Diagnostic imaging and pathogenesis of the traumatic intratumoural haemorrhage of schwannoma causing acute high radial nerve palsy: case report.

We describe the first report of haemorrhaged schwannoma presenting with acute high radial nerve palsy after traumatic injury of the upper arm. Anticoagulant therapy may induce intratumoural haemorrhage, resulting in symptomatic increases in size within the limited space of the upper arm. The radiological, surgical and pathological findings are discussed.

Intramedullary nailing of humeral shaft fractures.

The development of interlocking humeral nail systems has greatly broadened the indications for nailing of humeral shaft fracture. Rotational control is better than with earlier nail systems, and most nails have an oblong distal hole that allows axial loading of the fracture site with muscle contraction. When nailing is done with closed technique, loss of the fracture hematoma and periosteal stripping are avoided. Even when open reduction is required, periosteal stripping can be kept to a minimum. Surgical wounds are smaller, even when open reduction is necessary, and when closed nailing is don

Isolated Pectoralis Major and Tricep Atrophy Secondary to Neuralgic Amyotrophy.

Neuralgic amyotrophy (NA), or Parsonage-Turner syndrome, describes a spectrum of relatively rare peripheral neuropathies characterized by acute pain followed by patchy, multifocal sensory disturbances, weakness, and atrophy, predominantly in the upper extremity. While typically affecting the upper brachial plexus, NA can also present with selective involvement of distal nerve branches. This case report details a unique presentation of NA in a 39-year-old male bodybuilder who developed isolated weakness and atrophy of the left pectoralis major and a single head of the left triceps following a p

Focal posterior interosseous neuropathy in the presence of hereditary motor and sensory neuropathy, type I.

A 30-year-old male with hereditary motor and sensory neuropathy, type I (HMSN I), presented with asymmetric weakness of finger extension and radial deviation with left wrist extension, previously felt to be a manifestation of the peripheral neuropathy. Nerve conduction studies confirmed HMSN I; however, needle EMG revealed marked, ongoing axonal loss in muscles innervated by the left posterior interosseous nerve (PIN) only. At surgery there was focal fusiform swelling in the PIN at exit from the supinator muscle, compatible with localized hypertrophic neuropathy, which has not been reported be

Upper extremity neuropathies.

This chapter covers the electrodiagnostic (EDX) evaluation of upper extremity nerves and the brachial plexus. Carpal tunnel syndrome is the most common peripheral nerve disorder of the upper extremity. A number of techniques are used but there is no gold standard approach for its diagnosis. Needle EMG aids in the differentiation of proximal and distal median neuropathies. Ulnar neuropathy at the elbow and ulnar neuropathy at or distal to the wrist can be distinguished by EDX techniques. Radial neuropathy at the spiral groove has a specific EDX pattern. EDX assessment of proximal upper extremit…

A New Rerouting Technique for the Extensor Pollicis Longus in Palliative Treatment for Wrist and Finger Extension Paralysis Resulting From Radial Nerve and C5C6C7 Root Injury.

Wrist and finger extension paralysis is a consequence of an injury to the radial nerve or the C5C6C7 roots. Despite these 2 different levels of lesions, palliative treatment for this type of paralysis depends on the same tendon transfers. A large majority of the patients are able to compensate for a deficiency of the extension of the wrist and fingers. However, a deficiency in the opening of the first web space, which could be responsible for transfers to the abductor pollicis longus, the extensor pollicis brevis, and the extensor pollicis longus (EPL), frequently exists. The aim of this work

Unusual radial nerve injury by a locking compression plate for humerus fracture.

The management of primary and secondary radial nerve palsy associated with humeral shaft fractures is still controversial. Radial nerve function is likely to return spontaneously after primary as well as secondary radial nerve palsy in the absence of any level of neurotmesis. Identification and protection of the radial nerve during surgery may prevent secondary nerve palsy, but is not always performed and depends on the location of the fracture, and the experience and preference of the surgeon. We report a case of a healthy 40-year-old woman, referred to our hospital with a complete radial ner

Median to radial nerve transfer to restore wrist and finger extension: technical nuances.

BACKGROUND: Traditional methods for restoring finger and wrist extension following radial nerve palsy include interposition nerve grafting or tendon transfers. We have described the utilization of distal nerve transfers for the restoration of radial nerve function in the forearm. OBJECTIVE: We review the neuroanatomy of the forearm and outline the steps required for the implementation of this transfer. METHODS AND RESULTS: We use a step-by-step procedural outline and detailed photographs, line drawings, and video to describe the procedure. CONCLUSION: This approach is technically feasible a

An Acta Orthopaedica educational article: Treatment of adult patients with a humeral shaft fracture.

