Eye movements in a familial vestibulocerebellar disorder.

Eye movement abnormalities consisting of poor or absent smooth pursuit and vestibulo-ocular reflex suppression, gaze-paretic and rebound nystagmus, slow build-up of optokinetic nystagmus, mildly hyperactive vestibulo-ocular reflex, and a high incidence of strabismus were inherited in an autosomal dominant fashion in 10 members of a non-consanguineous English caucasian family. The onset was in early childhood, but was not congenital. In 7 cases there was no tremor, dizziness, consistent ataxia, or other cerebellar signs that are often associated with these ocular motor deficits, and apart from

Incremental Velocity Error as a New Treatment in Vestibular Rehabilitation (INVENT VPT) Trial: study protocol for a randomized controlled crossover trial.

BACKGROUND: A clinical pattern of damage to the auditory, visual, and vestibular sensorimotor systems, known as multi-sensory impairment, affects roughly 2% of the US population each year. Within the population of US military service members exposed to mild traumatic brain injury (mTBI), 15-44% will develop multi-sensory impairment following a mild traumatic brain injury. In the US civilian population, multi-sensory impairment-related symptoms are also a common sequela of damage to the vestibular system and affect ~ 300-500/100,000 population. Vestibular rehabilitation is recognized as a criti

Vestibular function in periodic alternating nystagmus.

The vestibulo-ocular reflex of 4 patients with periodic alternating nystagmus (PAN) was studied in detail. Rotational testing was used to investigate the horizontal semicircular canal-ocular reflex, canal-otolith interaction, and the dynamic otolith-ocular reflex. Results indicated abnormal gain and phase of the horizontal semicircular canal-ocular reflex during sinusoidal rotation and a variable rate of decay of postrotatory responses. Each patient had abnormal canal-otolith interaction. An enlarged modulation component of the dynamic otolith-ocular response was observed in each patient. This

Acute cerebral compression caused by an epidural hematoma associated with cryptococcosis in a dog.

OBJECTIVE: To describe acute cerebral compression caused by an epidural hematoma (EDH) in a dog with cryptococcal infection. CASE SUMMARY: An 18-month-old neutered male Neapolitan Mastiff was found comatose with no preceding clinical signs apart from a mild forelimb lameness. A CT scan of the head revealed a lesion within the right caudal nasal cavity that traversed the cribriform plate in addition to a right epidural lesion resulting in compression of the right cerebrum. Assessment of brain death was made based on the presence of coma, apnea, and absence of brain stem reflexes and included a

Cerebellar ataxia with neuropathy and vestibular areflexia syndrome (CANVAS) - a case report and review of literature.

CANVAS (cerebellar ataxia with neuropathy and vestibular areflexia syndrome) is a rare neurological syndrome of unknown etiology. The main clinical features include bilateral vestibulopathy, cerebellar ataxia and sensory neuropathy. An abnormal visually enhanced vestibulo-ocular reflex is the hallmark of the disease. We present a case of 58-year-old male patient who has demonstrated gait disturbance, imbalance and paresthesia of feet for 2 years. On examination ataxia of gait, diminished knee and ankle reflexes, absence of plantar reflexes, fasciculations of thigh muscles, gaze-evoked downbeat

Crouzon Syndrome: Relationship of Eye Movements to Pattern Strabismus.

PURPOSE: To characterize conjugate eye movements in Crouzon syndrome (CS) patients with and without strabismus. METHODS: Smooth pursuit, saccades, horizontal optokinetic nystagmus (OKN), and horizontal vestibulo-ocular reflex (VOR) were recorded using binocular video-oculography (VOG) in 10 children with CS (5 orthotropic, 5 strabismic) and 12 age-matched controls. Hess-Lancaster plots were generated from Orbit 1.8 using rectus muscle pulley locations from computed tomography imaging. Two-dimensional eye scan paths from VOG recordings were compared with the Hess-Lancaster plots. RESULTS: Tar

Disorders of head and eye movements in children.

Disturbances of the normal relationship of head and eye movements occur in several pathological conditions, including disorders of rapid eye movements and various forms of congenital nystagmus. In disorders of rapid eye movements the abnormal head movements may be a learned response using various techniques to improve eye velocity and amplitude. In congenital nystagmus with head shaking, the head movements may be compensatory, of common genesis, or suppressive. The vestibulo-ocular reflex may be manipulated in certain circumstances to achieve eye stability or to increase eye movement velocity.

Ocular and oculomotor signs in Joubert syndrome.

A number of oculomotor defects have been described in Joubert syndrome. This study systematically examined the oculomotor systems of 13 individuals previously diagnosed with Joubert syndrome. Twelve had the characteristic "molar tooth sign" seen on magnetic resonance imaging scan. In all individuals, smooth pursuit gain and vestibulo-ocular reflex cancellation were decreased in the horizontal and vertical directions and volitional saccades, when generated, were hypometric. We believe that these defects arise from a disorder in the posterior cerebellar vermis. All individuals also had partial t

New understanding on the contribution of the central otolithic system to eye movement and skew deviation.

The otolith organs consist of the utricle and saccule. The utricle mediates the utriculo-ocular reflex by detecting horizontal head translation and static head tilt. Skew deviation is a vertical strabismus caused by imbalance of the utriculo-ocular reflex pathway and is commonly caused by lesions in the brainstem or cerebellum. It is associated with abnormal utriculo-ocular reflexes including asymmetric reduction of the translational vestibulo-ocular and ocular counterroll responses. Skew deviation is also associated with head position-dependent changes in ocular torsion and vertical strabismu

Supranuclear eye movement dysfunction in mitochondrial myopathy with tRNA(LEU) mutation.

