients who developed this "jitteriness syndrome" while taking tricyclic antidepressants are described. The side effects in these patients were markedly relieved by the addition of perphenazine.

We report a patient in whom extreme hyperthermia, rhabdomyolysis, acute renal failure and a residual pancerebellar syndrome occurred while taking a combination of perphenazine and amitriptyline. We postulate that impaired thermoregulation due to psychotropic drugs was responsible for the development of heat stroke and that the cerebellar syndrome resulted directly from the elevated temperature.

A 74-year-old woman developed a slowly progressive hemichorea starting 3 weeks after discontinuation of low-dose amitriptyline and perphenazine. The duration of neuroleptic exposure was 3 months. The chorea was initially well controlled on 5.25 mg/day of reserpine, but after 21 months, required 2.0 mg/day. No complications of reserpine occurred.

Using either haloperidol or perphenazine in a fixed-dose protocol, plasma free homovanillic acid (HVA) and methoxyhydroxyphenethylglycol (MHPG) were decreased in 37 nonorganic psychotic inpatients at neuroleptic steady state (7-9 days) in comparison with pretreatment values. The data indicate that the magnitude of the decline in HVA and MHPG was associated with treatment response and not with neuroleptic plasma levels.

Prompt diagnosis of the uncommon Clerambault syndrome is important. We describe a 62-year-old woman with this disorder who developed a delusional conviction that a 65-year-old male physician was very much in love with her. She was treated by her family physician with perphenazine and psychotherapy. Her mental status improved and her delusion about the physician ceased.

An 8-year-old child was given perphenazine to prevent vomiting after surgery and developed an acute dystonic reaction; There were features of both tetany and tetanus without any of the classical features of extrapyramidal disturbance. A case of successful suicide from overdose of amitriptyline, perphenazine, and midazolam is described. Postmortem findings were inadequate to explain the death. Sudden cardiac arrest suggested that the death from overdose probably resulted from drug cardiotoxicity. The physicians should be aware of this serious complication when prescribing a combination of these potential lethal drugs. A limited supply should be given to depressed patients.

Intrathecal diamorphine is associated with a high incidence of emetic symptoms. Six anti-emetic drugs representing various chemical groups were given in random order to patients undergoing total hip replacement and who had received intrathecal diamorphine 0.5-1.0 mg. The phenothiazines, perphenazine and prochlorperazine, were more effective than the others. It is suggested that this might be a useful model for the evaluation of new anti-emetics.
