Normal weight individuals who develop type 2 diabetes: the personal fat threshold.

Type 2 diabetes (T2DM) is frequently regarded as a disease of obesity and its occurrence in individuals of normal body mass index (BMI) is often regarded as indicating a non-obesity-related subtype. However, the evidence for such a distinct, common subtype is lacking. The United Kingdom Prospective Diabetes Study (UKPDS) cohort of people diagnosed with T2DM in the 1970s and 1980s had a median BMI of only 28 kg/m2. UKPDS data form the basis of current understanding of the condition even though one in three of those studied had a BMI of less than 25 kg/m2. BMI, though, is a population measure an

The future of NMR metabolomics in cancer therapy: towards personalizing treatment and developing targeted drugs?

There has been a recent shift in how cancers are defined, where tumors are no longer simply classified by their tissue origin, but also by their molecular characteristics. Furthermore, personalized medicine has become a popular term and it could start to play an important role in future medical care. However, today, a "one size fits all" approach is still the most common form of cancer treatment. In this mini-review paper, we report on the role of nuclear magnetic resonance (NMR) metabolomics in drug development and in personalized medicine. NMR spectroscopy has successfully been used to evalu

Anxious personality is a risk factor for developing complex regional pain syndrome type I.

The aim of this study was to investigate the role of psychological factors in the development of complex regional pain syndrome (CRPS) type I following the fracture of the distal radius. Fifty patients (average age 57.70 ± 13.43 years) with a distal radius fracture were enrolled in the present study. All of the patients were treated by closed reduction and cast immobilization. The Toronto Alexithymia Scale-20, Anxiety Sensitivity Index, State-Trait Anxiety Inventory, and Beck Depression Inventory were used to determine the patients' psychological features 2 days after the fracture. The patient

Reliability of clinical guideline development using mail-only versus in-person expert panels.

BACKGROUND: Clinical practice guidelines quickly become outdated. One reason they might not be updated as often as needed is the expense of collecting expert judgment regarding the evidence. The RAND-UCLA Appropriateness Method is one commonly used method for collecting expert opinion. We tested whether a less expensive, mail-only process could substitute for the standard in-person process normally used. METHODS: We performed a 4-way replication of the appropriateness panel process for coronary revascularization and hysterectomy, conducting 3 panels using the conventional in-person method and

Drug development in solid tumors: personal perspective of Dr. Emil J Freireich's contributions.

The development of chemotherapy for patients with the major cancers progressed from the initial success attained in the treatment of acute leukemias and choriocarcinoma. Many of the principles of therapy were based on the concepts developed in the experimental laboratories and early clinical studies done at the NIH Clinical Center and other centers around the country. The purpose of this review is to describe some of the early advances in cancer therapy and show how many are based on the efforts of Dr. Emil J Freireich. Over his career, Dr. Freireich has published more than 500 papers and work

Preventing opportunistic infections in human immunodeficiency virus-infected persons: implications for the developing world.

More than 18 million persons in the world are estimated to have been infected with human immunodeficiency virus (HIV), the cause of the acquired immunodeficiency syndrome (AIDS). As immunodeficiency progresses, these persons become susceptible to a wide variety of opportunistic infections (OIs) The spectrum of OIs varies among regions of the world. Tuberculosis is the most common serious OI in sub-Saharan Africa and is also more common in Latin America and in Asia than in the United States. Bacterial and parasitic infections are prevalent in Africa; protozoal infections such as toxoplasmosis,

Incidence of persons with a learning disability detained in police custody. A needs assessment for service development.

The aim of this prospective study was to identify the number of people with a possible learning disability suspected of having committed a criminal offence who were taken into police custody during a defined period of time. Between 18 January 1993 and 18 March 1993 992 people were taken into custody at Parkside Police Station, Cambridge, of whom 251 (25.3%) were screened by the custody officer using a brief questionnaire to ascertain the presence or absence of reading and writing difficulties and to see if they had received extra help at school or if they had attended a special needs school. [Development of carcinoma of Vater's ampulla in an adenoma. Analysis of a personal case load and review of the literature].

A retrospective analysis was undertaken of our case material of 40 histological preparations of carcinoma of the ampulla of Vater, as well as a survey of the literature of 103 adenomas of the ampulla of Vater. Signs of malignant transformation were detected in 25% of the adenomas, whilst adenomatous residues were identified in 72% of the examined carcinomas of the ampulla of Vater. Thus, an adenoma-carcinoma sequence seems to be as significant in the case of Vater's papilla as in the colo-rectal region. An epidemiological investigation confirms this hypothesis since 1. adenomas of the papilla

Development and validation of interpretability cutoffs for the short forms of the Brain Injury Rehabilitation Trust Personality Questionnaires (BIRT-PQs).

BACKGROUND: Previous analyses demonstrated that Italian versions of the five short forms of the Brain Injury Rehabilitation Trust Personality Questionnaires (SF-BIRT-PQs) showed strong psychometric properties. However, evidence on their interpretability lacks. AIM: To calculate cutoffs for each of the SF-BIRT-PQ to discriminate between the subject's behavior that deviates significantly (D+) or not (D-) from the average of healthy individuals. DESIGN: Multicenter prospective study. POPULATION: One hundred and fifty-four subjects with acquired brain injury, their 154 caregivers, and 120 healt

The role of youths' cardiac autonomic balance and parental responses to youth emotion in vulnerability to borderline personality disorder development.

Developmental models of borderline personality disorder (BPD) emphasize the effects of youths' biological vulnerabilities and their experiences of parental responses to emotion, as well as the interaction between these two elements. The current study evaluated the independent and interactive effects of two indices of autonomic nervous system response and parental responses to youth negative emotions on severity and exacerbation of youths' BPD features during the transition to adolescence. The sample consisted of 162 psychiatric youth (10-14 years; 47.2% female) and their parents. At baseline,

Development and Preliminary Evaluation of a Rapid Screening Tool for Detecting Borderline Personality Disorder in People Aged over 60 Years.