This educational review outlines the core principles of humeral shaft fracture (HSF) management and is designed for orthopedic trainees, general orthopedic surgeons, emergency physicians, and allied health professionals who participate in the acute or postoperative care of patients with HSFs. The content integrates the authors' expert opinion with the current evidence. Humeral shaft fractures account for 1-3% of adult fractures, most often resulting from low-energy falls in older adults or high-energy trauma in younger patients. Although open fractures and neurovascular injuries are rare, prim

A Rare Occurrence of Claw Hand and Wrist Drop Deformity in Herpes Zoster.

Our patient, a 37-year-old nondiabetic woman, presented with severe pain, blistering eruptions, and weakness in the right arm. About a month prior to reporting, had felt mild pain over the length of the arm followed by erythematous after 4-5 days in the same distribution. The dermatitis increased over next 10-15 days along with the onset of weakness. In anamnesis, she recalled that she also had intraoral lesions.

Radial Nerve Palsy: Nerve Transfer Versus Tendon Transfer to Restore Function.

BACKGROUND: Radial nerve injuries cause profound disability, and a variety of reconstruction options exist. This study aimed to compare outcomes of tendon transfers versus nerve transfers for the management of isolated radial nerve injuries. METHODS: A retrospective chart review of 30 patients with isolated radial nerve injuries treated with tendon transfers and 16 patients managed with nerve transfers was performed. Fifteen of the 16 patients treated with nerve transfer had concomitant pronator teres to extensor carpi radialis brevis tendon transfer for wrist extension. Preoperative and post

Neuromuscular Choristoma Variant in the Forearm Presenting as a Posterior Interosseous Nerve Palsy: A Case Report.

CASE: A 14-year-old boy presented with an 18-month history of progressive left wrist drop. Magnetic resonance imaging studies were concerning for mass infiltration of the posterior interosseous nerve (PIN). Surgical resection and pathology confirmed a variant of neuromuscular choristoma (NMC), infiltrated with and surrounded by proliferation of smooth muscle, rather than skeletal muscle. Given the wide-spanning nerve involvement, the patient underwent tendon transfers at the time of surgical resection. CONCLUSION: We report here the first case report of a NMC in the PIN and the first patholog

Differences in nerve excitability properties across upper limb sensory and motor axons.

OBJECTIVE: The excitability of motor and sensory axons of the main upper limb nerves were compared to characterise the differences between nerves and provide a guide for future studies in human diseases with median neuropathy at the wrist. METHODS: Axonal excitability studies were undertaken on median and ulnar motor (APB and ADM) and sensory axons (D2 and D5) and the superficial radial axons (D1) using a threshold tracking technique. RESULTS: Compared to the median, ulnar motor axons had reduced early depolarising threshold electrotonus (TEd40(10-20 ms) p = 0.02) and superexcitability (p =

Median nerve fascicle to posterior interosseous nerve transfer to restore finger extension.

BACKGROUND: A 19-year-old man who developed wrist drop due to a radial nerve injury secondary to a right humeral shaft fracture sustained during a motor vehicle accident that required open reduction and internal fixation, complicated by infection requiring plate removal. Electromyography (EMG) showed severe radial neuropathy distal to the innervation of the triceps with prolonged latency and decreased amplitude. The patient's history, physical examination, and EMG findings were convincing for traumatic radial neuropathy. We therefore recommended a two-stage radial nerve surgery. Stage I, a med…

Nerve injuries about the elbow.

The ulnar, radial, median, medial antebrachial cutaneous, and lateral antebrachial cutaneous nerves are subject to traction and compression in athletes who place forceful, repetitive stresses across their elbow joint. Throwing athletes are at greatest risk, and cubital tunnel syndrome (involving the ulnar nerve) is clearly the most common neuropathy about the elbow. The anatomy and innervation pattern of the nerve involved determines the characteristic of the neuropathy syndrome. The most important parts of the work-up are the history and physical examination as electrodiagnostic testing and i

Acute intermittent porphyria with peripheral neuropathy: a follow-up study after hematin treatment.

We report a patient with acute intermittent porphyria who presented with progressive motor neuropathy, particularly in the upper limbs. The electrophysiological studies showed an asymmetric motor neuropathy with a prominent involvement of both the radial and left peroneal nerves. During the 1-year follow-up period, 6 courses of hematin infusion, with 150 mg daily for 4 consecutive days every month, were administrated. The motor neuropathy showed a steady and gradual improvement following the hematin treatment. Molecular analysis of the porphobilinogen deaminase gene revealed a short segment de
