Myasthenia gravis in Latin America and the Caribbean: epidemiology, resources, and accessibility to diagnosis and treatment.

Latin-American and Caribbean (LAC) countries face persistent challenges in achieving universal health coverage, with out-of-pocket healthcare expenditures posing a significant barrier. Access to care for rare diseases, such as myasthenia gravis (MG), is significantly impacted. We conducted an exploratory descriptive study between LAC countries, and we assessed the availability of diagnostic and therapeutic resources for MG across 12 LAC countries, along with associated costs and access barriers. We found marked inequalities regarding the availability and coverage of diagnostic tests and therap

Comparative Assessment of Out-of-Pocket Health Expenditure in Haemodialysis and Peritoneal Dialysis Patients.

Background: Dialysis is a life-sustaining treatment for patients with end-stage renal disease (ESRD), but it requires high financial costs due to the need for continuous treatment and the associated expenses of medical supplies, equipment, and related care. Objective: This study aims to compare the out-of-pocket (OOP) health expenditure incurred by patients undergoing haemodialysis (HD) and continuous ambulatory peritoneal dialysis (CAPD). Methods: The data for this observational cross-sectional study were obtained through a survey at two public hospitals, comprising a sample of 220 ESRD patie

Advanced Therapy Medicinal Products: Availability, Access and Expenditure in Italy.

BackgroundAdvanced therapy medicinal products (ATMPs) are an innovative output of biomedical research, characterized by a high level of uncertainty on long-term efficacy and safety, elevated price tags and often complex administration. All these elements compounded make their European authorization, national price negotiation for reimbursement and subsequent dispensation and administration to the patient less straightforward and often less successful than for less innovative drugs. To assess if these hurdles have affected patient access and how are ATMPs used in Italy, we have analysed availab

Gambling Participation, Expenditure and Risk of Harm in Australia, 1997-1998 and 2010-2011.

Gambling-related harm results primarily from financial losses. Internationally Australia continues to rank as the largest spending nation per capita on gambling products. This would suggest that Australian gamblers are at disproportionately high risk of harm despite almost two decades of industry scrutiny and regulation, and investment in research, treatment and education programs. However, declines in participation rates, per capita expenditure, household expenditure, national disposable income spent on gambling and problem gambling rates have been cited as evidence that fewer people are gamb

Experience implementing a university-based mass immunization program in response to a meningococcal B outbreak.

Neisseria meningitidis serogroup B (MenB) has caused several recent outbreaks of meningococcal disease on US college campuses. In January 2015, a case of MenB was reported at a university in Oregon, culminating in an outbreak with a total of 7 cases (including 1 fatality) identified over a 5-month period. In response to the outbreak, the university organized a mass immunization campaign with 4 "opt-in" immunization clinics. The preparation, challenges, and resources required for organization and implementation of a mass immunization program in response to an outbreak at a large public universi

Out of Pocket Expenses for Birthing in Public and Private Birthing Facilities Among Women Aged 15 to 49 Years in Bihar, India.

Institutional delivery refers to the birth of a child in public and private health facilities under the supervision of trained, skilled healthcare professionals, thereby enhancing neonatal survival and reducing maternal mortality rates. The low- and middle-income nations continue to have substantial disparities in the proportion of pregnant women accessing healthcare facilities to give safe and healthy births at affordable out-of-pocket (OOP) expenditure. The entire research work aims to analyse the delivery expenses incurred for availing institutional & non-institutional health services and t

FADIS: Fast-food attributed diabetes index study: An ecological State-level exploration of nutritional transitions and diabetes burden in India.

Background and objectives India's rapid urbanisation, economic liberalisation, and rising fast food consumption are driving a nutritional transition that parallels the growing epidemic of non communicable diseases, particularly type 2 diabetes mellitus. This study examines the association between fast food expenditure and diabetes prevalence across 18 Indian States, stratified by gender and urban-rural residence. Methods The fast-food attributed diabetes index study (FADIS) utilised publicly available State-level data to evaluate correlations between fast food spending and diabetes prevalence.

The accuracy of VO2 max, heart rate and energy expenditure measurements from Google Pixel Watch 3.

Consumer wearables enable scalable physiological assessment and pre-symptomatic disease detection. However, validation of wearable data remains insufficient in scope and rigour, hindering integration into clinical and public health practice. Here, we evaluated the validity of VO2max, heart rate and energy expenditure from the Google Pixel Watch 3. Thirty-four participants completed a maximal exercise treadmill test using indirect calorimetry, while wearing a Polar H10 chest strap. Pixel Watch estimates of VO2max were generated in free-living conditions; heart rate and energy expenditure estima

Striving Towards National Lower-Risk Gambling Guidelines: An Empirical Investigation Among a Sample of Swedish Gamblers.

