Reclaiming 'abnormal' embryos after preimplantation genetic testing for aneuploidy: patients' perspectives on transferring embryos against prior institutional advice.

STUDY QUESTION: How do patients make sense of and justify the transfer of embryos labelled 'abnormal' by preimplantation genetic testing for aneuploidy (PGT-A)? SUMMARY ANSWER: Patients described a process of unlearning the authority of genetic testing and reframing their decisions as morally reasoned acts of reproductive agency-a reclaiming of possibility amid biomedical exclusion. WHAT IS KNOWN ALREADY: PGT-A is widely used in IVF, although its clinical utility and interpretation remain debated, and its claimed benefit on cumulative live birth rates unproven. Many clinics refuse transfer o

Promoting physical activity in rheumatoid arthritis through a physiotherapist led behaviour change-based intervention (PIPPRA): a feasibility randomised trial.

Physical activity (PA) is recommended as a key component in the management of people with rheumatoid arthritis (RA). The objective of this study was to examine the feasibility of a physiotherapist led, behaviour change (BC) theory-informed, intervention to promote PA in people with RA who have low levels of current PA. A feasibility randomised trial (ClinicalTrials.gov NCT03644160) of people with RA over 18 years recruited from outpatient rheumatology clinics and classified as insufficiently physically active using the Godin-Shephard Leisure Time Physical Activity Questionnaire. Attrition and bias in the MRC cognitive function and ageing study: an epidemiological investigation.

BACKGROUND: Any hypothesis in longitudinal studies may be affected by attrition and poor response rates. The MRC Cognitive Function and Ageing study (MRC CFAS) is a population based longitudinal study in five centres with identical methodology in England and Wales each recruiting approximately 2,500 individuals. This paper aims to identify potential biases in the two-year follow-up interviews. METHODS: Initial non-response: Those not in the baseline interviews were compared in terms of mortality to those who were in the baseline interviews at the time of the second wave interviews (1993-1996)

Evaluation of community compliance with annual ivermectin treatment of onchocerciasis in Patigi, Nigeria.

A low community acceptance and compliance with annual ivermectin treatment was recorded in Patigi, Nigeria where ivermectin is being distributed centrally in designated centres for the fifth time. The average community acceptance rate (ACAR) for the treatment period was 53.48% while the community compliance rate (CCR) of 1.9% of the total subjects interviewed was recorded. Absenteeism from treatment was a major reason for non-treatment (average, 34.6%). Inadequate and poor mobilisation was identified as a major reason for the low ivermectin acceptance and compliance with treatment in this comm

COVID-19 vaccine hesitancy in a representative working-age population in France: a survey experiment based on vaccine characteristics.

BACKGROUND: Opinion polls on vaccination intentions suggest that COVID-19 vaccine hesitancy is increasing worldwide; however, the usefulness of opinion polls to prepare mass vaccination campaigns for specific new vaccines and to estimate acceptance in a country's population is limited. We therefore aimed to assess the effects of vaccine characteristics, information on herd immunity, and general practitioner (GP) recommendation on vaccine hesitancy in a representative working-age population in France. METHODS: In this survey experiment, adults aged 18-64 years residing in France, with no histo

Opt-Out HIV Testing of Inmates in North Carolina Prisons: Factors Associated with not Wanting a Test and not Knowing They Were Tested.

Opt-out HIV testing is recommended for correctional settings but may occur without inmates' knowledge or against their wishes. Through surveying inmates receiving opt-out testing in a large prison system, we estimated the proportion unaware of being tested or not wanting a test, and associations [prevalence ratios (PRs)] with inmate characteristics. Of 871 tested, 11.8 % were unknowingly tested and 10.8 % had unwanted tests. Not attending an educational HIV course [PR = 2.34, 95 % confidence interval (CI) 1.47-3.74], lower HIV knowledge (PR = 0.95, 95 % CI 0.91-0.98), and thinking testing is n

Locked out of the ward: Workflow-based care refusal for people living with HIV in China's general hospitals.

BACKGROUND: With the scale-up of antiretroviral therapy (ART), people living with HIV (PLWH) increasingly require non-HIV inpatient and procedural care in general hospitals, yet access remains precarious. AIMS: To examine how refusals of non-HIV care for PLWH unfold within routine general-hospital workflows. METHODS: We conducted an interpretive qualitative study in three provincial capitals in China (Changsha, Guangzhou, and Nanning) between May 2024 and October 2025. We interviewed 58 PLWH about one index non-HIV care-seeking episode each (80 interviews, including 22 follow-ups) and 16 key

Perioperative telemonitoring of older adults with cancer: Can we connect them all?

OBJECTIVES: Although the increasing cancer incidence in older patients is widely recognised, older patients remain underrepresented in clinical cancer trials and eHealth studies. The aim of this research is to identify technological and patient-related barriers to inclusion of this population in a clinical eHealth study. MATERIAL AND METHODS: This is a retrospective analysis of a prospective cohort study with older patients (≥ 65 years) undergoing cancer-related surgery, who were identified for a perioperative telemonitoring study. Reasons for ineligibility and refusal had been prospectively

A custom-made mandibular repositioning device for obstructive sleep apnoea-hypopnoea syndrome: the ORCADES study.

BACKGROUND: Mandibular repositioning devices (MRDs) are usually recommended as the first therapy option in patients with mild-to-moderate obstructive sleep apnoea (OSA). However, data on the long-term efficacy of MRDs are limited, not only in OSA patients who are noncompliant with continuous positive airway pressure (CPAP) but also in those with more severe OSA. The ORCADES study aimed to prospectively determine the long-term efficacy and tolerability of two custom-made Narval(™) MRDs for obstructive sleep apnoea-hypopnoea syndrome (OSAHS) patients. The interim 3- to 6-month data are reported.

[Results of a preliminary study on the various factors of HIV infection and sexually transmitted disease in rural west Africa].

A pilot study has been carried out in two Senegalese rural communities in January-February 1996 in order to evaluate the feasibility of a study that is being launched in several rural sites in West Africa. This study aims to shed light on the factors of HIV infection and other sexually transmitted diseases. A sample of 100 adults were asked to answer a questionnaire on at risk behaviors for these infections. Few of them have refused to answer the questionnaire: 9% (6/66) in a first site and 3% (1/34) in the other site. Interviewers', remarks made after this pilot study showed that sex and age

A qualitative study on barriers and facilitators of COVID-19 vaccination during pregnancy among pregnant women in Malaysia.

BACKGROUND: COVID-19 vaccination is crucial for pregnant women to protect against severe complications during pregnancy. However, vaccine hesitancy and refusal can affect acceptance rates among this vulnerable group, posing risks to both maternal and fetal health. This research was conducted to explore the barriers and facilitators influencing COVID-19 vaccination among Malaysian pregnant women. METHODS: This qualitative study adopted a hermeneutic phenomenological design. Nineteen pregnant women receiving antenatal care at three clinics in Kelantan between February 2024 and June 2024 were pu

Sex, gender, and sexuality and the oral health workforce in Africa.

This study explores how sex, gender, and sexuality intersect with Nigeria's oral health workforce crisis, highlighting structural barriers that hinder inclusion and equitable care. Nigeria faces an acute shortage of dental professionals-just 0.03 dentists per 100,000 people-amid entrenched inequities shaped by colonial, patriarchal, and heteronormative systems. Originally designed to center the voices of sexual and gender minority (SGM) oral health professionals through interviews, the study encountered complete refusal to participate. This collective silence redirected the research focus, ref…
