A 43-year-old white woman presented to the emergency department with confusion, agitation, and progressive dyspnea. Chest x-ray revealed pulmonary edema. Initial diagnostic considerations were pneumonia, pulmonary embolism, sepsis, central nervous system infection, substance toxicity, and heart failure. METHODS: Cases, case series, and related articles on the subject identified through a comprehensive literature search were examined.

A 61-year-old man with hypertrophic cardiomyopathy developed acute pulmonary edema 29 h following cardioversion of chronic atrial fibrillation to sinus rhythm. Doppler echocardiographic evaluation of atrial function showed return of right atrial contraction but absent left atrial systole. This has not been reported previously in a case of postcardioversion pulmonary edema.

PAE can occur as a consequence of heart failure. Depending upon the extent of left ventricular failure, pulmonary edema can vary widely in severity. To anticipate potential health care needs and, ultimately, to treat PAE, the critical care nurse identifies common nursing diagnoses. Once the nurse understands the physiologic or psychological mechanisms for the particular nursing diagnoses, he or she can serve as a reference point to identify expected nursing outcomes.

A case of maternal pulmonary edema is presented as a late complication after intravenous administration of ritodrine in combination with dexamethasone for the suppression of premature uterine contraction and for the stimulation of fetal lung maturity. Swan-Ganz catheterization showed normal pulmonary capillary wedge pressures. Two-dimensional echocardiography disclosed good ventricular performance. The patient recovered very soon in 2 days post termination of pregnancy. Relative fluid overload might probably be responsible for the development of pulmonary edema in this case.

To assess the course and prognosis of salicylate-induced pulmonary edema, we reviewed the records of 36 consecutive patients admitted with serum salicylate levels greater than 30 mg/dL.
