IMPORTANCE: Psychotic experiences, such as hallucinations and delusions, are reported by approximately 5% to 10% of the general population, although only a small proportion develop psychotic disorders such as schizophrenia. Studying the genetic causes of psychotic experiences in the general population, and its association with the genetic causes of other disorders, may increase the understanding of their pathologic significance.
OBJECTIVES: To determine whether genetic liability to psychotic experiences is shared with schizophrenia and/or other neuropsychiatric disorders and traits and to ide

GOAL: The aim of this study is to assess to what extent psychomotor assessment can aid the clinician in differentiating between schizophrenia and other psychotic disorders.
METHODS: Enrolled subjects were recent in remission patients (n=304), who all met DSM-IV (APA, 2013) criteria for either schizophrenia (Sz; n=117), schizoaffective disorder (SaD; n=36), psychotic disorder not otherwise specified (P-NOS) (n=86), substance/medication-induced psychotic disorder (SIPD; n=33) or major depressive disorder with psychotic features (MDD-p; n=32). The patients were submitted to a psychomotor test ba

Patients with acute psychotic disorders are often considered as inappropriate candidates for cardiac surgery as well as for other surgical interventions. Post-operative care and patient compliance, which are the main problems associated with such patients, are the most important issues for conventional cardiac surgery. Robot-assisted cardiac surgery may be a new solution in this respect. In this report we aimed to present our acute psychotic patient with serious mitral insufficiency secondary to huge atrial myxoma, treated with robotic cardiac surgery.

OBJECTIVE: Cases of narcolepsy in association with psychotic features have been reported but never fully characterized. These patients present diagnostic and treatment challenges and may shed new light on immune associations in schizophrenia.
METHOD: Our case series was gathered at two narcolepsy specialty centers over a 9-year period. A questionnaire was created to improve diagnosis of schizophrenia or another psychotic disorder in patients with narcolepsy. Pathophysiological investigations included full HLA Class I and II typing, testing for known systemic and intracellular/synaptic neurona

BACKGROUND: Persons with schizophrenia and other psychotic disorders have a high prevalence of obesity, impaired glucose tolerance, and lipid abnormalities, particularly hypertriglyceridemia and low high-density lipoprotein. More detailed molecular information on the metabolic abnormalities may reveal clues about the pathophysiology of these changes, as well as about disease specificity.
METHODS: We applied comprehensive metabolomics in serum samples from a general population-based study in Finland. The study included all persons with DSM-IV primary psychotic disorder (schizophrenia, n = 45;

Translational research is urgently needed to turn basic scientific discoveries into widespread health gains and nowhere are these needs greater than in conditions such as schizophrenia and other psychotic disorders. In this article, we discuss one type of translational research--called T1--which is needed to take advantage of developments in the basic neurosciences and translate them into more efficacious diagnostic, preventive, and therapeutic interventions. However, ensuring that interventions from T1 research actually benefit patients will require a second form of translational research--ca

BACKGROUND: Evidence suggests that, as a group, patients with schizophrenia have intellectual deficits that may precede the manifestation of psychotic symptoms; however, how successfully intelligence tests are able to discriminate schizophrenia from other psychotic disorders has yet to be investigated in detail.
METHODS: Using Wechsler Adult Intelligence Scale-Revised (WAIS-R) data for 55 inpatients with schizophrenia and 28 inpatients with non-schizophrenic psychotic disorders (NSPD) (schizophreniform disorder, brief psychotic disorder, delusional disorder, psychotic disorder due to a genera

OBJECTIVE: To illustrate and discuss issues relevant to treatment of four elderly psychotic patients with multiple comorbidities, including a history of mood disorder and some level of movement disorder.
PARTICIPANTS: Four patients diagnosed with a major mood disorder and comorbid Parkinson's disease were treated successfully with adjunctive ziprasidone at doses of 80 to 160mg/d. Two of the patients had major depressive disorder, recurrent, with psychotic features (Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition [DSM-IV] DSM-IV 296.34) and two had bipolar disorder, not o

RATIONALE: Short- and long-term compliance to prescribed antipsychotic drugs is of particular concern in regard to medication choice and treatment outcome in the care of psychotic disorders.
OBJECTIVE: We evaluated patient-related and treatment-related factors associated with medication compliance in inpatients with a diagnosis of schizophrenia, schizoaffective disorder, or other psychotic disorder.
METHODS: Within a naturalistic study in seven psychiatric hospitals, individuals with a psychotic disorder were assessed weekly on mental state, social functioning, side effects, and medication c

