Racial differences in the evaluation of pediatric fractures for physical abuse.

CONTEXT: Child maltreatment is a significant problem within US society, and minority children have higher rates of substantiated maltreatment than do white children. However, it is unclear whether minority children are abused more frequently than whites or whether their cases are more likely to be reported. OBJECTIVES: To determine whether there are racial differences in the evaluation and Child Protective Services (CPS) reporting of young children hospitalized for fractures. DESIGN, SETTING, AND PATIENTS: Retrospective chart review conducted at an urban US academic children's hospital among

The factors of child physical abuse in Korean immigrant families.

OBJECTIVE: This study explores Korean immigrant mothers' attitudes toward child physical abuse based on an ecological perspective. METHOD: One hundred and forty-four Korean immigrant mothers who came to the US after age 16 and have at least one child under 18 years old participated in this study. Data were collected using instruments translated in Korean that measure mothers' attitudes toward child physical abuse in four areas: degree of agreement with physical abuse, conflict tactics, belief in the use of physical punishment, and perceptions regarding physical abuse. RESULTS: This study fou…

The emerging problem of physical child abuse in South Korea.

South Korea has had remarkably high incidence and prevalence rates of physical violence against children, yet the problem has received only limited public and professional attention until very recently. This article represents the first attempt in English to systematically analyze South Korea's recent epidemiological studies on child maltreatment. Discussed are sociocultural factors that have contributed both to delays in child protection laws and a low public awareness of the problem of child abuse. The article highlights methodological issues concerning the definition of physical abuse in So…

Childhood sexual and physical abuse. Incidence in patients with vulvodynia.

OBJECTIVE: To compare the incidence of sexual and/or physical abuse in women with vulvodynia (chronic, burning vulvar pain in the absence of clear medical findings) as compared to women with chronic vulvar symptoms due to specific, objective vulvar disease and with women from a general dermatology practice. STUDY DESIGN: An invitation to participate in this questionnaire study was sent to 300 women over 18 years of age from a vulvo-vaginal clinic and 280 women from a general dermatology practice. These questionnaires asked for basic demographic information as well as information on childhood …

Effects of sexual and physical abuse: a comparison of adolescent inpatients.

The present study was designed to collect descriptive data on a sample of hospitalized adolescents who were victims of sexual and/or physical abuse and to compare these groups to patients without a history of abuse. A questionnaire was administered to 51 inpatient teenagers at Fort Logan Mental Health Center, a state psychiatric hospital in Denver, Colorado. Information was obtained about family background, type, extent and duration of abuse, drug and alcohol abuse and self-reported mental health and general adjustment problems. In addition, clinical staff completed the Revised Behavior Proble…

Developmental play ages of physically abused and nonabused children.

Although the specific effects of physical abuse are largely unknown, studies have shown deficiencies in the social behaviors of physically abused children. Because play is an important developmental process influencing socialization, physical abuse may adversely affect the developmental play age of children. This study compared the developmental play age of two groups of children aged 1 to 5 years, 12 of whom were physically abused and 12 of whom were not physically abused. Children's play behaviors were recorded using the Preschool Play Scale. Deficits in developmental play age and play imita…

Socioeconomic position and adverse childhood conditions in child physical abuse.

Child physical abuse (CPA) is a serious public health concern with lifelong adverse consequences. Socioeconomic position (SEP) is a well-established determinant of adverse exposures. However, evidence of its role as a risk factor for CPA and its interaction with other risk factors remains limited. This study aimed to identify the effect of SEP on CPA and how SEP might condition other risk factors, comparing police-reported and forensically examined children with the background population in Denmark. We extracted register-based data on 356 children who underwent forensic examinations in Copenha…

Association between missed appointments and hospitalization for child physical abuse.

Missed cases of child physical abuse (CPA) persist despite known risk factors. Prior studies have not evaluated missed medical appointments as a risk factor for CPA. The objective of this study was to determine if an association exists between missed appointments and hospitalization for CPA. We conducted a 20-year, single health system, retrospective chart review of hospitalized patients ≤36 months of age meeting International Classification of Diseases (ICD) 9/10 criteria for CPA with ≥1 scheduled appointment in our system prior to their admission. Cases were categorized as definite CPA, high…

Head computed tomography in suspected physical abuse: time to rethink?

BACKGROUND: National guidance recommends CT-head for all children <1 year old with suspected physical abuse, and to be considered for those <2 years old to exclude abusive head trauma. OBJECTIVES: To investigate whether this guidance is followed, and the associations between clinical presentation and CT findings, to determine whether guidance could be refined. MATERIALS AND METHODS: A retrospective case note review of all children <2 years old who underwent medical assessment for suspected abuse (2009-2017). Outcome measures were frequency of CT-head, and diagnostic yield of intracranial inj…

A life-threatening presentation of child physical abuse: jejunal perforation.

Intra-abdominal injuries from impacts are the second most common cause of death in battered children. However, it may be difficult to distinguish between accidental abdominal injury and abuse, especially in the absence of other clinical findings. Published reports are also limited about the diagnosis of abuse in children with intra-abdominal injury. We report a case with jejunal perforation, multiple soft tissue injuries, and occipital fracture secondary to child abuse who was initially admitted to our hospital with complaint of fever, cough, and vomiting. An exploratory laparotomy revealed pe

Childhood physical abuse, aggression, and suicide attempts among criminal offenders.

Childhood physical abuse (CPA) has numerous short and long-term negative effects. One of the most serious consequences of CPA is an increased risk for suicide attempts. Clarifying the mechanisms by which CPA increases risk for suicidal behavior may enhance preventive interventions. One potential mechanism is a tendency toward aggression. In a sample of 266 criminal offenders, ages 18-62, we examined the relationships among CPA, lifetime aggression, and suicide attempts and tested lifetime history of aggression as a mediator of the relationship between CPA and suicide attempts. Results indicate…

[Total duodenal section. A reliable indicator of severe physical abuse].

BACKGROUND: Rupture of the hollow viscera due to battered child syndrome is an unusual clinical finding and it is even less likely when we encounter total duodenum section. The literature on child abuse does not include visceral lesions as part of the spectrum. The aim of this study was to analize if complete duodenal section is a reliable indicator of severe physical abuse. Data were statistically analyzed using chi-square tests, likelihood ratios and the Cochran-Mantel-Haentzel test. CLINICAL CASES: Four children were admitted with a battered child syndrome diagnosis. RESULTS: All particip…

Child physical abuse and rhabdomyolysis: case report and literature review.

An 8-year-old child who was physically abused and developed rhabdomyolysis is presented. A literature review revealed 13 previously reported cases of child physical abuse and rhabdomyolysis. Including the patient in this report, 13 of 14 children had extensive soft tissue muscle injuries to the buttock or legs. Nine patients developed acute renal failure, but none required renal dialysis treatment. A simple urine dipstick test can be used to screen children at risk for rhabdomyolysis. The importance of early recognition and treatment is made clear by a review of the pathophysiology of rhabdomy…

Sexual and physical abuse among Korean female inpatients with schizophrenia.

The study reports the prevalence and correlates of sexual and physical abuse in a sample of 100 female inpatients with schizophrenia in South Korea. Clinical assessment interviews consisted of the Positive and Negative Syndrome Scale, the Scale for the Assessment of Positive Symptoms, an interview assessing history of sexual and repeated physical abuse, and demographic and psychosocial factors. Lifetime prevalence of severe sexual or physical abuse was 52%. Compared with nonabused patients (n = 48), the abused patients (n = 52) showed more nonpsychotic symptoms (i.e., anxiety, depression) and …