A patient with multiple neurological deficits and biopsy-proven mitochondrial myopathy with mutation of tRNA(LEU) at nucleotide 3243 was referred for eye movement evaluation. He had restricted range of voluntary motions in all directions and full range of eye movements on passive rotation of head while fixating a visual target. Eye movement recordings revealed decreased horizontal and vertical saccadic velocities and markedly decreased smooth pursuit gain in both directions. The vestibulo-ocular reflex showed gain abnormalities with many saccadic intrusions on the smooth reflex response. His b

Effects of prochlorperazine on normal vestibular ocular and perceptual responses: a randomised, double-blind, crossover, placebo-controlled study.

BACKGROUND: The present study investigated whether prochlorperazine affects vestibulo-ocular reflex (VOR) and vestibulo-perceptual function. METHODS: We studied 12 healthy naïve subjects 3 h after a single dose of oral prochlorperazine 5 mg in a randomised, placebo-controlled, double-blind, crossover study in healthy young subjects. Two rotational tests in yaw were used: (1) a threshold task investigating perceptual motion detection and nystagmic thresholds (acceleration steps of 0.5°/s(2)) and (2) suprathreshold responses to velocity steps of 90°/s in which vestibulo-ocular and vestibuloperc

Acquired nystagmus.

Nystagmus is the repetitive to and fro movement of the eyes, which may be physiological or pathological. The movements can be horizontal, vertical, torsional or a combination of these movements. It starts by a slow movement of the eye away from the visual target. The second movement brings the eye back to the visual target. If this second movement is quick, the nystagmus is called jerk nystagmus. If the second movement is slow, the nystagmus is said to be pendular. Maintaining steady gaze is dependent upon visual fixation, the vestibulo-ocular reflex and the gaze-holding neural integrator syst

Cerebellar ataxia, neuropathy, vestibular areflexia syndrome (CANVAS): a review of the clinical features and video-oculographic diagnosis.

The association of bilateral vestibulopathy with cerebellar ataxia was first reported in 1991 and delineated as a distinct syndrome with a characteristic and measurable clinical sign--an absent visually enhanced vestibulo-ocular reflex--in 2004. We reviewed 27 patients with this syndrome and show that a non-length-dependent sensory deficit with absent sensory nerve action potentials is an integral component of this syndrome, which we now call "cerebellar ataxia with neuropathy and bilateral vestibular areflexia syndrome" (CANVAS). All patients had brain MRI and 22/27 had evidence of cerebellar

Ocular motor apraxia after sequential bilateral striatal infarctions.

Ocular motor apraxia has been reported in bilateral frontoparietal lesions. We report a case of ocular motor apraxia after bilateral striatal infarctions. The patient had impaired voluntary saccades and smooth pursuits in the vertical and horizontal planes with an intact vestibulo-ocular reflex. Magnetic resonance imaging showed an old left putaminal infarction and an acute infarction in the right caudoputaminal area. This case demonstrates that ocular motor apraxia may occur in bilateral basal ganglia lesions.

Primary position upbeat nystagmus. Clinicopathologic study of four patients.

We report an electro-oculographic study (EOG) of upbeat nystagmus and neuroradiological correlations in 4 patients and neuropathological findings in 1 patient. All 4 patients revealed responsible lesions in the lower pontine tegmentum. The EOG data suggest that our patients had a deficit in vertical smooth eye movement balance. The nystagmus stopped or reversed direction during convergence or changes of head position. These EOG findings might be caused by vertical imbalance in the otolithic ocular reflex, superimposed on an imbalanced vestibulo-ocular reflex (VOR), secondary to a damage to the

Can a structural leg length discrepancy contribute to persistent concussion symptoms? A case report.

In the past several years, concussions and post-concussion syndrome (PCS) have become more commonly recognized conditions. However, with limited physiological explanation for post-concussion syndrome, there is also limited evidence supporting effective treatment. The vestibular system plays a role in postural reflexes and coordinated eye and cervical spine movements and is often disrupted in patients with prolonged concussion symptoms. This disruption has contributed to some of the most debilitating symptoms in PCS patients including dizziness, nausea, and balance deficits. Ongoing, post-concu

[Combined lateral gaze palsy with unilateral internuclear ophthalmoplegia in high mesencephalic-pontine hemorrhage. Report of a case].

The authors report a case of right midbrain and high pons haemorrhage with complete external ophthalmoplegia, except bilateral convergence, voluntary and automatic-reflex abduction of the left eye and adduction of this eye with vestibulo-ocular reflex ("one-and-a-half syndrome"). There is voluntary-automatic dissociation of the eyelids motricity and the vertical gaze. Still, there is sensitive-motor hemiplegia at left and asterixis.

Subclinical vestibular dysfunction in migraine patients: a preliminary study of ocular and rectified cervical vestibular evoked myogenic potentials.

BACKGROUND: Many studies have identified various vestibular symptoms and laboratory abnormalities in migraineurs. Although the vestibular tests may be abnormal, the changes may exist without vestibular symptoms. To date, vestibular-evoked myogenic potential (VEMP) has been the easiest and simplest test for measuring vestibular function in clinical practice. Cervical VEMP (cVEMP) represents a vestibulo-collic reflex, whereas ocular VEMP (oVEMP) reflects a vestibulo-ocular pathway. Therefore, we determined whether ocular and rectified cervical VEMPs differed in patients with migraine or tension