Existing diagnostic criteria for catatonia have been based exclusively on theoretical assumptions. The present study aimed to develop empirical criteria for diagnosing catatonia. The same patient population (n=187) described in Part I (Peralta, V., Cuesta, M.J., Motor features in psychotic disorders. I. Factor structure and clinical correlates. Schizophr. Res., 00, 000-000) was used in this study. Fourteen catatonic signs with potential diagnostic value were cluster-analyzed to derive groups with and without a catatonic syndrome. Taking the cluster solution as the reference standard, the diagn

BACKGROUND AND OBJECTIVES: Cannabis use is an increasing phenomenon worldwide and represents a controversial issue, as it has direct implications in the development of psychosis. There are no widely accepted tools of evaluating the risk for users and therefore no prevention strategies. As psychosis and schizophrenia are two major psychiatric disorders, developing a screening tool is of great need. The current study evaluates the feasibility of the cannabis experience questionnaire 6 (CEQ-6) questionnaire as a screening tool for cannabis users that are at high risk of developing psychosis or sc

IMPORTANCE: As cannabis becomes more accessible and socially accepted, concerns have grown about its potential implications for adolescent mental health. While prior research has linked adolescent cannabis use to psychiatric symptoms, few large, population-based, longitudinal studies have examined associations with clinically diagnosed psychiatric disorders.
OBJECTIVE: To evaluate whether adolescent cannabis use is associated with an increased risk of incident psychotic, bipolar, depressive, and anxiety disorders during adolescence and young adulthood.

BACKGROUND: Schizophrenia (SCH) is a severe neuropsychiatric disorder associated with complex neurobiological alterations and suboptimal therapeutic outcomes. Increasing evidence indicates that mitochondrial dysfunction may contribute to SCH pathophysiology through impaired energy metabolism, oxidative stress, altered calcium homeostasis, and dysregulation of neuronal function. This review aims to summarize current knowledge regarding mitochondrial abnormalities in schizophrenia and to explore their potential involvement in the mechanisms of action of antipsychotic drugs.

IMPORTANCE: Antipsychotics are increasingly used in the perinatal period. However, evidence of their effectiveness in the perinatal population is limited.
OBJECTIVES: To examine the risk of severe psychiatric relapse during the perinatal period associated with discontinuation of antipsychotic treatment before or during pregnancy in women with primary psychotic disorders or bipolar disorder.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study used linked Danish and Swedish national birth registers to identify pregnancies leading to singleton live births from 1998 to 2022 in Denmark and from

Suicidal behavior is prevalent with first psychotic episodes, but reports of associated factors involve inconsistent findings and emphasis on schizophrenia. We evaluated suicide attempt rates and associated risk factors in 395 first-episode patients with various DSM-5-TR diagnoses with psychotic features, comparing 83 suicidal patients to others. Suicide attempt risk averaged 21.0%, with the final diagnosis ranked as follows: major depression, bipolar I depression, bipolar I mixed features, schizoaffective-depressed, unspecified psychosis, schizophrenia, schizoaffective-bipolar, bipolar I mani

INTRODUCTION: Anti-LGI-1 limbic encephalitis, the second most common autoimmune encephalitis, typically presents with psychotic symptoms. However, systematic characterization of psychotic features in this disease remains scarce, with insufficient granularity in existing symptom descriptions. This systematic review aims to characterize the psychotic manifestations in anti-LGI-1 encephalitis through a comprehensive analysis of published cases and institutional data.
METHODS: Following PRISMA guidelines, we systematically searched PubMed, Embase, and Web of Science for case reports of anti-LGI-1

BACKGROUND: The profiles of cortical gyrification across schizophrenia, bipolar I disorder, and schizoaffective disorder have been studied to a limited extent, report discordant findings, and are rarely compared in the same study. Here we assess gyrification in a large dataset of psychotic disorder probands, categorized according to the DSM-IV. Furthermore, we explore gyrification changes with age across healthy controls and probands.
METHODS: Participants were recruited within the Bipolar-Schizophrenia Network of Intermediate Phenotypes study and received T1-MPRAGE and clinical assessment. G

Prevailing factor models of psychosis are centered on schizophrenia-related disorders defined by the Diagnostic and Statistical Manual of Mental Disorders and International Classification of Diseases, restricting generalizability to other clinical presentations featuring psychosis, even though affective psychoses are more common. This study aims to bridge this gap by conducting exploratory and confirmatory factor analyses, utilizing clinical ratings collected from patients with either affective or nonaffective psychoses (n = 1,042). Drawing from established clinical instruments, such as the Po

OBJECTIVE: There are ongoing discussions on updating various standard psychiatric terms, including schizophrenia, which can be confusing, and personality disorders, which can be pejorative. To contribute to this process, suggestions and recommendations on terminology were sought from academic psychiatrists with substantial clinical experience.
METHODS: In an online survey, 263 psychiatrists were asked how often they used alternative instead of standard terms for the diagnosis or symptom description of psychotic disorders and DSM Cluster B personality disorders. They were also asked what speci
