Pulmonary valve stenosis represents the most common congenital heart defect of dogs and appears to be increasing in prevalence due to the growing popularity of brachycephalic breeds. Current treatments include beta-blockade and balloon pulmonary valvuloplasty, though evidence-based approaches to this disease are lacking. Balloon pulmonary valvuloplasty is most effective for fused, doming valves leaving a large population of dogs with thick, dysplastic valves that fail to respond adequately to balloon dilation. Transpulmonary stent implantation is an emerging therapy to consider for dogs with v…

Management of aortic valve (AV) disease in the pediatric population is challenged by a lack of attractive valve replacement options. The Ross procedure has been prominent for these patients; however, long-term reports have revealed concerns, namely neoaortic root dilation and regurgitation. More recently, AV neocuspidization has emerged as an alternative to the Ross operation or other valve replacement procedures in pediatric patients with congenital AV disease. We report a successful case of AV neocuspidization of a previously implanted pulmonary autograft.

Right ventricular outflow tract (RVOT) dysfunction is common following surgical repair of tetralogy of Fallot and other forms of complex con … nal devices undergoing clinical trials. Hopefully, within the not-too-distant future, transcatheter pulmonary valve implantation will be feasible in the vast majority of patients with RVOT dysfunction following surgical repair of congenital heart disease.

With rapid advancements in imaging technology, cardiac computed tomography (CT) and magnetic resonance (MR) imaging are increasingly being used for anatomic evaluation, functional assessment, and pathologic diagnosis of the pulmonary valve and right ventricle. MR imaging is especially helpful in evaluating postoperative pulmonary valve function and grading of pulmonary regurgitation. On the other hand, CT has the advantage of high-resolution isovolumetric whole-chest coverage and is able to depict anatomic detail of the pulmonary valve, perivalvular structures, and pulmonary artery branches.

BACKGROUND: Absent pulmonary valve with ventricular septal defect is a rare cardiac malformation. The aim of our study is to specify the anatomic characteristics and the clinical and echocardiographic features of this cardiac malformation and to discuss its management.
METHODS: We report 8 cases of absent pulmonary valve with ventricular septal defected collected over a period of 24 years. The diagnosis was made during the first year of life in all cases in the presence of respiratory symptoms and/or cyanosis. It was confirmed by cardiac catheterization in 4 cases and echocardiography in 4 ca…

A 37-year-old man with stenosis of a bioprosthetic pulmonary valve underwent percutaneous balloon valvoplasty. After the procedure the patient had moderate pulmonary regurgitation which has been well tolerated. The patient remains well with good functional capacity on follow-up at 2 years. Percutaneous balloon dilatation may be effective as a palliative procedure in the management of stenosis of a bioprosthetic valve in the pulmonary position.

Extracardiac valved conduits represent one of the weakest facets of reconstructive surgery for congenital heart disease in that they invariably need to be replaced because of growth of the patient or because of valve or conduit failure. Between 1979 and 1989, 141 patients had 169 valved conduits placed between the heart and the pulmonary artery circuit. There were 81 male and 60 female patients, aged 2 days to 35 years (mean age, 5.9 years), with 46 patients less than 1 year of age. We performed primary repair in 117 patients; in this group, there have been 28 conduit replacements in 27 patien

BACKGROUND: Severe pulmonary hypertension (PH) is a relative contraindication to left-sided valve replacement given elevated risk of decompensation.
CASE SUMMARY: A 64-year-old woman with prior atrioventricular canal repair, bioprosthetic mitral valve replacement, and suprasystemic group II PH presented with recurrent heart failure admissions in the setting of severe bioprosthetic valve regurgitation. Considered at prohibitive risk for redo surgery, she underwent valve-in-valve transcatheter mitral valve replacement (TMVR) under intracardiac echocardiography (ICE) guidance and conscious sedat

Objectives: To evaluate the feasibility and preliminary clinical results of transcatheter pulmonary valve replacement (TPVR) with the domestically-produced balloon-expandable Prizvalve system. Methods: This is a prospective single-center observational study. Patients with postoperative right ventricular outflow tract (RVOT) dysfunction, who were admitted to West China Hospital of Sichuan University from September 2021 to March 2023 and deemed anatomically suitable for TPVR with balloon-expandable valve, were included. Clinical, imaging, procedural and follow-up data were analyzed. The immediat…

BACKGROUND: Quadricuspid pulmonary valve (QPV) is a rare congenital anomaly. Simple QPV had been mainly diagnosed at the time of autopsy before 2000, and the frequency rates of QPV are approximately 0.02%-0.41%. QPV was initially diagnosed using transthoracic echocardiography (TTE) after 2000 and with contrast computed tomography (CT) or cardiac magnetic resonance imaging (CMR) after 2009. Obtaining the cross-sectional view of the pulmonary valve using TTE is difficult. We aimed to review the papers regarding the incidence, embryology, diagnosis, associated congenital heart anomalies, and prog…

OBJECTIVES: We report our experience in simultaneously implanting multiple stents and valves mounted on a single balloon before and during transcatheter pulmonary valve placement.
BACKGROUND: Heterogeneity and complexity of the right ventricular outflow tract (RVOT) may complicate stent deployment when preparing a landing zone for transcatheter pulmonary valve implantation.
METHODS: Retrospective analysis of patients from Children's Hospital of Colorado, USA; and Oslo University Hospital, Norway, undergoing transcatheter pulmonary valve replacement that had at least two stents mounted on a s…

Transfemoral and right transjugular approach has been described for transcatheter pulmonary valve implantation (TPVI). However, the use of left transjugular approach through the left superior vena cava to coronary sinus has not been well described. We present a 22-year-old male with Tetralogy of Fallot status post transannular patch, who underwent successful TPVI using the Sapien valve, preceded by pre-stenting of native right ventricular outflow tract. All the transcatheter intervention was performed through the left superior vena cava to coronary sinus.

BACKGROUND: Pulmonary valve replacement (PVR) is often performed in patients with repaired tetralogy of Fallot (TOF). Concomitant tricuspid valvuloplasty (TVP) in those with tricuspid regurgitation (TR) at the time of PVR is still controversial.
METHOD: We retrospectively reviewed clinical records of patients who underwent PVR between 2001 and 2012. We analyzed the impact of concomitant TVP on the tricuspid valve function and right ventricle function and size in mid-term.
RESULTS: 119 patients with mild to moderate TR at the time of PVR were enrolled. 33 patients underwent concomitant TVP (T…