OBJECTIVES: Screening and diagnostic instruments for Borderline Personality Disorder (BPD) are not validated in people aged over 60. We report a pilot study examining the sensitivity and specificity of a de-novo screening instrument in older adults. METHODS: The BPD-OA screening tool incorporates DSM 5 and literature describing the expression of BPD in older adults. This study was conducted using a case control design. The Diagnostic Interview for Borderlines-Revised (DIB-R) and the McLean Screening Instrument for BPD (MSI-BPD) were used as comparators. Comprehensive assessment by psychiatric

Psychometric Evaluation and Refinement of the Level of Personality Functioning Scale-Self Report: Development of the LPFS-SR Short-Forms.

The DSM-5 Alternative Model for Personality Disorders (AMPD) provides a dimensional framework for assessing personality dysfunction, and the Level of Personality Functioning Scale-Self Report (LPFS-SR) is a widely used instrument for evaluating the construct. However, its psychometric properties and practical feasibility remain areas of concern due to its extensive item length and structural misspecification. A sample of 344 participants was recruited to assess the psychometric properties of the LPFS-SR using bifactor modeling and item response theory (IRT) analyses. We replicated general fact

Is the development of personality beliefs associated with levels of childhood traumas and emotional intelligence? A prospective cross-sectional analysis.

BACKGROUND: Personality disorders (PD) are associated with personality beliefs (PBs). Childhood traumas (CTs) and emotional intelligence (EI) are effective in the development of PDs. However, the relationships between CTs and EI and PBs are not well known. This study examined the effects of CTs and EI on PBs. METHOD: Three hundred and eighty-seven university students who applied to the psychiatry clinic and did not receive any psychiatric diagnosis according to Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) diagnostic criteria were recruited for the study. Evaluating chronic suicide risk with the Personality Assessment Inventory: Development and initial validation of the Chronic Suicide Risk Index (S_Chron).

The current study sought to develop and validate a new measure of chronic suicide risk (the S_Chron) from the Personality Assessment Inventory in a mixed sample of psychiatric inpatients and outpatients. In an initial development sample (N=397), hierarchical logistic regression identified six PAI variables uniquely associated with multiple versus single/no prior suicide attempts after controlling for sample demographics: Negative Impression Management, Situational Stress, Mania - Grandiosity, Borderline - Negative Relationships, Borderline - Self-Harm, and Antisocial Behaviors. These indicator

Multifinality in the development of personality disorders: a Biology x Sex x Environment interaction model of antisocial and borderline traits.

Although antisocial personality disorder (ASPD) is more common among males and borderline PD (BPD) is more common among females, some author … the two disorders are high. In this article, we present a common model of antisocial and borderline personality development. We begin by reviewing issues and problems with diagnosing and studying PDs in children and adolescents. Next, we discuss dopaminergic and serotonergic mechanisms of trait impulsivity as predisposing vulnerabilities to ASPD and BPD. Finally, we exte …

The Mini Version of the Level of Personality Functioning Scale-Brief Form (Mini-LPFS): Development and Validation in Two Clinical Samples.

This study developed and validated a mini (4-item) version of the Level of Personality Functioning Scale-Brief Form 2.0 (LPFS-BF 2.0) to assess personality functioning as defined in the DSM-5 Alternative Model for Personality Disorders. Two samples were used: a U.S. clinical sample (N = 700) and a Dutch mixed clinical sample (N = 298). In the U.S. sample, participants completed the LPFS-BF 2.0, the Big Five Inventory-2-Extra-Short Form, and several mental health measures. Item combinations that spanned each of the four LPFS elements were evaluated using confirmatory factor analyses and graded

Genome-Wide Association Studies of Delay Discounting and Impulsive Personality Traits in Children From the Adolescent Behavior and Cognitive Development Study.

Impulsivity, often operationalized as delay discounting (DD) and as impulsive personality traits via the UPPS-P scales, is a key transdiagnostic construct across psychiatric disorders. Recent genome-wide association studies (GWAS) have studied the genetic basis of impulsivity in adults, but it remains unclear how similar the genetic architecture of DD is in children. The present study conducted GWAS of DD and impulsivity traits in 5548 children (ages 9-10 years old) of genetically inferred European ancestry from the Adolescent Brain Cognitive Development (ABCD) Study. Heritability estimates fo

The empirically derived Violence and Aggression Risk Index from the Personality Assessment Inventory: Development, validation, and application in general psychiatric settings.

The current study developed a new index of violence risk from scales within the Personality Assessment Inventory (PAI). Clinical patients from inpatient and outpatient settings completed the PAI along with reports of violence history and arrest record. A forward-selection hierarchical logistic regression was employed (N = 388) to identify the PAI scales (AGG-P, AGG-A, NON, PAR-H, and ANT-A) that could effectively distinguish patients with and without a history of violence. These indicators were then aggregated into a single index, labeled the Violence and Aggression Risk Index (VARI). The VARI

Personality predictors of the development of elementary school children's intentions to drink alcohol: the mediating effects of attitudes and subjective norms.

The authors tested a mediation model in which childhood hostility and sociability were expected to influence the development of intentions to use alcohol in the future through the mediating mechanisms of developing attitudes and norms. Children in 1st through 5th grades (N=1,049) from a western Oregon community participated in a longitudinal study involving 4 annual assessments. Hostility and sociability were assessed by teachers' ratings at the 1st assessment, and attitudes, subjective norms, and intentions were assessed by self-report at all 4 assessments. For both genders, latent growth mod
