Psychiatric disorders among drug dependent subjects: are they primary or secondary?

The relationship between substance use disorders and comorbid psychiatric conditions was investigated among 425 persons in drug treatment who met DSM-III-R criteria for drug dependence. Using the NIMH Diagnostic Interview Schedule to ascertain DSM-III-R psychiatric disorders among these drug dependent subjects, lifetime prevalence rates were 64% for alcohol abuse/dependence, 44% for antisocial personality disorder, 39% for phobic disorders, 24% for major depression, 12% for dysthymia, and 10% for generalized anxiety disorder. We found that antisocial personality disorder and phobias generally

Knocking on prison's door: a 10-fold rise in the number of psychotic prisoners in Finland during the years 2005-2016.

BACKGROUND: As in many European countries, Finnish psychiatric services experienced a rapid process of deinstitutionalization in the 1990s. In recent decades, the decrease in numbers of psychiatric hospital beds has in several countries been found to be linked with increasing criminality among severely mentally ill individuals. It has been concluded that deinstitutionalization could be the main reason for this development. AIM: To investigate whether the prevalence of severely mentally ill persons to prison is a growing trend also in Finland. METHODS: We searched for the annual data in elect

A family study of outpatients with borderline personality disorder and no history of mood disorder.

While several studies have examined psychiatric disorders in the relatives of individuals with borderline personality disorder, many of these studies have not employed a family study methodology and suffer from other methodological shortcomings. Thus, the conclusions from family data addressing the validity of borderline personality disorder, its relation to other conditions, and its distinction from mood disorders, continue to be debated. The present investigation employed a family study design with direct interviews with relatives, structured diagnostic interviews with both probands and rela

Personality in adolescents with chronic fatigue syndrome.

Our aim was to study the presence of personality traits and disorder in adolescents with Chronic Fatigue Syndrome (CFS). Personality was then compared to other measures of functioning such as presence of psychiatric disorder and rating on the Child Behavior Checklist 4-18 (CBCL) and in relation to CFS outcome. Twenty-five adolescents with CFS followed-up after contacts with tertiary paediatric/psychiatric clinics were compared with 15 matched healthy controls. Interviews and questionnaires from parents and youngsters included Personality Assessment Schedule (PAS), Kiddie-SADS Psychiatric Inter

APA's model law: a commitment code by and for psychiatrists.

The author argues that the APA model law is seriously flawed because it lacks sufficient mechanisms for questioning the judgment of psychiatrists throughout the commitment process and for ensuring the best disposition of patients. By failing to provide for independent screening of commitment petitions, to mandate multiple psychiatric evaluations of respondents, to provide indigent respondents a free psychiatric examination to help them prepare for the commitment hearing, and to address the shortcomings of legal advocacy, the model law sets the stage for improper or unwarranted commitments. Overweight and obesity are associated with psychiatric disorders: results from the National Epidemiologic Survey on Alcohol and Related Conditions.

OBJECTIVE: This study evaluated associations between body mass index (BMI) and psychiatric disorders. METHODS: Data from 41,654 respondents in the National Epidemiologic Survey on Alcohol and Related Conditions were analyzed. Psychiatric correlates of snuff and chewing tobacco use.

Compared to the association between cigarette smoking and psychiatric disorders, relatively little is known about the relationship between smokeless tobacco use and psychiatric disorders. To identify the psychiatric correlates of smokeless tobacco use, the analysis used a national representative sample from the National Epidemiologic Survey on Alcohol and Related Conditions (NESARC) wave 1. Smokeless tobacco use was classified as exclusive snuff use, exclusive chewing tobacco, and dual use of both snuff and chewing tobacco at some time in the smokeless tobacco user's life. Lifetime psychiatric

Involuntary detention and treatment of the mentally ill: China's 2012 Mental Health Law.

The long-awaited Mental Health Law of China was passed on 26 October 2012 and took effect on 1 May 2013. Being the first national legislation on mental health, it establishes a basic legal framework to regulate mental health practice and recognizes the fundamental rights of persons with mental disorders. This article focuses on the system of involuntary detention and treatment of the mentally ill under the new law, which is expected to prevent the so-called "Being misidentified as mentally disordered" cases in China. A systematic examination of the new system demonstrates that the Mental Healt

Tertiary-level interventions for hard-core smokers.

It has become more commonplace to find that today's smokers fit into one of four target populations: (1) persons with a history of cessation failures; (2) persons with a history of other addictive disorders; (3) persons with smoking-related diseases; and (4) persons with a history of psychiatric impairment. This article addresses the need to expand and create a new methodology for smoking cessation to suit these patients. Interventions geared toward a tertiary level of treatment are offered. This article also examines treatment within the context of phase-specific interventions.

Mental health disparities between Spanish and foreign prison inmates: reasons for referral from prison to a specific unit in a general hospital.

OBJECTIVES: Spain has one of the highest incarceration rates in Europe, and its prison population shows a significantly higher prevalence of mental illness and toxic substances use disorder compared to the general population. Despite this, there is a lack of studies focused on mental disorders in Spanish prisons. This study aims to explore the differences between Spanish and foreign persons deprived of liberty with severe mental disorders. MATERIAL AND METHOD: We evaluated the sociodemographic (Spanish vs. foreign) and clinical characteristics of 106 persons deprived of liberty admitted to a

[Qualitative survey of the law of 5 July 2011 in the field of psychiatry].

The law of 5 July 2011 concerning the rights and protection of subjects in psychiatric care and the modalities of their management has been severely criticized and often rejected, both before and following its application in August 2011. This study was designed to describe and compare perceptions of this law by actors in the field, six months after application of the law. This qualitative study based on twenty-four interviewees from Isère and Savoie - including psychiatrists, judges, patients and families - shows that this law is not completely rejected in practice and that it provides a numbe

European Psychiatric Association (EPA) guidance on forensic psychiatry: Evidence based assessment and treatment of mentally disordered offenders.

Forensic psychiatry in Europe is a specialty primarily concerned with individuals who have either offended or present a risk of doing so, and who also suffer from a psychiatric condition. These mentally disordered offenders (MDOs) are often cared for in secure psychiatric environments or prisons. In this guidance paper we first present an overview of the field of forensic psychiatry from a European perspective. We then present a review of the literature summarising the evidence on the assessment and treatment of MDOs under the following headings: The forensic psychiatrist as expert witness, ri

Are patients with coexisting mental disorders more likely to receive CABG surgery from low-quality cardiac surgeons? The experience in New York State.

BACKGROUND: Presence of a mental disorder has been shown to be associated with reduced access to medical and surgical services. Little is known, however, about the relationship between mental disorders and the quality of medical/surgical care received. METHODS: We analyzed New York State hospital discharge data between 2001 and 2003 and New York's publicly-released Cardiac Surgery Report of surgeons' risk-adjusted mortality rate, to evaluate whether differences exist between persons with and without mental disorders (specifically, psychiatric and/or substance-use disorders) in receiving care

Psychiatric comorbidity in temporal DNET and improvement after surgery.

BACKGROUND: "Alien tissue" may be responsible for a higher frequency of psychiatric disorders in patients with temporal lobe epilepsy (TLE). Also, ganglioglioma and dysembryoplastic neuroepithelial tumors (DNET) could represent a risk-factor for the development of post-surgical psychoses. Classically, severe psychiatric disorders contra-indicate epilepsy surgery. OBJECTIVES: Assessment of inter-ictal psychiatric disorders in 10 consecutive patients with temporal DNET, before and after epilepsy surgery with a minimum of a 2-year follow-up evaluation. METHODS: DNETs were confirmed on histologi
