This study analyzes 4,095 proactive safety inspection records obtained from a large dispatching centre by utilising the HFACS framework. These proactive safety inspection records offer comprehensive documentation of incidents, capturing major accidents and numerous minor discrepancies and lapses that often go unnoticed in accident reports. The analysis revealed that most incidents were attributed to unsafe actions, primarily skill-based errors and poor decision-making. This study investigated train air conditioning filters, interior ventilation systems, tunnel environments and platform air quality as factors affecting the concentrations of airborne particles inside trains and provides information on the exposure of passengers, train drivers and service staff to particles. Particle sampling was done inside the passenger cabin, the driver cabin and the service staff cabin during on-board measurement campaigns in 2016 and 2017. Eight cases of primary adenocarcinoma of the nasal cavity associated with wood dust exposure are reported. The patients include seven men and one woman between the ages of 31 and 81 years.
