Circulating anti alveolar basement membrane antibodies have been detected by indirect immunofluorescence on lung rat in two out of five children presenting with pulmonary hemosiderosis. The two cases are reported and seem to be consistent with an auto-immune disease due to anti-basement membrane antibodies. Authors outline the presence of anti-reticulin antibodies in the two cases reported and the high level of IgE which seems a frequent finding in pulmonary hemosiderosis.

Twenty-five children and adults with concomitant coeliac disease and selective IgA deficiency are described. IgG-class reticulin antibodies were positive in 94%. The clinical course of coeliac disease did not differ from that of patients with normal serum IgA level. Patients with IgA deficiency also had other concomitant diseases, especially autoimmune diseases. Patients with selective IgA deficiency have at least a tenfold risk of coeliac disease compared with the population in general.

Morphologic and chromium release studies demonstrated degeneration in monolayer cultures of endothelial cells exposed to the anabolic steroidal hormone oxymetholone. The concentrations approximated those occurring transiently in the hepatic sinusoidal blood after oral administration of the drug during treatment for aplastic anemia. It is suggested that damage to sinusoidal endothelium, with impairment of its ability to maintain sinusoidal collagenous reticulin, is a mechanism by which oxymetholone can cause peliosis hepatis in these patients.

The opium poppy, Papaver somniferum L., and its narcotic and analgesic alkaloids, have an ancient history of use (and abuse) by humankind. A recent article by Allen and co-workers describes the metabolic engineering of morphine biosynthesis to block morphine formation and accumulate a potentially valuable pathway intermediate, (S)-reticuline. This work highlights the potential for modifying the production of pharmaceuticals in plants, but also raises questions about the complex regulation of biosynthetic pathways.

Four cases of nodular lipogranulomas in the breast, the arms and in the omentum are discussed. Nodular lipogranulomas develop from single fat cells, the fat of which is enclosed and phagocytized by lipophages. The small areas grow and become confluent into larger granulomas, which become scars by the formation of reticulin fibers. The rosette-like pattern, the primary degenerative alteration of the fat cells and the progression of the lesion all suggest insufficient blood supply. Inflammation is secondary and appears only occasionally.

We report a case of fulminant hepatic necrosis occurring in the setting of acetaminophen overdose and chronic alcohol use. The pathology of severe acute toxicity is documented with a surgical biopsy. Follow-up liver biopsy 4 yr later documents hepatic regeneration without cirrhosis. Maintenance of the hepatic reticulin framework in the face of acute hepatic necrosis was well documented, and may be a useful prognostic feature indicating a potential for the maintenance of normal hepatic architecture following regeneration.

8 patients with clinical and haematological features suggestive of hairy cell leukaemia are described. During their initial illness morphologic and cytochemical evidence of hairy cell leukaemia were lacking in all but 2 patients. A common highly characteristic histopathologic finding was reticulin fibrosis in the bone marrow, present in all patients at first presentation. The term spongy lymphoid myelofibrosis is proposed for such cases, which may represent a variant type of hairy cell leukaemia or an early stage of the disease.

Erythroblastic sarcoma (ES) is an extremely rare extramedullary solid tumor of immature erythroid cells, with around a dozen human cases to our knowledge presented so far in the english literature, none of the cases presenting as pleural effusion. Myelofibrosis is observed in 1/5 of the cases of C.M.L. It consists of reticulin fibers with few collagen and no osteosclerosis. It involves signs which usually indicate the extension of the myelosis to other organs and other types of cells: hepatosplenomegaly, erythroblastosis, thrombocytemia. Its prognosis is always bad.