Several countries, including Canada and Australia, have developed public health-based lower-risk gambling limits to differentiate lower-risk from higher-risk gambling. This study aimed to identify a preliminary set of lower-risk gambling limits (gambling frequency, duration, expenditure, expenditure as a proportion of personal net income, and diversity), and investigate if gambling types are linked to additional harms, in a Swedish context. The study involved secondary analyses of two online survey studies using the Gambling Disorder Identification Test (GDIT). Receiver operating curve analyse

Demographic Change and the Future of Austria's Long-Term-Care Allowance: A Dynamic Microsimulation Study.

Background/Objectives: Europe's demographic shift is putting increasing pressure on long-term care (LTC) systems and raising concerns about the sustainability of LTC financing. In this paper, we analyse Austria's LTC system, particularly its universal long-term-care allowance (LTCA), and aim to project LTCA expenditure under different future scenarios. Methods: We use a dynamic microsimulation model to project LTCA expenditure under four scenarios up to the year 2080. Combining LTCA statistics with pooled data from the Survey of Health, Ageing and Retirement in Europe (SHARE), we estimate care

Pathways to Care, Out- of- Pocket Expenditure, and Associated Factors Among Breast Cancer Patients in Kerala, India: A Hospital-Based Cross- Sectional Study.

BACKGROUND: Breast cancer, the most common cancer among women in India, poses a significant public health challenge. However, data on pathways to care and out-of-pocket expenditure (OOPE) among breast cancer patients are limited in India. Kerala, despite its high literacy rates and excellent healthcare access, continues to experience significant delays in cancer care, making it an important setting for this study. Therefore, we aimed to study the pathways to care, estimate OOPE, and identify factors associated with OOPE among breast cancer patients in the Indian state of Kerala. METHODS: We c

Policy-relevant differences between secondhand and thirdhand smoke: strengthening protections from involuntary exposure to tobacco smoke pollutants.

Starting in the 1970s, individuals, businesses and the public have increasingly benefited from policies prohibiting smoking indoors, saving thousands of lives and billions of dollars in healthcare expenditures. Smokefree policies to protect against secondhand smoke exposure, however, do not fully protect the public from the persistent and toxic chemical residues from tobacco smoke (also known as thirdhand smoke) that linger in indoor environments for years after smoking stops. Nor do these policies address the economic costs that individuals, businesses and the public bear in their attempts to

Environmental Quality, Renewable Energy, and Life Expectancy in Gulf Cooperation Council Countries.

Life expectancy is a key indicator of public health and sustainable development in Gulf Cooperation Council (GCC) countries, where rapid economic growth, urbanization, and fossil-fuel dependence create environmental and health challenges. This study examines the determinants of life expectancy in six Gulf Cooperation Council countries from 2000 to 2023, focusing on death rates, renewable energy consumption, gross domestic product (GDP) per capita growth, government health expenditure, and carbon dioxide (CO2) emissions. The empirical strategy combines cross-sectional dependence and slope heter

The effect of community-based health insurance on healthcare utilization and out-of-pocket health expenditure in the Tigray Regional State, Ethiopia.

Community-based health insurance (CBHI) is increasingly recognized as an effective mechanism for protecting low-income populations from health-related financial shocks and advancing universal health coverage (UHC) in developing countries. This study evaluates the effect of CBHI on healthcare utilization and out-of-pocket (OOP) health expenditures in the Tigray region of Ethiopia. To evaluate this effect, a cross-sectional survey was conducted in 2020 among 800 households (400 CBHI members and 400 non-members) selected through multistage random sampling. In the first stage, three woredas were p

How Stable Are Individual Preferences for Health Expenditure in Germany?

BACKGROUND/OBJECTIVES: This study analyzed the stability of individual preferences for the allocation of expenditure in the healthcare system using an experimental setting. Understanding these preferences can support policy decisions aimed at achieving a more needs-based allocation of scarce resources in healthcare systems. Stability in preferences might be essential in order to avoid frequent legislative changes and can potentially enhance public satisfaction with the healthcare system. METHODS: Individual preferences were assessed through two questionnaire-based experimental studies conduct

Community water fluoridation policy and population oral health: a policy analysis.

Dental caries (tooth decay) remains the most common noncommunicable disease worldwide and continues to impose significant dental and systemic health and economic burdens despite being largely preventable. Untreated disease contributes to pain, infection, reduced quality of life, and increased demand for restorative dental care. Governments, therefore, face policy decisions regarding population-level prevention strategies. This analysis evaluates policy options related to population-level fluoride exposure, including maintaining existing community water fluoridation (CWF) programs, expanding fl
