[Psychotic and non-psychotic manifestations of alcoholism in forensic psychiatry].

The paper presents the results of examination of 110 alcoholic patients who have committed criminal actions and were recognized as irresponsible at forensic examination. It was established that wide spectrum of mental disorders were present in such cases--from superacute psychotic states (15 patients) and acute disorders (49) to chronic psychoses (33) and encephalopathy (13). According to clinical manifestations mental disorders correspond in such cases to reactions of exogenic type. In contrast to general medical departments where patients with alcoholic delirium prevail, the studied sample o

Central anticholinergic syndrome after ofloxacin overdose and therapeutic doses of diphenhydramine and chlormezanone.

A 14-year-old female with no history of psychiatric disease ingested an unknown amount of ofloxacin, diphenhydramine and chlormezanone after an argument with her patients. Approximately 12 hours after ingestion, the patient was admitted to the hospital in a delirious state with extreme mydriasis and warm and dry skin. Analytical data on admission were consistent with ofloxacin overdose and ingestion of therapeutic doses of diphenhydramine and chlormezanone. The patient received activated charcoal and forced diuresis was instituted. Psychosis and anticholinergic symptoms lasted in the next 2 da

Side effects of clozapine.

In addition to the low risk of agranulocytosis, several more frequent side effects are associated with clozapine therapy. We tried to estimate the incidence of these side effects. We analysed 391 treatments in 315 inpatients, who received clozapine alone or combined with other neuroleptic and antidepressant drugs. Two thirds were combined treatments, one third were treatments with clozapine alone (i.e., no other neuroleptic, antidepressant or anticonvulsive drugs were allowed). The numbers in brackets show the incidence based on the analysis of the treatments with clozapine alone. In 49% (61%)

Carbon disulphide poisoning with increased ethereal sulphate excretion.

A clear-cut change in urinary sulphate excretion has been found in a case of subacute carbon disulphide poisoning, the ethereal fraction being greatly increased to 1·59 g./l., that is, 70% of the total urinary sulphate. Three weeks later the figure was still significantly abnormal: 1·25 g./l., i.e., 59·6% of total sulphate excretion. The normal pattern of sulphate excretion was reached about six weeks after the last exposure: ethereal sulphate 0·20 g./l. (8·7%); inorganic sulphate 1·97 g./l. (85·7%); neutral sulphur 0·13 g./l.

[Drug screening among patients aged 17-40 admitted with psychosis].

BACKGROUND: High frequency of co-occurring substance use disorders and psychiatric disorders is reported in several studies. An important issue is whether we can rely on patient reports only. The purpose of this investigation was to find the prevalence of substance use among younger psychotic inpatients in Norway and to compare patients' self reports with the results of screening for drugs. MATERIAL AND METHODS: 65 patients aged 17 to 40, consecutively admitted to Blakstad Psychiatric Hospital in 2001 with psychosis were interviewed with regard to substance use 30 days prior to admittance. Characteristics of freebase cocaine psychosis.

Psychosis was present in 29 percent of cocaine-disordered patients hospitalized in 1985 during an epidemic of freebase cocaine abuse in the Bahamas. Record reviews revealed that a variety of psychotic phenomenologic patterns were present. Prior major mental disorders and increased dosage of cocaine were more common among psychotic than non-psychotic patients. Violent behavior was common among cocaine patients, especially those with psychosis. We conclude that freebase cocaine psychosis is neither rare nor benign.

LSD 'flashback' as a cause of diagnostic error.

An emaciated, but otherwise physically normal young man presented with an acute psychosis resembling hallucinogenic drug abuse. His behaviour was so strange that the underlying pathology of severe pyloric stenosis was only detected when a chance measurement of urea and electrolytes was made, revealing gross biochemical abnormalities. His abnormal mental state persisted for more than one week and an LSD 'flashback' was postulated as the cause of the prolonged psychosis
